RESUMO

Los GRDs (grupos relacionados en el diagnóstico) se han mostrado muy útiles para lograr aproximaciones a los costes generados por la actividad médica con referencia a los diferentes diagnósticos, sin embargo en Psiquiatría las categorías diagnósticas actualmente utilizadas son tan limitadas que requieren una mayor profundización y discernimiento. Los autores estudian los 10.974 ingresos psiquiátricos producidos entre los años 1975 al 2000 en el Departamento de Psiquiatría del Hospital Clínico Universitario de Zaragoza (España), mostrando las diferencias en cuanto a las estancias medias según diferentes factores como el sexo de pertenencia, la edad, el estado civil, la situación laboral, la clase social, el lugar de residencia, la dependencia asistencial del paciente, el tipo de ingreso, la tasa de reingresos y por supuesto el diagnóstico psiquiátrico (AU)

RESUMO

As a part of the SAMAR 89 Project on mental health, we present the results of a psychiatric epidemiological research (in two stages) in a representative sample of the elderly urban population of Zaragoza (Spain). We study the relation between psychological disorders and sociodemographic factors, and network and social support. In the first stage we use a adaptation of the Enquête Santé Quebec and the General Health Questionnaire of 28 items of Goldberg. The Clinical Interview Schedule and the monumental state of Folstein were used for the psychiatric diagnoses in the second stage (diagnoses criteria of the DSM-III). The psychological disorders were more frequent on the women, the more aged and the ones of low socioeconomic level. Also was detected a correspondence between bad mental health and problems in the network and social support.

RESUMO

The author reviews the problem of the traditional psychiatric assistance based essentially in the custodial work. The psychiatric beds in that sense are not valid at the present moment. The changes in the kind of patients that to day we see in our clinics and general hospitals, are quite different and also the possibility of our treatments. In this paper, the author analyzes some of these changes observed in the psychiatric admissions in the province of Zaragoza (Spain) between 1966 and 1977. Essentially the increase was in the alcoholic group, and the diminution was in the schizophrenic one. Finally the author gives some numbers concerning the University Psychiatric Department that works in the context of one General Hospital, and concludes with some questions very difficult to answer.
