RESUMO

Health services in Saudi Arabia have developed enormously over the last two decades, as evidenced by the availability of health facilities throughout all parts of the vast Kingdom. The Saudi Ministry of Health (MOH) provides over 60% of these services while the rest are shared among other government agencies and the private sector. A series of development plans in Saudi Arabia have established the infra-structure for the expansion of curative services all over the country. Rapid development in medical education and the training of future Saudi health manpower have also taken place. Future challenges facing the Saudi health system are to be addressed in order to achieve the ambitious goals set by the most recent health development plan. These include the optimum utilization of current health resources with competent health managerial skills, the search for alternative means of financing these services, the maintenance of a balance between curative and preventive services, the expansion of training Saudi health manpower to meet the increasing demand, and the implementation of a comprehensive primary health care program.

A health development project was established in a rural area of Bangladesh that entailed training village health promoters to provide health education and to motivate families to install tubewells and sanitary latrines. Following a 2-y period of project implementation, the authors sought to assess knowledge and practice of mothers and family members about use of safe water and household tubewells. A household survey in the project area was compared with a similar one in a nearby control area. Three-hundred households in each area were selected, and mothers were interviewed with a standard questionnaire. Significantly more mothers in the health development project area (45.7%) used tubewell water for domestic purposes than in the control area (32.8%). However, hygienic practices of mothers were inadequate. Sanitary latrines were present in less than 20% of households in both areas. Approximately 97% of mothers and 78% of adult family members always used household latrines. However, the use of household latrines by children was low (26.7%). There was no statistically significant difference in the use of household latrines between the project and control areas. The results showed an improvement in use of household tubewells in the project area; however, there was no improvement in sanitation practices of families in the project area. Health education alone, without improvement of socioeconomic status, is not effective in changing behavior.

The orientation about Primary Health Care among staff working in the PHC centers was assessed. Staff members numbering 909 were studied. The main criteria for judging orientation were a working knowledge of the definition and elements of PHC in addition to knowledge of the meaning of the word Alma Ata. Differences of this knowledge depending on sex, age, spoken language, type of job, postgraduate experience, previous experience in PHC and previous training in PHC were assessed. The main findings of the study were that the correct definition of PHC was known by only 51.4%, functions of PHC by 62.6%, and what Alma Ata, means in terms of PHC was known by 76.2% of the staff. This knowledge was significantly better in females than males, non-Arabic speaking staff than those who spoke Arabic, General practitioners and nurses than other staff; it was better in those staff who had long postgraduate experience, previous experience or previous training in PHC.In conclusion, the study reveals the current status of awareness of PHC staff of the implications of simple concepts of PHC and points to the importance of the orientation of staff towards these concepts in order to help them practice PHC effectively.

In Saudi Arabia, increased attention is being given to the prevention of illness, communities are being encouraged to take part in the planning, implementation and evaluation of health measures, and the training of health workers is being suitably modified. The progress achieved in primary care, and suggestions for further advance, are outlined below with particular reference to a health centre in the Al-Baha region.

RESUMO

The article reviews the literature on the problem of nutritional disorders in Saudi Arabia. Most of the studies reveal problems of anaemia and a mild to moderate degree of stunting and wasting among preschool children. The causes are, apparently, ignorance and misconception rather than purely economic. Recently, overnutrition has been observed as a problem among the middle and upper class of urban societies. In general, an improvement in the nutritional status of the Saudis has been observed over the last two decades. This is a repercussion of socioeconomic development and improvement in education, dietary habits, and environmental conditions. The time is ripe to create Saudi standards for health and nutrition. These will be used as yardsticks for planning and evaluation of health and nutritional programmes.

RESUMO

Five years have passed since the Alma Ata meeting, and with 17 years remaining in this century, one has to ponder whether the goal of health for all by the year 2000 through primary health care is achievable. This article explores the general situation of health and primary health care, especially in the developing world, where health problems are more acute. The slogan "health for all by the year 2000" has succeeded in creating an awareness of the problems throughout the world. The path toward this goal is not necessarily smooth or paved. Nevertheless, with goodwill and planned actions it can be achieved, though not necessarily by all nations or by the year 2000. This is the biggest challenge facing nations, professionals, international health organizations, and humanity. To achieve this goal they will need to improve and better use national resources, both physical and human. Developing physical resources entails comprehensive and integrated socioeconomic growth, improved health budgeting with multisectorial programming, the use of appropriate technology, and the adoption of scientific managerial processes; but what is even more important is the development of human resources at all levels.

RESUMO

A field work project was conducted in Khulais villages by the Department of Community Medicine, Riyadh Faculty of Medicine, in February 1977. The project involved jointly Riyadh University and the Saudi Arabian Ministry of Health, as well as other organizations. Over a 10-day period, twenty-five students of the fourth-year medical class under the supervision of seven members of the staff studied health problems and rendered selected health services to the community. Project activities included a study of a sample of 280 schoolchildren by clinical examination, anthropometric measurement and selected laboratory investigations; a house-to-house survey; a study of the epidemiology of schistosomiasis and malaria; immunization of pre-school children; a health education programme and an evaluation of the health centre's activities. The main objective was to give the students an appreciation of the need for team-work in approaching community health and help to cultivate their interest in community medicine as an applied discipline.

A total of 292 randomly selected subjects belonging to two indigenous Arab tribes (Harbi and Ghamid) and two immigrant tribes (Mograbi and Mowallad), residents in Western Saudi Arabia, have been tested for genetic variants of six blood groups, four serum proteins, and five red cell enzyme systems. The distribution of the polymorphic systems was different between indigenous and immigrant tribes, and the present Arab population shows a considerable degree of admixture from the surrounding countries, in particular Africa.

638 subjects from six tribes in Western Saudi Arabia were tested for haemoglobin and G-6-PD variants. In the bedouins of Harbi and Sahafi tribes there was no sickling but a low frequency of both G-6-PD deficiency (1.7%) and the Gd A+ variant (1.7%) in Harbi. In Mograbi and Mowallad tribes who immigrated recently from Africa, the frequency of Hb S was 4.1 and 3.5%, G-6-PD deficiency 7.5 and 8.5% and the Gd A+ variant 15.1 and 8.5% respectively, which are higher than in bedouins. In Ghamid tribe 4% had Hb S and there was a low frequency of both G-6-PD deficiency (4.8%) and Gd A+ variant (1.6%). However a new B slow variant is common among the Ghamid (16.1%). Hb S frequency in Zahran tribe was 2.6%. Six of the 12 g-6-PD deficient (screening) were found to be positive on starch gel electrophoresis.

Although our projections are not completely accurate, due to inadequate data and crude methods of approximation, three conclusions are inescapable: 1. The nation should give priority to planning new schools for health administrators and rural health workers, and to generally expanding present schools for paramedicals. 2. Plans should proceed rapidly for the two new medical schools. 3. A Health Manpower plan based on census information and special studies should be developed over the next 2 years. This plan should have input from all concerned ministries. (The Ministry of Health has regarded a proposal from University Associate "a group of Harvard, Johns Hopkins and A.U.B. professors" for technical cooperation in health services planning.)

Of 620 out-patients of Bisha hospital, 110 showed cutaneous leishmaniasis. Leishmania bodies were demonstrated in smears of 73 patients. Both sexes were similarly affected. The highest incidence was among children in the age group one to 10 years. A single lesion was the predominant feature. The face was the commonest site of infection.
