RESUMO

THE AIM: To compare efficiency of various techniques of anesthesia after Cesarean section on degree ofpain expression in randomized clinical trial. MATERIALS AND METHODS: 120 puerperas aged from 21 till 33 years who were undergone to Caesarean section were studied. Patients were divided into four equal groups by number of observations. In groups 1st and 2nd patients were performed local anesthesia with continuous and bolus anesthetic. In group 3rd were performed bilateral blockade of the cross-space belly. In the 4th groups was used multimodal analgesia system. Each patient during the postoperative period was estimated the severity ofpain on a visual analog scale at rest and movement at 3, 6, 12, 24 and 48 hours. RESULTS: Intensity ofpain after surgery in patients of all groups decreased and reached lows of 48 hours after a Cesarean section. At the same time the 2nd group ofpatients (6,1 ? 1,4) had pain significantly less than that ofpuerperas from the 3rd and the 4th groups (13,2 J 2,4 and 18,0 ? 2,7). Meanwhile, indices offemale patients pain severity from the 1st and 2nd groups of the study were not statistically different. When studying distribution ofpatients on degree ofpostoperative pain it is established that atpuerperas of the 1st and 2nd group pains prevailed weak (47% and 67%) and moderately expressed (50% and 33%). In the 3rd group expression of a pain syndrome in all cases was moderate. In the fourth group the pain syndrome was considerably more expressed (the moderated - 53%; the strong - 4 7%). CONCLUSION: Pain syndrome after Cesarean section using the localanalgesia with continuous or bolus is reduced by 28-35%, which was significantly more than the blockade of the cross-space stomach and systemic analgesia.

RESUMO

Early breast-feeding is a standard of perinatal care currently. After cesarean section it can be possible in case of early mother activation (verticalization). Assessment of perioperative analgesia influence on activation timing was the aim of our research. We included 120 parturient women. It was proved, that local analgesia using in postoperative period promotes early mother verticaliration, and optimal breast-feeding starting.

RESUMO

120 patients with primary or recurrent soft tissue sarcomas were treated during 1995-2010 years. Analysis of clinical materials allowed singling out most frequent mistakes in diagnostics and treatment that had been made in 52 (43%) patients. High frequency of mistakes caused by rarity of disease, complexity in diagnostics and choice of adequate treatment tactics demands the necessity to concentrate these patients in medical institutions that specialize in treatment of soft tissue sarcomas.

RESUMO

We report on results of a measurement of meson production in central Pb-Au collisions at E(lab) = 158A GeV. For the first time in the history of high energy heavy-ion collisions, phi mesons were reconstructed both in the K+K- and the dilepton decay channels in the same experiment. This measurement yields rapidity densities near midrapidity, from the two decay channels, of 2.05 +/- 0.14(stat) +/- 0.25(syst) and 2.04 +/- 0.49(stat) +/- 0.32(syst), respectively. The shape of the measured transverse momentum spectrum is also in close agreement in both decay channels. The data rule out a possible enhancement of the phi yield in the leptonic over the hadronic decay channel of a factor 1.6 or larger at the 95% C.L. This rules out the discrepancy reported in the literature between measurements of the hadronic and dimuon decay channels by two different experiments.

RESUMO

We report on first measurements of low-mass electron-positron pairs in Pb-Au collisions at the CERN SPS beam energy of 40 AGeV. The observed pair yield integrated over the range of invariant masses 0.2e(+)e(-) annihilation with a modified rho propagator. They may be linked to chiral symmetry restoration and support the notion that the in-medium modifications of the rho are more driven by baryon density than by temperature.

RESUMO

Based on an evaluation of data on pion interferometry and on particle yields at midrapidity, we propose a universal condition for thermal freeze-out of pions in heavy-ion collisions. We show that freeze-out occurs when the mean free path of pions lambda(f) reaches a value of about 1 fm, which is much smaller than the spatial extent of the system at freeze-out. This critical mean free path is independent of the centrality of the collision and beam energy from the Alternating Gradient Synchrotron to the Relativistic Heavy Ion Collider.

RESUMO

Computer-aided tomographic (CT) examination of 94 patients with esophageal cancer was carried out. Densitometric analysis of computer image of esophageal tumors making use of profile density histograms is presented for the first time. Objective graphic criteria for assessment of the depth of tumor invasion have been developed. CT results were correlated to intraoperative findings and data of histological examination of removed tumors. The sensitivity of the method in assessment of esophageal cancer dissemination was 97.7%, specificity 93.4%.

RESUMO

Computed tomography (CT) findings were compared with operative ones for 50 patients with esophageal and proximal gastric cancer. In diagnosis of metastases to the upper abdominal and retroperitoneal lymph nodes ST sensitivity reached 93.3%, specificity 85% and accuracy 90.3%. The best diagnostic results were obtained in detection of metastases to the nodes of the celiac trunk branch and paraaortal fat (94.2%). Lower CT resolution appeared in diagnosis of perigastric node metastatic involvement in the primary tumor location in the proximal stomach.

RESUMO

The examination of 170 patients with cancer of the esophagus and proximal stomach made it possible to specify x-ray, ultrasonic and CT signs of cancer metastases to the upper abdomen and retroperitoneum. A scheme of radiodiagnosis of these metastases is suggested. The diagnostic value of x-ray, echography and CT was considered in relation to various lymph nodes involvement.

RESUMO

The paper is based on analysis of the results of ultrasound investigation of 67 patients with esophageal cancer with metastatic involvement of abdominal and retroperitoneal lymph nodes verified at operation. For effective diagnosis of these changes during ultrasound scanning one should observe the main methodological principles and methods including polyprojection, polyposition and staging of investigation. It should be noted that unchanged lymph nodes cannot be visualized.

RESUMO

X-ray and endoscopic investigations were compared in 80 operated on patients to estimate the role of the x-ray method for a choice of adequate therapeutic tactics for esophageal cancer patients. Operations were explorative or palliative resulting from the involvement of the tracheobronchial tree in a tumor process. The efficacy of the x-ray method in the detection of tracheal compression by an esophageal tumor is higher than that of tracheobronchoscopy which in its turn, has its advantages in the determination of tumor spreading to the left major bronchus.

RESUMO

The prognostic and clinicodiagnostic significance of an x-ray picture of ulceration in esophageal cancer was established on the basis of the correlation of x-ray, morphological, endoscopic, operative findings and the control x-ray results following radio- and chemotherapy. Altogether 70 patients were investigated. X-ray symptoms were classified; their significance in the choice of optimum therapeutic tactics was defined.
