RESUMO

UNLABELLED: Diastolic heart dysfunction occurs essentially in patients who suffer from chronic heart failure. In this context an investigation and application of different methods for diastolic function improvement remain a priority in the therapy of cardiovascular diseases. OBJECTIVE: Our ain was to estimate coronary artery bypass grafting effect on the left ventricularfunctionality in patients with coronaty heart disease. METHODS: Patients (30 men without myocardial infarction and 44 - with myocardial infarction) were investigated by Doppler Echocardiography before and after coronary artery bypass grafting for left ventricular diastolic function assessment. In addition, during the year after operation examinations were carried out. Analysis of the data allowed detecting two types of disorders of transmitral flow: rigid and pseudononnal. RESULTS: Our investigation showed that transinitral parameters improved in patients after operation. Patients without myocardial infarction in anamnesis had a myocardium cell relaxation improvement (IVRT decreasing; p =0,046), but an active myocardium relaxation turned for the better (increasing of E/A (p <0,001) by component A lowering (p =0, 003). Changes of pulmonary veins parameters were followed by the improvement of left heart ventricle relaxation: active - PVD decreasing, p =0,051; and passive - PVA decreasing, p =0,028. Patients with myocardial infarction in anamnesis had an IVRT decreasing (p =0,040) and A decreasing (p =0,041). In addition, in this group left atrial function improved (PVS decreasing; p =0,037). CONCLUSION: Coronary artery bypass grafting is effective in left ventricular diastolic function improvement and Doppler Echocardiography method can become an important criterion to estimate the treatment adequacy.

RESUMO

The blood content of total lipids, triglycerides, total cholesterol, alpha- and beta-lipoprotein cholesterol was measured in 79 patients with diffuse toxic goiter during treatment, in 25 patients with hypothyroidism, in 33 with thyroid hyperplasia, and in 54 healthy women aged 35-60 years from the group of native population of the European North. In all the forms of thyroid dysfunction, the patients manifested dyslipoproteinemia in the form of hypercholesterolemia with an increase of the concentration of beta-cholesterol, hypertriglyceridemia, and with a rise of the cholesterol atherogenic index to 4.0 and over. The signs of dyslipoproteinemia were marked to the greatest degree in thyrotoxicosis, at the stages of euthyroidism attainment or disease remissions.

The paper is concerned with the results of a retrospective analysis of the diagnostic value of infrared thermography of the thyroid by comparing clinical, histological (75.8% of observed cases) and thermographic diagnoses. Altogether 300 persons were examined, of them 30 were healthy subjects. The clinical diagnosis was based on the results of RIA, radio-isotope, immunological and preoperative cytological methods of investigation. The thermographic criteria of euthyroidism, hypo- and hyperthyroidism permitted accurate characterization of thyroid function in all the examinees, the diagnosis of toxic types of goiter (90.3% of coincidence with clinical diagnosis and 88.3 - with histological diagnosis for diffuse toxic goiter, 83.3 and 85.7% - the nodular toxic goiter), and assessment of the effect of thyrostatic therapy. During pregnancy (48 cases) thermograms of the thyroid of pregnant women were similar to the euthyroid ones, indicating a possibility to use thermography as an objective method of diagnosis of thyroid pathology in pregnancy. This method is informative in the diagnosis of benign nodular lesions (96% of coincidences with histological diagnosis) and less informative in malignant neoplasms and autoimmune thyroiditis (53.8 and 23.5% of coincidences with histological diagnosis).

The paper is devoted to the results of a study of IL-2 production by T-helpers in the course of treatment of patients with diffuse toxic goiter (DTG); 30 patients and 30 healthy controls were investigated. The diagnosis of DTG was confirmed by modern methods including radioimmunoassays. A study of the blood level (relative and absolute) of T-lymphocytes in the patients before therapy, during 2 and 12 mos. of thimazol therapy and during remission of disease showed its significant decrease as compared to the controls. IL-2 production in the blood lymphocyte culture supernatant of the DTG patients was assessed by testing on 3H-thymidine labeled ConA-T-blasts. A rise of the stimulation index before therapy and during 2 mos. of therapy, its normalization at the subsequent stage of therapy and the reduction in the patients with remission of thyrotoxicosis were established. A double control of IL-2 production (resulting from therapy and the level of thyroid hormones T3 and T4 decreasing during therapy) was assumed. The results of the study suggested the selectivity of affection of T-lymphocyte subpopulations in DTG and confirmed the preserving of T-helper function.

RESUMO

A study of the blood plasma content of total cholesterol in 575 healthy persons and 675 patients with diffuse toxic goiter (DTG), and the content of total lipids and triglycerides in 52 healthy persons and 132 DTG patients showed the development of adaptation hyperlipidemia in persons living long in the North. It was preserved in DTG patients irrespective of the gravity of thyrotoxicosis, stages of its therapy and the patients' age. Basing on the analysis of the level of lipid indices of erythrocytic membrane malonic dialdehyde and their refractoriness to peroxide hemolysis in 52 healthy persons and 53 DTG patients, a conclusion was made of excessive stable activation of lipid peroxidation biomembranes which can underlie the development of visceral complications.

RESUMO

Con A-induced production of interleukin 2 was studied in peripheral blood lymphocytes obtained from 15 normal subjects and 42 patients with different thyroid diseases. It was found that thyroid diseases are accompanied by changes in IL 2 production. The level of IL 2 production by T lymphocytes is determined by the functional status of thyroid glands, their anatomy, stages of the disease and mode of treatment.
