RESUMO

Fasting pancreatic glucagon was observed in Jamaican infants during malnutrition and subsequent recovery. Rehabilitation in two groups of children with isocaloric diets rich either in carbohydrate or fat produced no differences in the rate of weight gain. During malnutrition, plasma pancreatic glucagon concentration was 104 +/- 11 (n = 20) pg/ml (mean +/- S.E.) significantly lower than during recovery when the maximum value was 180 +/- 24 (n = 13) pg/ml during the later recovery phase. After clinical recovery glucagon levels declined to 127 +/- 13 (n = 15) pg/ml. Plasma insulin followed a similar pattern, increasing significantly during catch-up growth and declining after recovery. Slower rates of growth were associated with the simultaneous decline in the concentrations of both hormones after clinical recovery.

A group of children who had been treated for protein-energy malnutrition with a high-energy, high-faty diet, presented very low levels of fasting serum triglycerides (50.8 +/- 5.9 mg/dl). When the diet was changed to a high-energy, low-fat formula, these levels increased rapidly to normal values (104.9 +/- 19.2 mg/dl). Reversing the dietary change produced the opposite effect within 2 days. This effect was also studied in 14 normal adults, who received a high-fat diet ad libitum during 5 days. Meals were prepared from common foodstuffs with a high fat content. The hypotriglyceridemic effect occurred in all volunteers. A close relationship was observed between the initial triglyceride concentration and the decrement produced by the high-fat diet (r = 0.97), which did not affect serum cholesterol levels. No consistent response of serum free-fatty acids was detected, while serum proteins remained within normal values. Despite the rather high energy intake, most subjects lost weight during the experimental period. The level of fasting serum triglycerides of the adult volunteers was negatively correlated to the fat content of their self-selected diets. These results are discussed in relation to current ideas about fat requirements.

RESUMO

1. Total body water (TBW) was measured using tritiated water in sixty-five children. The measurements were distributed throughout rehabilitation in order to define the effect of changing energy intakes. 2. Oedematous children had a high TBW which decreased to the normal range during loss of oedema providing they were not receiving more than maintenance amounts of energy during this period. 3. Marasmic children who had not received greater than maintenance amounts of energy had a normal TBW. 4. Treatment with a high-energy diet was associated with an initial increase in TBW. 5. The possible mechanisms for this phenomenon are discussed.
