RESUMO

Schizophrenia (SZ) is a severe, chronic mental disorder without specific treatment. Due to the increasing prevalence of SZ in societies and the similarity of the characteristics of this disease with other mental illnesses such as bipolar disorder, most people are not aware of having it in their daily lives. Therefore, early detection of this disease will allow the sufferer to seek treatment or at least control it. Previous SZ detection studies through machine learning methods, require the extraction and selection of features before the classification process. This study attempts to develop a novel, end-to-end approach based on a 15-layers convolutional neural network (CNN) and a 16-layers CNN- long short-term memory (LSTM) to help psychiatrists automatically diagnose SZ from electroencephalogram (EEG) signals. The deep model uses CNN layers to learn the temporal properties of the signals, while LSTM layers provide the sequence learning mechanism. Also, data augmentation method based on generative adversarial networks is employed over the training set to increase the diversity of the data. Results on a large EEG dataset show the high diagnostic potential of both proposed methods, achieving remarkable accuracy of 98% and 99%. This study shows that the proposed framework is able to accurately discriminate SZ from healthy subject and is potentially useful for developing diagnostic tools for SZ disorder.

Approximately 25% of all oral cavity carcinomas involve the lips, and the primary management of these lesions is complete surgical resection. Loss of tissue in the lips after resection is treated with a variety of techniques, depending on the extension and location of the defect. Here we review highly accepted techniques of lip reconstruction and some of new trials with significant clinical results. Reconstruction choice is primarily depend to size of the defect, localization of defect, elasticity of tissues. But patient's age, comorbidities, and motivation are also important. According to the defect location and size, different reconstruction methods can be used. For defects involved less than 30% of lips, primary closures are sufficient. In defects with 35-70% lip involvement, the Karapandzic, Abbe, Estlander, McGregor or Gillies' fan flaps or their modifications can be used. When lip remaining tissues are insufficient, cheek tissue can be used in Webster and Bernard advancement flaps and their various modifications. Deltopectoral or radial forearm free flaps can be options for large defects of the lip extending to the Jaws. To achieve best functional and esthetic results, surgeons should be able to choose most appropriate reconstruction method. Considering defects' size and location, patients' expects and surgeon's ability and knowledge, a variety of flaps are presented in order to reconstruct defects resulted from tumor ablation. It's necessary for surgeons to trace the recent innovations in lip reconstruction to offer best choices to patients.

RESUMO

Nostril stenosis (narrowing of the nasal inlet) is an uncommon deformity which results in aesthetic and breathing discomfort in patients. The literature review shows that trauma, infection, iatrogenic insults and congenital lesions are major causes of stenosis. Nowadays, rhinoplasty is one of most popular aesthetic surgeries which may have complications such as bleeding, swelling, bruising, asymmetry, obstruction of nasal airways. We present a 30-year-old female patient, who complained about breathing and aesthetic difficulties due to external nasal valve obstruction and nasal deformity. Past medical history showed that the patient had undergone three unsuccessful rhinoplasty surgeries with aesthetic goals.

RESUMO

OBJECTIVE: Squamous cell carcinoma (SCC) is the most common malignancy of the oral cavity. This study aims to evaluate different treatment procedures including neo-adjuvant, concurrent and adjuvant therapy in treating squamous cell carcinoma of the head and neck by a meta-analysis. MATERIALS AND METHODS: The authors searched all electronic databases (Medline, Embase and Cochrane) for all the articles published from 1970 to January 2011. Data of the evaluated treatment procedures (chemotherapy or radiotherapy), number of patients, publishing date and the authors' names have all been extracted from the articles and have been categorized in a table. RESULTS: Forty-six researches are included in this study. All three ways show that using chemotherapy after or with radiotherapy improves the vitality rate significantly (p-value< 0.01). CONCLUSION: It is concluded that after deciding not to perform a surgery for treating SCC, the recommended treatment plan is chemotherapy and radiotherapy simultaneously.

INTRODUCTION: Cutting back on the staff and equipment and also reduction of costs and devices, balanced distribution of resources for optimal use and prevention of unreasonable costs for patients are some advantages of the fulfillment of referral system process. Accordingly the purpose of the present study is to determine factors affecting the fulfillment of referral system process from the viewpoints of clients of Isfahan's public hospitals so that health system could benefit its advantages. METHODOLOGY: The research is a descriptive-analytic survey and the population consisted of clients referring to public hospitals of Isfahan Province. Hospitals conforming to health network in cities of Falavarjan, ShahinShahr, Shahreza, Naein and some public hospitals of Isfahan city were selected for study. Cluster sampling was applied and at the end 400 subjects were selected by simple random sampling. FINDINGS: Results showed that awareness and satisfaction in urban communities and motivation in rural are of higher levels. Regarding insurance status, those covered by Social Security have the most referrals to all levels and those covered by Health Service Insurance have more referral to the first level. The mean score of awareness of clients about referral system among the hospitals investigated shows a significant difference (p-value≤0.05). CONCLUSION: Results indicate that from the viewpoints of urban clients, the rate of the awareness of and satisfaction with the way of offering health services at primary levels and from that of rural clients motivation affect the fulfillment of referral system process. Therefore improvement of these factors could make the system more efficient.
