RESUMO

Dermatofibrosarcoma protuberans (DFSP) is a locally aggressive soft tissue tumor with a high propensity of local recurrence after surgery. Radiotherapy as an adjuvant therapy has been shown to reduce recurrent rates of this disease. Surface mould brachytherapy is an effective and safe modality for the delivery of radiotherapy in soft tissue tumors, though its utilization and popularity have decreased in recent years. Here, we presented a case of a recurrent DFSP of the scalp who was treated with surgery followed by adjuvant surface mould brachytherapy to avoid dose inhomogeneity likely to occur in this anatomic region with external beam radiotherapy in the absence of intensity-modulated radiotherapy. The treatment was delivered successfully with minimal adverse reactions, and the patient is disease-free at 18 months post-treatment with no treatment toxicity.

RESUMO

ABSTRACT: Various treatment modalities are available for treatment of ocular tumors, which include chemotherapy, laser, and radiotherapy (external beam radiation therapy or brachytherapy). Brachytherapy using plaque applicator is preferred over external beam radiation therapy when the tumor is well localized, as this therapy delivers radiation dose to the tumor with lower doses to normal tissues in the vicinity. However, plaque therapy is expensive and beyond the reach of many poor patients in India. The Bhabha Atomic Research Center (BARC) recently introduced an indigenous Ruthenium-106 plaque to make brachytherapy treatment available and affordable to all needy patients in India. In the present case series, we report our experience using the indigenous Ru-106 plaque for the treatment of a spectrum of ocular tumors.

Background: Accurate assessment of surface and build-up doses has a key role in radiotherapy, especially for the superficial lesions with uncertainties involved while performing measurements in the build-up region. Objective: This study aimed to assess surface and build-up doses for 6 MV photon beam from linear accelerator using parallel plate ionization chamber, EBT3 Gafchromic films, and PRIMO Monte Carlo (MC) simulation code. Material and Methods: In this experimental study, parallel plate chamber (PPC05) and EBT3 Gafchromic films were used to measure doses in a build-up region for 6 MV beam from the linear accelerator for different field sizes at various depths ranging from 0 to 2 cm from the surface with 100 cm source to surface distance (SSD) in a solid water phantom. Measured results were compared with Monte Carlo simulated results using PENELOPE-based PRIMO simulation code for the same setup conditions. Effect of gantry angle incidence and SSD were also analyzed for depth doses at the surface and build-up regions using PPC05 ion chamber and EBT3 Gafchromic films. Results: Doses measured at the surface were 14.78%, 19.87%, 25.83%, and 31.54% for field sizes of 5×5, 10×10, 15×15, and 20×20 cm2, respectively for a 6 MV photon beam with a parallel plate chamber and 14.20%, 19.14%, 25.149%, and 30.90%, respectively for EBT3 Gafchromic films. Both measurement sets were in good agreement with corresponding simulated results from the PRIMO MC simulation code; doses increase with the increase in field sizes. Conclusion: Good agreement was observed between the measured depth doses using parallel plate ionization chamber, EBT3 Gafchromic films, and the simulated depth doses using PRIMO Monte Carlo simulation code.

Menthol mint (Mentha arvensis L., Family: Lamiaceae), popularly known as corn mint or Japanese mint, is an important industrial crop that is widely grown for its valued essential oil. Nitrogen (N) is an important macro-nutrient and an essential factor for optimizing the yield and quality of crops. Hence, rapid and accurate estimation of the N content is crucial for nutrient diagnosis in plants and to make precise N fertilizer recommendations. Generally, N concentration is estimated by destructive sampling methods; however, an indirect assessment may be possible based on spectral characteristics. This study aimed to compare the foliar N concentration based on non-destructive (reflectance) and destructive (laboratory analyses) methods in menthol mint. Foliar N concentration was measured through the Kjeldahl method and reflectance by Miniature Leaf Spectrometer C-710 (CID Bio-Science). Using reflectance data, several vegetation indices (VIs), that is, normalized difference red edge (NDRE), red edge normalized difference vegetation index (reNDVI), simple ratio (SR), green-red vegetation index (GRVI), canopy chlorophyll content index (CCCI), photochemical reflectance index (PRI), green chlorophyll index (CI Green ), red edge chlorophyll index (CI Red Edge ), canopy chlorophyll index (CCI), normalized pigment chlorophyll ratio index (NPCI), and structure insensitive pigment index (SIPI), were developed to determine the foliar N concentration. The highest correlation (r) between VIs and foliar N concentrations was achieved by NDRE (0.89), followed by reNDVI (0.84), SR (0.83), GRVI (0.78), and CCCI (0.76). Among the VIs, the NDRE index has been found to be the most accurate index that can precisely predict the foliar N concentration (R 2 = 0.79, RMSE = 0.18). In summary, the N deficiencies faced by the crop during its growth period can be detected effectively by calculating NDRE and reNDVI, which can be used as indicators for recommending precise management strategies for the application of nitrogenous fertilizers.

Long delays in renewal of calibration of secondary standards radiation dosimeters in radiation oncology centers due to the COVID19 pandemic have aroused concerns regarding accuracy in dose delivery to patients. The concerns are mainly due to the uncertainty in the absorbed dose to water calibration factor (NDW) over a period of time. In this study, the NDWfactor for two ion chambers, thimble type (Farmer) and parallel plate type (Markus), used in most of the radiotherapy centers, were retrospectively reviewed for 20 years. The calibration on all occasions except once was carried out at the Secondary Standards Dosimetry Laboratory, Bhabha Atomic Research Centre, Mumbai. The change in the NDWfactor over this period was < ±3%. We, therefore, believe that a dosimeter with no history of repairs showed reasonable stability in the NDWfactor over a long period.

Emerging infectious diseases (EID) are serious problems caused by fungi in humans and plant species. They are a severe threat to food security worldwide. In our current work, we have developed a support vector machine (SVM)-based model that attempts to design and predict therapeutic plant-derived antifungal peptides (PhytoAFP). The residue composition analysis shows the preference of C, G, K, R, and S amino acids. Position preference analysis shows that residues G, K, R, and A dominate the N-terminal. Similarly, residues N, S, C, and G prefer the C-terminal. Motif analysis reveals the presence of motifs like NYVF, NYVFP, YVFP, NYVFPA, and VFPA. We have developed two models using various input functions such as mono-, di-, and tripeptide composition, as well as binary, hybrid, and physiochemical properties, based on methods that are applied to the main data set. The TPC-based monopeptide composition model achieved more accuracy, 94.4%, with a Matthews correlation coefficient (MCC) of 0.89. Correspondingly, the second-best model based on dipeptides achieved an accuracy of 94.28% under the MCC 0.89 of the training dataset.

BACKGROUND: The process of radiotherapy treatment planning and delivery involves multiple steps and professionals causing it to be prone to errors. Radiotherapy centers equipped with old telecobalt machines have certain peculiar challenges to workflow. We designed and tested a checklist for radiotherapy technicians (RTTs) to reduce chances of error during treatment delivery on a telecobalt machine. MATERIALS AND METHODS: A physical checklist was designed for RTTs to use in the pretreatment pause using a template advocated by the American Association of Physicists in Medicine. It was tested on 4 RTTs over 1000 radiotherapy delivery sittings. RESULTS: The checklist helped to rectify 41 documentary lapses and 28 errors in radiotherapy treatment parameters while also identifying 12 instances where treatment plan modifications were due and 30 where the patient was due for review by the radiation oncologist. The average time to go through the checklist was between 2.5 and 3 min. CONCLUSIONS: The development and use of the checklist has helped in reducing errors and also improving workflow in our department. It is recommended to utilize such physical checklists in all radiotherapy centers with telecobalt machines. The success of the checklist depends upon leadership, teamwork, acceptance of a need to inculcate a "safety culture," with voluntary error-reporting and a willingness to learn from such errors.

OBJECTIVE: The aim of this retrospective study is to reduce the dose of heart, both lung and opposite breast and left anterior descending artery (LAD) and avoid long term complication and radiation induced secondary malignancies in radiotherapy left breast/chest wall without losing homogeneity and conformity of the Planning Target Volume (PTV), contoured using Radiotherapy Oncology Group (RTOG 1005) guideline. MATERIALS AND METHODS: The treatment plans were generated retrospectively by TFIF, VMAT and Composite techniques for 30 patients. Dose-Volume Histograms (DVHs) were evaluated for PTV and organs at risk (OAR's) and analyzed in two groups BCS and MRM using Wilcoxon signed rank test. RESULTS: The homogeneity index (HI) was improved in Composite technique by 32.72% and 21.81% of VMAT, 50.66% and 49.41% of TFIF in BCS and MRM group respectively. The Conformity Index (CI) for composite plan was statistically same as VMAT and superior by 27.94% and 41.37% of TFIF in BCS and MRM group respectively. The low dose volume V5Gy and V10Gy of the heart were improved in Composite plan by 47.9% and 26.1% of VMAT respectively in BCS group and in MRM group, improved by 21.2% and 45.6% of VMAT. The V5Gy and V10Gy of ipsilateral lung were improved in Composite plan by 16% and 13.7% of VMAT respectively in BCS and 8.4% and 3% of VMAT respectively in MRM group. CONCLUSION: The Composite plan consisting of VMAT and TFIF plan with an optimum selection of fractions can achieve lower low dose exposure to the OAR's without compromising coverage compared to VMAT.

AIMS: This study aims to derive simple yet robust formula(s) for the calculation of cranial tumor volume using linear tumor dimensions in anterioposterior (AP), mediolateral (ML) and craniocaudal (CC) directions and also propose a reproducible methodology for tumor dimension measurements. MATERIALS AND METHODS: Magnetic resonance images (MRI) of 337 patients planned for Gammaknife Stereotactic Radiosurgery for different types of brain tumors were analyzed using Leksell Gamma Plan (LGP) software. Tumor volume in three dimensional was outlined and maximum tumor diameters were measured in three orthogonal directions AP, ML, and CC on the MRI. Formulas were derived to calculate tumor volume from AP, ML, and CC diameters using linear regression technique. An agreement between the calculated volume and standard volume observed from LGP software was determined using Bland Altman (B-A) plot. A comparison was made between the volume calculated using traditionally used formula of ellipsoid, standard volume obtained from LGP software and volume calculated from formulas derived in the present study. RESULTS: The tumors were divided into two categories based on their size for better volume prediction. The tumors having product of their diameters in the range 0-2.5cc were called "small tumors" and the formula proposed for their volume estimation (V = 1.513) × (AP × ML × CC) + 0.047 ) was found to predict the tumor volume with an average bias of 0.0005cc. For "large tumors," having product of diameters in the range 2.5-36cc, the proposed formula (V = 0.444 × (AP × ML × CC) + 0.339 ) predicted the tumor volume with an average bias of 0.007cc. CONCLUSIONS: The two formulas proposed in the study are more accurate as compared to the commonly used formula that considers the tumors as ellipsoids. The methodology proposed in the study for measurement of linear tumor dimensions is simple and reproducible.

Emerging infectious diseases (EIDs) are a severe problem caused by fungi in human and plant species across the world. They pose a worldwide threat to food security as well as human health. Fungal infections are increasing now day by day worldwide, and the current antimycotic drugs are not effective due to the emergence of resistant strains. Therefore, it is an urgent need for the finding of new plant-origin antifungal peptides (PhytoAFPs). Huge numbers of peptides were extracted from different plant species which play a protective role against fungal infection. Hundreds of plant-origin peptides with antifungal activity have already been reported. So there is a requirement of a dedicated platform which systematically catalogs plant-origin peptides along with their antifungal properties. PlantAFP database is a resource of experimentally verified plant-origin antifungal peptides, collected from research articles, patents, and public databases. The current release of PlantAFP database contains 2585 peptide entries among which 510 are unique peptides. Each entry provides comprehensive information of a peptide that includes its peptide sequence, peptide name, peptide class, length of the peptide, molecular mass, antifungal activity, and origin of peptides. Besides this primary information, PlantAFP stores peptide sequences in SMILES format. In order to facilitate the user, many tools have been integrated into this database that includes BLAST search, peptide search, SMILES search, and peptide-mapping is also included in the database. PlantAFP database is accessible at http://bioinformatics.cimap.res.in/sharma/PlantAFP/.

PURPOSE: The purpose of the study was to estimate the backscatter electron dose in internal shielding during electron beam therapy using Monte Carlo (MC) simulations and Gafchromic film measurements. MATERIALS AND METHODS: About 6 and 9 MeV electron beams from a Varian 2100C linac were simulated using BEAMnrc MC code. Various clinical situations of internal shielding were simulated by modeling water phantoms with 2 mm lead sheets placed at different depths. Electron backscatter factors (EBF), a ratio of dose at tissue-shielding interface to the dose at the same point without the shielding, were estimated. The role of 2 mm aluminum in reduction of backscatter was investigated. The measurements were also performed using Gafchromic films and results were compared with MC simulations. RESULTS: For particular beam energy, the EBF value initially increased with depth in the buildup region and then decreased rapidly. The highest value of EBF for both the energies is nearly same though at different depths. Decreased EBF was observed for 9 MeV beam in comparison to the 6 MeV beam for the same depth of shielding placement. Two millimeter aluminum reduced the backscatter by nearly 25% at maximum backscatter condition for both the energies, though the effectiveness slightly decreased at higher energy. The range of backscatter electrons was varying from 5 to 12 mm in the upstream direction from the interface. The Gafchromic film-measured EBF and MC-simulated EBF were matching well within the clinically acceptable limits except in close vicinity of tissue-lead interface. CONCLUSIONS: This study provides an important clinical data to design internal shielding at the local clinical setup and confirms applicability of MC simulations in backscatter dose calculations at interfaces where physical measurements are difficult to perform.

INTRODUCTION: Does metabolic imaging help in better definition of target during radiation treatment planning by bringing about changes in dimensions of the primary tumor in terms of diameter, length, and picking up new skip lesions or nodal stations which in turn prevents geographic misses by including more 18F-fluorodeoxyglucose avid regions not visible on conventional imaging? MATERIALS AND METHODS: We compared the length and radial dimensions of the primary tumor as well as changes brought about due to addition of new nodal stations, involved structures, and skip lesions in 50 patients of carcinoma esophagus treated between 2011 and 2013, as seen on contrast-enhanced computed tomography (CT) thorax and positron emission tomography (PET)/CT and drew conclusions regarding the technical changes brought about in treatment planning by the additional input of PET/CT. RESULTS AND CONCLUSIONS: PET-CT tremendously changes treatment plans by expanding the gross tumor volume and including regions which might otherwise have been missed on purely CT-based plans. Of the 50 patients, it changed the contouring and treatment planning of 35 patients and did not impact the remaining 15. Whether this translates into better long-term controls requires further validation by randomized controlled trials, which was not our present objective.

In traditional, herbal medicine, and aromatherapy, use of essential oils and their aroma compounds have been known since long, for the management of various human diseases. The essential oil is a mixture of highly complex, naturally occurring volatile aroma compounds synthesized by medicinal and aromatic plants as secondary metabolites. Essential oils widely used in pharmaceutical, cosmetic, sanitary, food industry and agriculture for their antibacterial, antiviral, antifungal, antiparasitic, insecticidal, anticancer, neuroprotective, psychophysiological, and anti-aging activities. Moreover, volatile aroma compounds comprise a chemically diverse class of low molecular weight organic compounds with significant vapor pressure. However, aroma compounds produced by plants, mainly attract pollinators, seed dispersers and provide defense against pests or pathogens. However, in humans, about 300 active olfactory receptor genes are involved to detect thousands of different aroma compounds and modulates expression of different metabolic genes regulating human psychophysiological activity, brain function, pharmacological signaling, and therapeutic potential. Keeping in mind this importance, present database, namely, AromaDb (http://bioinfo.cimap.res.in/aromadb/) covers information of plant varieties/chemotypes, essential oils, chemical constituents, GC-MS profile, yield variations due to agro-morphological parameters, trade data, aroma compounds, fragrance type, and bioactivity details. The database includes 1,321 aroma chemical structures, bioactivities of essential oil/aroma compounds, 357 fragrance type, 166 commercially used plants, and their high yielding 148 varieties/chemotypes. Also includes calculated cheminformatics properties related to identification, physico-chemical properties, pharmacokinetics, toxicological, and ecological information. Also comprises interacted human genes affecting various diseases related cell signaling pathways correlating the use of aromatherapy. This database could be a useful resource to the plant's growers/producers, an aroma/fragrance industrialist, health professionals, and researchers exploring the potential of essential oils and aroma compounds in the development of novel formulations against human diseases.

PURPOSE: Dose received by organs at risk (OAR) in high-dose-rate (HDR) intracavitary brachytherapy (ICBT) for locally advanced cervical cancer impacts the late toxicity profile of the treatment. In the present study, we analyzed the inter-fraction variations of the minimum dose received by the most irradiated 2cc volumes (D2cc) of the OARs in ICBT. METHODS AND MATERIALS: This prospective study included 40 patients with cervical cancer stage FIGO IIB-IVA treated with HDR ICBT and concomitant chemoradiotherapy with Computerized tomography (CT)- based three-dimensional planning. In addition, for 20 (of the 40) patients, the first fraction plan was superimposed on the second fraction images for studying its dosimteric impact on the OAR. The D2cc data for the OAR was statistically analyzed for interfraction variations with Chi-square test or Fisher exact test as applicable. Paired t-test was used to compare the difference in means for the D2cc values between the three fractions. RESULTS: The interfraction variations of the D2cc values of the OAR were statistically insignificant having P = 0.41, 0.8, and 0.20 for bladder, rectum, and sigmoid, respectively. Further, in 6 out of 20 cases, wherein first fraction plan was superimposed on second fraction images, the OAR doses exceeded the prescribed tolerance limits. CONCLUSION: We did not find variations in the OAR doses when each fraction was planned and treated individually. However, we found that if a single plan is used to treat subsequent fractions, OAR doses may exceed tolerance in about 30% of the cases. We believe that a larger sample size with improved compliance of bladder and bowel protocols would be needed to arrive at definitive conclusions.

Breast cancer is a serious problem causing the death of women across the world. At present, one of the major challenges is to design drugs to target breast cancer specific gene(s). RNA interference (RNAi) is an important technique for targeted gene silencing that may lead to promising novel therapeutic strategies for breast cancer. Therefore, identification of such molecules having high oncogene specificity is the need of the hour. Here, we have developed a database named as Breast Oncogenic Specific siRNAs (BOSS, http://bioinformatics.cimap.res.in/sharma/boss/ ) on the basis of the current research status on siRNA-mediated repression of oncogenes in different breast cancer cell lines. BOSS is a resource of experimentally validated breast oncogenic siRNAs, collected from research articles and patents published yet. The present database contains information on 865 breast oncogenic siRNA entries. Each entry provides comprehensive information of an siRNA that includes its name, sequence, target gene, type of cells, and inhibition value, etc. Additionally, some useful tools like siRNAMAP and BOSS BLAST were also developed and linked with the database. siRNAMAP can be used for the selection of best siRNA against a target gene while BOSS BLAST tool helps to locate the siRNA sequences in deferent oncogenes.

It is known that the output factors (OPFs) for external-beam radiotherapy units increase with field size due to increased scattered radiation from the collimator system. Saturation in the OPF value is generally reported beyond approximately 30 × 30 cm(2). For the first time, to the best of our knowledge, we report on a drop in OPF values, although marginal, measured for a telecobalt machine beyond the 38 × 38 cm(2) field size. We believe that reporting and explaining the results will lead to a better understanding of the scatter composition of the radiation from telecobalt machines. This also has the potential to impact the estimation of low dose regions in patients, in addition to being a purely scientific inquiry. We used Monte Carlo (MC) simulations to validate the measured values. The MC data showed that the decrease in OPF was due to decreased scatter from the machine head.

Lichens are known to synthesize a variety of secondary metabolites having multifunctional activity in response to external environmental condition. Two common lichen extrolites, atranorin and salazinic acid, are known to afford antioxidant as well as photoprotectant nature depending on the abiotic/biotic stress. The present investigation aims to study the influence of altitudinal gradient on the quantitative profile of atranorin and salazinic acid in three lichen species, Bulbothrix setschwanensis (Zahlbr.) Hale, Everniastrum cirrhatum (Fr.) Hale and Parmotrema reticulatum (Taylor) Choisy, Parmeliaceae using liquid chromatography-mass spectrometry (LC-MS/MS) technique. Samples were collected from high-altitude area, usually considered as non-polluted sites of Garhwal Himalaya. Characterization and quantification of the lichen substances in samples were carried out comparing with the standards of atranorin and salazinic acid. Results indicated significant variation in the chemical content with the rising altitude. All the three lichen species showed higher quantities of chemical substances with the altitudinal rise, while among the three lichen species, E. cirrhatum showed the highest quantity of total lichen compounds. The higher abundance and frequency of E. cirrhatum with increasing altitude as compared to B. setschwanensis and P. reticulatum may be attributed due to the presence of higher quantity of photoprotecting/antioxidant chemicals especially salazinic acid. Thus, the present study shows the prominent role of secondary metabolite in wider ecological distribution of Parmelioid lichens at higher altitudes.

Intracavitary brachytherapy (ICBT) and interstitial brachytherapy (IB) techniques are commonly practiced for treating carcinoma of the cervix, either alone or in combination with external beam radiotherapy. Both these brachytherapy techniques have their own advantages and limitations in terms of tumor coverage and normal tissue sparing. Limited studies have been reported comparing the dosimetric features of these two techniques, especially from a single institution. We carried out a prospective clinical dosimetric comparison between ICBT and IB for patients treated at one center to bring out the inherent dosimetric features of these to two techniques. The study was carried out on 26 patients treated with ICBT and 55 with IB using CT-based planning. Of the 55 patients treated with IB, 27 included tandem source loading (IBT) and 28 without the tandem loading (IBWT). The high-dose volumes covered by 200% and 180% isodose surfaces were considerably larger in ICBT as compared to IB, whereas the treated volume was larger in IB as compared to ICBT. The bladder and rectal doses were the highest in ICBT and IBWT, respectively. The larger treated volume in IB as compared to ICBT was mainly because patients with larger tumor volumes were generally considered for IB. The results also indicated that in interstitial brachytherapy, better rectal sparing was achieved by including the tandem for treatment delivery.

Interspecific comparison in metals and PAHs profile in three lichen species, Flavoparmelia caperata, Phaeophyscia hispidula and Pyxine sorediata, were studied in different altitudinal gradients of the Western Himalayas. The species collected from 14 sites, enroute from Dehradun to Morinda (243 Km) including the trekking route 42 Km from Taluka to Morinda having an altitudinal gradient between 850-3,750 m, were analysed for their metals and PAHs. The species showed similar metal as well as PAHs profile under similar altitudinal gradients in the sequence of F. caperata > P. hispidula > P. sorediata. The difference in pollutant concentrations within each lichen species may be related to intrinsic attributes of the species, such as thallus morphology and the presence of lichen substances which are responsible for the sensitivity and accumulation potential of a particular species. Novelty of the present study lies on the fact that all the species show a similar efficiency of reflecting the environmental condition of the area, albeit the coefficient values of individual species for individual pollutant obtained by three-factor ANOVA revealed that the bioaccumulation affinity of F. caperata is significantly higher than P. hispidula and P. sorediata. For individual metals, F. caperata has a higher affinity for Al, Cr, Fe, Pb and Zn while P. hispidula has a significant positive affinity for Fe and Pb. PCA analysis of sites with respect to pollutant revealed the segregation of sites based on source and distance. Combining the bioaccumulation potential parameters along with geostatistical (GIS) techniques establishes that F. caperata species is a better accumulator of metals and PAHs in comparison to P. hispidula and P. sorediata in the temperate regions of the Himalaya.

The ferrous sulphate-benzoic acid-xylenol orange (FBX) chemical dosimeter, due to its aqueous form can measure average volume doses and hence may overcome the limitations of point dosimetry. The present study was undertaken to validate the use of FBX dosimeter for rectum and bladder dose measurement during intracavitary brachytherapy (ICBT) and transperineal interstitial brachytherapy (TIB). We filled cylindrical polypropylene tubes (PT) and Foley balloons (FB) with FBX solution and used them as substitutes for rectum and bladder dose measurements respectively. A water phantom was fabricated with provision to place the Fletcher-type ICBT and MUPIT template applicators, and FBX filled PT and FB within the phantom. The phantom was then CT scanned for treatment planning and subsequent irradiation. Our results show that the average difference between DVH derived dose value and FBX measured dose is 3.5% (PT) and 13.7% (FB) for ICBT, and 9% (PT) and 9.9% (FB) for TIB. We believe that the FBX system should be able to provide accuracy and precision sufficient for routine quality assurance purposes. The advantage of the FBX system is its water equivalent composition, average volume dose measuring capability, and energy and temperature independent response as compared to TLD or semiconductor dosimeters. However, detailed studies will be needed with regards to its safety before actual in-vivo dose measurements are possible with the FBX dosimeter.
