RESUMO

The Smoke Free North East Office (SFNEO) is the first dedicated tobacco control office in the UK coordinating a regional tobacco control network, Smoke Free North East (SFNE). On the basis of ethnographic research conducted between 2006 and 2008, this article examines the context for SFNEO's emergence at this time and in this region of England, and the main policy and practice challenges it has faced in its early years. SFNE formed in a favourable political and cultural climate, although regional champions were crucial in setting it up. It has worked well in branding itself and in taking advantage of the opportunity to lobby in support of comprehensive smoke-free legislation, although the success of the legislation presents a risk that people will regard SFNE's work as finished. There is a need for independent sustainable funding, strong partnership working and the 'bringing together' of existing organizations under its leadership for an organization such as SFNE to succeed. SFNE offers a model that is transferable to other places as well as to other public health concerns such as alcohol, and has been taken up by public health planners and policy makers with alacrity. This indicates a general perception that SFNE plays an effective role in public health delivery.

RESUMO

This paper highlights the importance of understanding management theories to development of the settings approach to health promotion. It then provides an overview of two areas of especial pertinence: organization development and systems thinking. This is followed by the articulation of the Healthy Living and Working Model, which is proposed as a mechanism for applying the principles of the settings approach in practice within organizations. The paper concludes by pointing to future challenges in developing this Model and reiterating that facilitating improvement at the organizational level should be the defining characteristic of the settings approach to health promotion.

RESUMO

Clinical governance within the National Health Service (NHS) and best value across local authorities in England have emphasised the need for decisions and actions being demonstrably evidence based. In an attempt to progress these related initiatives and facilitate a closer working relationship between NHS Trusts and local government departments, a multi-agency and interprofessional training event on evidence-based practice (EBP) was organised across three boroughs within Tyne and Wear. While there are advantages to different agencies learning and working in partnership to develop quality systems for health improvement, the reality of introducing EBP concepts and creating an evaluative culture within and across public sector organisations represents a major challenge. This article has adopted a case study approach to describe the key lessons learnt through critically reflecting on the planning and delivery of this training event, and it is hoped that it will be of value to other attempting similar projects in the future.

RESUMO

The Teaching Company Scheme allows subsidized employment of a graduate on a fixed-term contract. TCS is gradually being taken up by NHS organisations although it can be challenging to reconcile with its traditional focus on private sector criteria. South Tyneside's project involved developing a healthy living and working strategy.
