UNLABELLED: Nicotine addiction (NA) is a serious public health problem and helping patients to stop consumption is a major concern for general practitioners (GP). The positive effects of "brief advice" are well known. The aim of our paper is to evaluate the effectiveness of adding exhaled carbon monoxide (CO) measurement using a CO analyser to a normal smoking cessation practice in a GP's office. METHODS: This was a descriptive study, using a questionnaire, among randomised smoking patients; one group receiving brief advice and the other brief advice plus exhaled CO analysis in their GP's office. RESULTS: Five hundred and seventy-eight questionnaires were analysed: 60% of smoking patients wished to stop smoking and 72% within the next 6 months, without significant difference between the two groups. In the group "minimal advice plus CO analyser" (282), 60% of the smokers reported that they had been influenced by the results of the level of CO in the exhaled air and were more motivated to quit because of this. CONCLUSION: Measuring the level of exhaled CO in smokers helps to inform them of the dangers of their addiction and helps to motivate them to quit.

INTRODUCTION: Ketogenic diets have been employed for the treatment of intractable epilepsy in children since 1921, although underlying mechanism remains unknown. OBSERVATION: We report the case of a 54-year-old man with partial refractory status epilepticus who exhibited a favourable outcome about seven days after introduction of a ketogenic diet in association with antiepileptic drugs. DISCUSSION: Although its efficiency was largely demonstrated in children, little is known about the impact of a ketogenic diet in adults with refractory epilepsy. CONCLUSION: Introduction of a ketogenic diet requires a multidisciplinary approach. Its usefulness in adult intractable epilepsy and/or refractory status epilepticus merits further study into its efficacy in reducing the frequency of seizures and a possible prolonged effect.

OBJECTIVES: To evaluate the influence of season on side effects of antihypertensive treatments in hypertensive patients followed by cardiologists. METHODS: In 12,071 hypertensive subjects, referred to cardiologists, a questionnaire with 14 possible side effects due to antihypertensive treatment was administered during the consultation. Details on antihypertensive treatment and cardiovascular characteristics were obtained from medical records. This evaluation was obtained in patients recruited during summer (7,438 patients) and during winter (4,633 patients). RESULTS: In this population of treated hypertensives, blood pressure level was < 140/90 mmHg in 28%, and 50% were treated with a monotherapy. At least one side effect was declared by 40% of cases, and it was observed dyspnea (10%), dizziness (8%), fatigue (8%), palpitations (6%), ankle oedema (5%), headaches (5%). The higher rate was observed in patients on monotherapy, most frequent side effects were fatigue (9%) on beta-blockers, cough (9%) on ACEI, oedema (18%) on calcium antagonists, dyspnea (9%) on diuretics, dizziness on ARB (8%) and central acting agents (18%). Regression analysis indicates that female patient, uncontrolled BP, age less than 50, and greater number of antihypertensive tablets were independent determinants for at least one side effects under antihypertensive treatments. Season was not a significant determinant of side effect appearance. CONCLUSION: This study indicates that during hypertensive treatments, at least one side effect is declared by 40% of patients. A female patient, uncontrolled for BP, aged less of 50 and treated with a multiple therapy is more prone to complain of side effects, but season was not a significant determinant of its appearance.

RESUMO

Setoperone, a piperidine derivative known for its potent serotonin and moderate dopamine receptor blocking properties was labelled with the positron emitter 18F using a nucleophilic substitution on the nitro derivative. The general pattern of the in-vivo and in-vitro rat brain distribution of this new radioligand was consistent with the mapping of serotonin (5HT2) and dopamine (D2) receptors. The cortical binding of 18F-setoperone was selectively inhibited by ketanserin and not by sulpiride. The affinity of the radiofluorinated ligand for the serotonin receptors was in the nanomolar range (Kd = 0.7 nM).
