Objectives: To compare prosthetic joint infection (PJI) and acute kidney injury (AKI) rates among cohorts before and after changing our hospital's antimicrobial prophylactic regimen from cefuroxime to teicoplanin plus gentamicin. Methods: We retrospectively studied all patients undergoing primary total joint arthroplasty at our hospital 18 months pre- and post-implementation of the change in practice. All deep infections identified during follow-up were assessed against the European Bone and Joint Infection Society (EBJIS) definitions for PJI. Survival analysis using Cox regression was employed to adjust for differences in baseline characteristics and compare the risk of PJI between the groups. AKIs were identified using pathology records and categorized according to the KDIGO (Kidney Disease - Improving Global Outcomes) criteria. AKI rates were calculated for the pre- and post-intervention periods. Results: Of 1994 evaluable patients, 1114 (55.9 %) received cefuroxime only (pre-intervention group) and 880 (44.1 %) patients received teicoplanin plus gentamicin (post-intervention group). The overall rate of PJI in our study was 1.50 % (30 of 1994), with a lower PJI rate in the post-intervention group (0.57 %; 5 of 880) compared with the pre-intervention group (2.24 %; 25 of 1114). A corresponding risk reduction for PJI of 75.2 % (95 % CI of 35.2-90.5; p=0.004) was seen in the post-intervention group, which was most pronounced for early-onset and delayed infections due to coagulase-negative staphylococci (CoNS) and cefuroxime-resistant Enterobacteriaceae. Significantly higher AKI rates were seen in the post-intervention group; however, 84 % of cases (32 of 38) were stage 1, and there were no differences in the rate of stage-2 or -3 AKI. Conclusions: Teicoplanin plus gentamicin was associated with a significant reduction in PJI rates compared with cefuroxime. Increases in stage-1 AKI were seen with teicoplanin plus gentamicin.

The phase delay introduced by photodetectors can be affected by intensity, reverse bias, and temperature through different effects. An optical pilot tone superimposed on the detected signal allows an independent measurement of such phase errors in the complete photodetection chain and provides an opportunity to correct them. This allows to further separate readout noise from the measurement, providing a more performant and intensity-invariant phase readout. We test the functional principle on a setup demonstrating an improved phase noise performance and a reduced phase walk below 10 mHz in particular. This benefits applications that require accurate timing or signal phase determination with photodiodes.

We demonstrate a method for measuring a surface map of a spherical body with interferometric optical point sensors while rotating the test subject. The setup takes advantage of the excellent performance of heterodyne interferometry at nanometer levels and suppression of common-mode errors, as a cylindrical mirror mounted adjacent to the sphere is used as a reference. Future space based missions for gravitational wave research demand an improved inertial reference sensor with reduced acceleration noise levels. Spherical test masses can enable increased performance by suspension-free operation, contrary to cuboid solutions suffering from cross-coupling of attitude control noise into test mass position. However, interferometric readout is affected by surface irregularities and test mass attitude. An accurate surface map for compensation of the center of gravity readout should be established, by characterization either a priori or in-flight, when optical path length changes due to the surface occur in the measurement bandwidth.

RESUMO

Perioperative pain management in patients undergoing major surgery with restrictive lung disease can be a challenge. Facioscapulohumeral muscular dystrophy, a rare genetic disorder, causes progressive proximal weakness resulting in chronic pain. We describe a patient undergoing elective scapulothoracic fusion to improve chronic pain. We demonstrated the use of 2 continuous regional catheters for perioperative analgesia to reduce opioid consumption. We used interscalene brachial plexus and erector spinae plane block with catheter insertion using continuous local anesthetic infusion. The aim was to reduce potential respiratory complications in a patient with severe restrictive lung disease and reduce hospital stay.

RESUMO

Interferometric laser ranging is an enabling technology for high-precision satellite-to-satellite tracking within the context of Earth observation, gravitational wave detection, or formation flying. In orbit, the measurement system is affected by environmental influences, particularly satellite attitude jitter and temperature fluctuations, imposing an instrument design with a high level of thermal stability and insensitivity to rotations around the spacecraft center of mass. The new design concept presented here combines different approaches for dynamic heterodyne laser ranging and features the inherent beam-tracking capabilities of a retroreflector in a mono-axial configuration. It allows for a continuously adjustable distance between the optical bench and the location of its fiducial point, facilitating future inter-satellite tracking with nanometer accuracy, e.g., the next-generation gravity mission.

Attosecond spectroscopy and precision frequency metrology depend on the stabilization of the carrier-envelope phase (CEP) of mode-locked lasers. Unfortunately, the phase of only a few types of lasers can be stabilized to jitters in the few-hundred millirad range. In a comparative experimental study, we analyze a femtosecond Ti:sapphire laser and three mode-locked fiber lasers. We numerically demodulate recorded time series of the free-running carrier-envelope beat note. Our analysis indicates a correlation between amplitude and frequency fluctuations at low Fourier frequencies for essentially all lasers investigated. While this correlation typically rolls off at frequencies beyond 100 kHz, we see clear indications for a broadband coupling mechanism in one of the fiber lasers. We suspect that the observed coupling mechanism acts to transfer intracavity power fluctuations into excess phase noise. This coupling mechanism is related to the mode-locking mechanism employed and not to the gain medium itself. We further verify this hypothesis by numerical simulations, which identify resonances of the saturable absorber mirror as a possible explanation for the coupling mechanism. Finally, we discuss how to avoid a detrimental influence of such resonances.

In the context of our investigations on novel inertial reference sensors for space applications, we have explored a design utilizing an optical readout of a spherical proof mass. This concept enables full drag-free operations, hence reducing proof mass residual acceleration noise to a minimum. The main limitations of this sensor are errors in position determination of the center of mass of the proof mass due to the surface topography and the involved path length changes upon rotation. One solution is to apply a surface map for correction of the measurement data, thus improving the precision of position determination. This article presents the results of our one-dimensional interferometric surface topography measurements of a sphere, achieving uncertainties of ≈10 nm, as a first step to realize a complete surface map. The measurement setup consists of two heterodyne interferometers positioned in an opposing configuration, which measure the surface topography while the sphere is continuously rotated by a rotation stage.

We characterize an Er:fiber laser frequency comb that is passively carrier envelope phase-stabilized via difference frequency generation at a wavelength of 1550 nm. A generic method to measure the comb linewidth at different wavelengths is demonstrated. By transferring the properties of a comb line to a cw external cavity diode laser, the phase noise is subsequently measured by tracking the delayed self-heterodyne beat note. This relatively simple characterization method is suitable for a broad range of optical frequencies. Here, it is used to characterize our difference frequency generation (DFG) comb over nearly an optical octave. With repetition-rate stabilization, a radiofrequency reference oscillator limited linewidth is achieved. A lock to an optical reference shows out-of-loop linewidths of the comb at the hertz level. The phase noise measurements are in excellent agreement with the elastic tape model with a fix point at zero frequency.

RESUMO

We report on efficient generation of millijoule-level, kilohertz-repetition-rate few-cycle laser pulses with radial polarization by combining a gas-filled hollow-waveguide compression technique with a suitable polarization mode converter. Peak power levels >85 GW are routinely achieved, capable of reaching relativistic intensities >10(19) W/cm2 with carrier-envelope-phase control, by employing readily accessible ultrafast high-energy laser technology.

RESUMO

We demonstrate the generation of 22.6 µJ of combined energy at 3 µm for sub-300fs pulses at a repetition rate of 1 kHz using a LGSe optical parametric amplifier (OPA). The LGSe OPA is pumped by the 140-fs 1.6 µm pulses from a 300-mW KTA optical parametric chirped pulse amplifier (OPCPA) based on an all-optical synchronization scheme. By using a highly-nonlinear fiber, the output of an erbium-doped fiber laser operating at 1560 nm is shifted to 1050 nm in order to coherently seed a Nd:YLF regenerative amplifier. The LGSe OPA is seeded using the MIR coming from the amplification of the 1.6 µm in the OPCPA.

RESUMO

Moving-coil loudspeakers typify the interdisciplinary nature of acoustics: in order to reproduce sound, these devices employ principles of electricity, magnetism, mechanics, and acoustics. The widespread use of loudspeakers has made them a familiar and valuable opportunity to introduce students to acoustics. A low-cost loudspeaker project/demonstration is presented here that is built from scratch using common household items and craft supplies. A variety of educational topics may be illustrated with this device, making it appropriate for a wide range of academic levels.

RESUMO

The waveguide invariant is a useful parameter for understanding the behavior of interference patterns (e.g., striations in time-frequency plots) resulting from broadband acoustic sources in shallow water waveguides. It is possible to model these striations for range-dependent environments using conventional parabolic equation methods; although this approach can be computationally intensive as a full field must be created for each frequency and azimuthally dependent geometry. This letter discusses the formulation and use of a range-dependent waveguide invariant distribution that can be used to describe spectral striation patterns using a fraction of the computing power required by parabolic equation methods.

We report on a kHz, mJ-level, carrier-envelope phase (CEP)-stable ultrabroadband optical parametric chirped-pulse amplifier (OPCPA) at 2.1-µm wavelength, pumped by a high-energy, 14 ps, cryogenic Yb:YAG pump laser, and its application to high-order harmonic generation (HHG) with Xe. The pre-amplifier chain is pumped by a 12-ps Nd:YLF pump laser and both pump lasers are optically synchronized to the signal pulse of the OPCPA. An amplified pulse energy of 0.85 mJ was obtained at the final OPCPA stage with good beam profile. The pulse is compressed to 4.5 optical cycles (<32 fs) with a spectral bandwidth of 474 nm supporting 3.5 optical cycles. The CEP stability was measured to be 194 mrad and the super-fluorescence noise is estimated to be ~9%. First HHG results are demonstrated with Xe showing significant cutoff extension to >85 eV with an efficiency of ~10-10 per harmonic, limited by the maximum gas pressure and flow into the chamber. This demonstrates the potential of this 2.1-µm source for scaling of photon energy and flux in the water-window range when applied to Ne and He at kHz repetition rate.

A passively phase-locked laser source based on compact femtosecond Er:fiber technology is introduced. The carrier-envelope offset frequency is set to zero via difference frequency generation between a soliton at a wavelength of 2 µm and a dispersive wave at 860 nm generated in the same highly nonlinear fiber. This process results in a broadband output centered at 1.55 µm. Subsequently, the 40 MHz pulse train seeds a second Er:fiber amplifier, which boosts the pulse energy up to 8 nJ at a duration of 125 fs. Excellent phase stability is demonstrated via f-to-2f spectral interferometry.

We demonstrate a scheme for efficient coherent anti-Stokes Raman scattering (CARS) microscopy free of nonresonant background. Our method is based on a compact Er:fiber laser source. Impulsive excitation of molecular resonances is achieved by an 11 fs pulse at 1210 nm. Broadband excitation gives access to molecular resonances from 0 cm(-1) up to 4000 cm(-1). Time-delayed narrowband probing at 775 nm enables sensitive and high-speed spectral detection of the CARS signal free of nonresonant background with a resolution of 10 cm(-1).

We present a compact coherent anti-Stokes Raman scattering microscope based on a widely tunable picosecond Er:fiber laser. Intense and bandwidth-limited 1 ps pump pulses at a center wavelength of 775 nm are generated via frequency mixing within the broadband fundamental at 1.55 microm. Narrowband Stokes pulses are obtained by frequency shifting of solitons in a highly nonlinear bulk fiber and subsequent second-harmonic generation. The tuning range from 850 nm to 1100 nm gives access to vibrational resonances between 1150 cm(-1) and 3800 cm(-1). A first imaging application in the spectral region of CH stretch vibrations is demonstrated.

We demonstrate an all-fiber turnkey source of extremely stable 2-cycle pulses at a center wavelength of 1.17 microm. Taylor-cut highly nonlinear germano-silica bulk fibers (HNFs) provide smooth supercontinua with a bandwidth of 560 nm and a spectral shape precisely controlled by the dispersion of the fiber and the phase of the 1.55 microm pump pulses. Alternatively these fibers are capable of generating pulses with central wavelengths continuously tunable from 0.9 microm up to 1.4 microm. These results are based on parameter-free simulations of nonlinear pulse propagation including higher-order dispersion as well as instantaneous Kerr and retarded Raman contributions.

Phase-locked terahertz transients with peak electric fields of 108 MV/cm and center frequencies continuously tunable from 10 to 72 THz are generated via difference-frequency mixing of two parametrically amplified pulse trains from a single white-light seed. Free space electro-optic sampling with 8 fs gating pulses from a two-branch Er:fiber laser allows us to monitor all transients directly in the time domain. We identify extreme terahertz nonlinearities in the detector crystal with subcycle resolution.

We present what is believed to be the first direct measurement of the relative timing jitter between the two parallel pulse trains of a two-branch femtosecond erbium-doped fiber laser, operated without active stabilization. The system provides independently tunable pulses in the near infrared with durations down to 13 fs. Using an interferometric optical cross-correlator, the phase-noise spectral density is measured with high sensitivity in a range from 1 Hz up to the Nyquist frequency of 24.5 MHz. We find an integrated jitter of 11 attoseconds directly after the amplifier stages and 43 as after propagation through free-space optics and nonlinear fibers for frequency conversion.
