RESUMO

PURPOSE: To evaluate the bond strength of repair systems (Ceramic Repair, Clearfil Repair) to computer-aided design/computer-assisted machining (CAD/CAM) restorative materials (IPS e.max CAD, Vita Suprinity, Vita Enamic, Lava Ultimate). MATERIALS AND METHODS: Thermally aged CAD/CAM restorative material specimens (5000 cycles between 5°C and 55°C) were randomly divided into two groups according to the repair system: Ceramic Repair (37% phosphoric acid + Monobond-S + Heliobond + Tetric N Ceram) or Clearfil Repair (40% phosphoric acid + mixture of Clearfil Porcelain Bond Activator and Clearfil SE Bond Primer + Clearfil SE Bond + Filtek Z250). The resin composite was light-cured on conditioned specimens. All specimens were stored in distilled water at 37°C for 24 hours and then additionally aged for 5000 thermal cycles. The shear bond strength test was performed using a universal testing machine (0.5 mm/min). Two-way ANOVA was used to detect significance differences according to the CAD/CAM material and composite repair system factors. Subgroup analyses were conducted using the least significant difference post-hoc test. RESULTS: The results of two-way ANOVA indicated that bond strength values varied according to the restorative materials (p < 0.05). No significant differences were observed between the CAD/CAM restorative materials (p > 0.05), except in the Vita Suprinity group (p < 0.05). Moreover, no differences were observed between the repair systems. CONCLUSIONS: Both the Clearfil and Ceramic repair systems used in the study allow for successful repairs.

BACKGROUND: Glass fiber-reinforced posts have been preferred frequently because of some physical properties similar to the dentin, chemically bonding to dentin, biocompatibility, and esthetics. OBJECTIVE: This study aimed to evaluate the microleakage and bond strength of glass fiber posts cemented with various adhesive systems on laser-etched root canal walls. METHODS: Roots of 120 human mandibular premolars were divided into two groups for push-out bond strength test and the microleakage test (n = 60). Erbium-doped yttrium-aluminum-garnet (Er:YAG) laser etching of the root canal walls was carried out on half of the specimens in both test groups. The laser-treated and laser-nontreated groups were divided again into three subgroups (n = 10). Glass fiber posts (everStick Post) were luted using three different resin cements: total-etch (Variolink N), self-etch (Panavia F 2.0), and self-adhesive (Rely X Unicem). Three dentin discs were obtained from each root, and the bond strength of the glass fiber posts was measured by push-out tests. The dye penetration method was used to investigate coronal microleakage. In addition, surface treatments and the bonding interfaces were observed using scanning electron microscope. RESULTS: The highest bond strengths were observed for the total-etch and self-adhesive resin cement groups with laser etching (p < 0.05), and the lowest bond strength was obtained for the self-etch resin cement group (p < 0.05). There were no statistically significant differences in terms of microleakage (p > 0.05), except for the self-adhesive resin cement group (p < 0.05). CONCLUSIONS: Er:YAG laser etching enhances the bonding strength of glass fiber posts. In addition, laser etching can reduce microleakage of self-adhesive resin cement.

PURPOSE: This study aimed to evaluate the effect of surface treatments on bond strength of indirect composite material (Tescera Indirect Composite System) to monolithic zirconia (inCoris TZI). MATERIALS AND METHODS: Partially stabilized monolithic zirconia blocks were cut into with 2.0 mm thickness. Sintered zirconia specimens were divided into different surface treatment groups: no treatment (control), sandblasting, glaze layer & hydrofluoric acid application, and sandblasting + glaze layer & hydrofluoric acid application. The indirect composite material was applied to the surface of the monolithic zirconia specimens. Shear bond strength value of each specimen was evaluated after thermocycling. The fractured surface of each specimen was examined with a stereomicroscope and a scanning electron microscope to assess the failure types. The data were analyzed using one-way analysis of variance (ANOVA) and Tukey LSD tests (α=.05). RESULTS: Bond strength was significantly lower in untreated specimens than in sandblasted specimens (P<.05). No difference between the glaze layer and hydrofluoric acid application treated groups were observed. However, bond strength for these groups were significantly higher as compared with the other two groups (P<.05). CONCLUSION: Combined use of glaze layer & hydrofluoric acid application and silanization are reliable for strong and durable bonding between indirect composite material and monolithic zirconia.

The aim of this study was to compare the effect of Er:YAG laser irradiation and adhesive systems on the microtensile bond strength (µTBS) of flowable resin to dentin. The flat dentin surfaces of 30 wisdom molar teeth were randomly divided into 3 groups (n=10): no laser etching (control), laser etching using QSP and MSP modes. After the application of the flowable and the composite resin, and 5,000 thermal aging, the µTBS results were analyzed with two-way analysis of variance and Tukey's honest standard of difference (HSD) tests. There were no significant differences between the µTBS of the self-etch group and the total-etch groups for MSP (p>0.05). MSP mode is a useful pulse mode for dentin surface treatment due to elimination of the acid-etching step.

RESUMO

From the currently available choices, esthetic restorative materials for posterior teeth are limited to composite and ceramic restoration. Ceramic inlays/onlays are reliable solutions for both of these treatments. For successful treatment planning, usable ceramic and adhesive systems should be chosen by the dentist. Since the Federal Drug Administration approval of the erbium: Yttrium-aluminum-garnet (Er:YAG) laser-for caries removal, cavity preparation and the conditioning of tooth substance-in 1997, there have been many reports on the use of this technique in combination with composite resins. In addition, cavity pretreatment with the Er:YAG laser (laser etching) has been proposed as an alternative to acid etching of enamel and dentin. This case report presents the use of the Er:YAG in cavity preparation for composite resin restoration and surface treatment for ceramic onlay restoration of adjacent permanent molars.

The purpose of this study was to evaluate the effect of polishing procedures on the color stability of different types of composites after aging. Forty disk-shaped specimens (Ø10×2 mm) were prepared for each composite resin type (an ormocer, a packable, a nanohybrid, and a microhybrid) for a total of 160 specimens. Each composite group was divided into four subgroups according to polishing method (n=10): control (no finishing and polishing), polishing disk, polishing wheel, and glaze material. Color parameters (L*, a*, and b*) and surface roughness were measured before and after accelerated aging. Of the polishing methods, glazed specimens showed the lowest color change (∆E*), ∆L*, and ∆b* values (p<0.05). Of the composite resins, the microhybrid composite showed the lowest ∆E* value, whereas the ormocer showed the highest (p<0.05). For all composite types, the surface roughness of their control groups decreased after aging (p<0.05). In conclusion, all composite resins showed color changes after accelerated aging, with the use of glaze material resulting in the lowest color change.

The neodymium/yttrium-aluminum-garnet (Nd/YAG) laser has been suggested to repair broken prostheses in the mouth. This study investigated the effects of different dentin thicknesses and air cooling on pulpal temperature rise during laser welding. Three intact human maxillary molars were prepared for full-veneer crown. For each tooth, dentin thicknesses in mesiobuccal cusp was 2, 3, or 4 mm. Twenty dies were duplicated from each of the prepared teeth. For metal copings with 0.5-mm thickness, wax patterns were prepared with dip wax technique directly onto each of dies. All patterns were sprued and invested. The castings were made using a nickel-chromium alloy (Nicromed Premium, Neodontics). A hole with 0.5-mm diameter was prepared on the mesiobuccal cusp of each crown. The Nd/YAG laser (9.85 W; 1 Hz repetition rate; fluence, 1.230 J/cm(2); Fidelis Plus 3, Fotona) was used for welding with or without air cooling (n = 10). The temperature rise was measured in pulpal chamber with a J-type thermocouple wire that was connected to a data logger. Differences between start and highest temperature reading were taken, and temperature rise values were compared using two-way analysis of variance and Tukey's honestly significant difference tests (α = .05). Pulpal temperature rise varied significantly depending on the dentin thickness and air cooling (p < 0.05). The non-air cooling group induced significantly the highest temperature increases. There were no significant differences between 2- and 3-mm dentin thicknesses groups (p > 0.05); however, pulpal temperature rise was the lowest for 4-mm dentin thickness group (p < 0.05). The highest values of thermal increase were found in the pulp chamber (6.8°C) when no air cooling was used in 2-mm dentin thickness group. Laser welding on base metal castings with Nd/YAG laser can be applied with air cooling to avoid temperature rises known to adversely affect pulpal health when dentin thickness is 2 or 3 mm.

OBJECTIVES: The purpose of this study was to evaluate the effect of three provisional cements and two cleaning techniques on the final bond strength of porcelain laminate veneers. METHODS: The occlusal third of the crowns of forty molar teeth were sectioned and embedded in autopolymerizing acrylic resin. Dentin surfaces were polished and specimens were randomly divided into four groups (n=10). Provisional restorations were fabricated and two provisional restorations were cemented onto each tooth. Restorations were fixed with one of three different provisional cements: eugenol-free provisional cement (Cavex), calcium hydroxide (Dycal), and light-cured provisional cement (Tempond Clear). Provisional restorations were removed with either a dental explorer and air-water spray, or a cleaning bur (Opticlean). In the control group, provisional restorations were not used on the surfaces of specimens. IPS Empress 2 ceramic discs were luted with a dual-cured resin cement (Panavia F). Shear bond strength was measured using a universal testing machine. Data were statistically analyzed by ANOVA, Tukey's HSD and Dunnett tests. Surfaces were examined by scanning electronic microscopy. RESULTS: Significant differences were found between the control group and both the light-cured provisional cement groups and the eugenol-free provisional cement-cleaning bur group (P<.05). Groups that had received light-cured provisional cement showed the lowest bond strength values. CONCLUSIONS: Selection of the provisional cement is an important factor in the ultimate bond strength of the final restoration. Calcium hydroxide provisional cement and cleaning with a dental explorer are advisable.

OBJECTIVE: The purpose of this in vitro study was to measure the temperature increase during the polymerization of a composite resin beneath acid-etched or laser-etched dentin discs. BACKGROUND DATA: The irradiation of dentin with an Er:YAG laser may have a positive effect on the thermal conductivity of dentin. This technique has not been studied extensively. MATERIALS AND METHODS: Forty dentin discs (5 mm in diameter and 0.5 or 1 mm in height) were prepared from extracted permanent third molars. These dentin discs were etched with 20% orthophosphoric acid or an Er:YAG laser, and were then placed on an apparatus developed to measure temperature increases. The composite resin was polymerized with a high-intensity quartz tungsten halogen (HQTH) or light-emitting diode unit (LED). The temperature increase was measured under the dentin disc with a J-type thermocouple wire that was connected to a data logger. Five measurements were made for each dentin disc, curing unit, and etching system combination. Differences between the initial and the highest temperature readings were taken, and the five calculated temperature changes were averaged to determine the value of the temperature increase. Statistical analysis was performed with a three-way ANOVA and Tukey HSD tests at a 0.05 level of significance. Further SEM examinations were performed. RESULTS: The temperature increase values varied significantly, depending on etching systems (p < 0.05), dentin thicknesses (p < 0.05), and curing units (p < 0.05). Temperature increases measured beneath laser-etched discs were significantly higher than those for acid-etched dentin discs (p < 0.05). The HQTH unit induced significantly higher temperature increases than the LED unit (p < 0.05). The LED unit induced the lowest temperature change (5.2°C) in the 1-mm, acid-etched dentin group. The HQTH unit induced the highest temperature change (10.4°C) for the 0.5-mm, laser-etched dentin group. CONCLUSION: The risk of heat-induced pulpal damage should be taken into consideration during the polymerization of composite resin after laser etching when the dentin thickness is less than 0.5 mm.

OBJECTIVES: The biomechanical behavior of the superstructure plays an important role in the functional longevity of dental implants. However, information about the influence of framework design on stresses transmitted to the implants and supporting tissues is limited. The purpose of this study was to evaluate the effects of framework designs on stress distribution at the supporting bone and supporting implants. METHODS: In this study, the three-dimensional (3D) finite element stress analysis method was used. Three types of 3D mathematical models simulating three different framework designs for implant-supported 3-unit posterior fixed partial dentures were prepared with supporting structures. Convex (1), concave (2), and conventional (3) pontic framework designs were simulated. A 300-N static vertical occlusal load was applied on the node at the center of occlusal surface of the pontic to calculate the stress distributions. As a second condition, frameworks were directly loaded to evaluate the effect of the framework design clearly. The Solidworks/Cosmosworks structural analysis programs were used for finite element modeling/analysis. RESULTS: The analysis of the von Mises stress values revealed that maximum stress concentrations were located at the loading areas for all models. The pontic side marginal edges of restorations and the necks of implants were other stress concentration regions. There was no clear difference among models when the restorations were loaded at occlusal surfaces. When the veneering porcelain was removed, and load was applied directly to the framework, there was a clear increase in stress concentration with a concave design on supporting implants and bone structure. CONCLUSIONS: The present study showed that the use of a concave design in the pontic frameworks of fixed partial dentures increases the von Mises stress levels on implant abutments and supporting bone structure. However, the veneering porcelain element reduces the effect of the framework and compensates for design weaknesses.

The aim of this study was to evaluate the mineral content of enamel etched at two different power settings with an erbium, chromium:yttrium-scandium-gallium-garnet (Er,Cr:YSGG) laser. Buccal, lingual and mesial or distal surfaces of five premolar teeth were cut, and three enamel slabs were obtained from each tooth. Fifteen enamel specimens were divided into three groups (1 W, 2 W and control) of five specimens each and subjected to Er,Cr:YSGG laser. The mean percentage weights of the five elements [calcium (Ca), potassium (K), magnesium (Mg), sodium (Na) and phosphorus (P)] in each slab were measured by inductively coupled plasma-atomic emission spectrometry (ICP-AES). One way analysis of variance (ANOVA) was used to analyze differences among the groups (1 W, 2 W and control). There were no significant differences among the groups (1 W, 2 W and control) for Ca, K, Mg, Na, or P, or for the Ca/P ratio (P > 0.05). Scanning electron microscopy (SEM) photographs indicated that the surface irregularities increased with increased power setting. Laser treatment did not affect the mean percentage weights of Ca, K, Mg, Na, and P, or the Ca/P ratio, in any group.

MATERIAL AND METHODS: Two implants were placed in a metal model simulating mandibular left second premolar and mandibular left second molar. Thirty standardized 3-unit all-ceramic fixed partial dentures with biconvex, convex or concave pontic designs were fabricated using IPS e.max system (n=10). Afterwards, specimens were centrally loaded on the pontics until failure with a universal testing machine. Results were analyzed by Kruskal-Wallis and Mann-Whitney U tests at 5% significance level. RESULTS: The fracture resistance values of all-ceramic fixed partial dentures designed with biconvex, convex or concave pontics were 349.71, 438.20 and 300.78 N, respectively. There were no statistically significant differences between the fracture resistances of the groups (p>0.05), except for convex and concave groups (p<0.05 and p=0.009, respectively). CONCLUSIONS: Convex design showed the best mechanical properties as demonstrated by the high values of fracture resistance.

Laser etching has an effect on the mineral content of dentin. The aim of this study was to evaluate the mineral content of dentin prepared at three different power settings with an erbium, chromium:yttrium scandium gallium garnet (Er,Cr:YSGG) laser. The enamel of five, lower, wisdom, molar teeth was removed to expose the dentin surface. Four dentin slabs were obtained, then each tooth was randomly divided into four portions (groups 1 W, 2 W, 3 W and control) so that we could evaluate the effect of laser treatment. The Er,Cr:YSGG laser used for the study had a pulse duration of 140 micros, a pulse repetition rate of 20 Hz and a power output range of 0 W to 6 W. Laser energy was delivered through a fiberoptic system to a sapphire tip terminal 6 mm long and 600 microm in diameter, using a non-contact mode. The levels of five elements: magnesium (Mg), phosphorus (P), calcium (Ca), potassium (K), and sodium (Na), in each slab were measured by inductively coupled plasma-atomic emission spectrometry (ICP-AES). There were significant differences between the groups (1 W, 2 W, 3 W and control) for Ca, Mg, Na, P and Ca/P ratio (P<0.05); however, there were no significant differences for K (P=0.43). Laser treatment at 1 W significantly affected the mean percentage weight of all element groups except K. Scanning electron microscopy (SEM) photographs indicated that the surface irregularities increased with increasing power setting.

RESUMO

Facial defects resulting from neoplasms, congenital malformations, or trauma can be restorated with facial prostheses using different materials and retention methods to achieve a lifelike look and function. For the successful result, a lot of factors as harmony, texture, color matching, and blending of tissue interface of prosthesis is important. This clinical report describes treatment using a silicone prosthesis with a mechanical - retentined design for a patient who received a partial rhinectomy. Defects resulting from diseases as squamous cell carcinoma can be managed with the prosthetic rehabilitation so that the patient more comfortably and confidently resumed regular daily activity. Mechanical retention alone was sufficient to retain the prosthesis; thus, prosthetic adhesives were not necessary.
