RESUMO

Efficacy of endoultrasonography application in conditions of moderate risk for choledocholithiasis occurrence was estimated in 33 patients, in whom ultrasonography (US) did not reveal the common biliary duct concrements. Sensitivity, specificity, accuracy, positive and negative prognostic values of the method in suspicion for choledocholithiasis presence while US performance have had constituted 78.6, 94.7, 82.0, 98.2 and 54.5%; vs for endoultrasonography--100, 94.4, 97, 93.8 and 100% accordingly. Endoultrasonography constitutes a safe and highly effective method of investigation in patients in suspicion of the biliary ducts disease presence, it permits to reveal choledocholithiasis in significant part of patients after ineffective US. This diagnostic option permits to reduce the need for performance of diagnostic endoscopic retrograde cholangiopancreaticography.

RESUMO

The results of treatment of 9103 patients, to whom laparoscopic cholecystectomy was performed, were adduced. In 6714 patients intraabdominal postoperative complications were absent, ultrasonographic investigation (USI) was not performed. Dynamical USI was done in 2389 patients, permitting to estimate timely the wound course severity in the gallbladder bed, to administer an adequate therapv, in 9 of them--to perform puncture of subhepatic formations under USI control. Intraabdominal infectious complications were revealed in 65 (0.71%) of patients.

RESUMO

In 46 patients with an destructive acute pancreatitis (AP) the invasive intervention under control of ultrasonography, in 38 of them--in stage of purulent complications, was performed. It had promoted stabilization of state in 39 (89.8%) patients, normalization of clinic--laboratory indexes, that had permitted to realize radical operative intervention with minimum risk in perspective. Invasive intervention under control of ultrasonography--highly effective primary surgical manipulation in complex of treatment of the destructive AP severe forms.

RESUMO

The ultrasonic semiotics of different diseases of hepar, pancreas, gallbladder, colon, postoperative complications was elaborated and complicated. New and complicated technological procedures for the complex ultrasonic investigation in before, intra- and postoperative periods were proposed. Minor traumatic transcutaneous intervention procedures under the ultrasonic control for the abdominal cavity and retroperitoneal space abscess, cysts of various localization were used for the first time in domestic practice. The treatment procedures applied ranged from puncture with subtle aspirating needle to one- or two-staged drainage performance. Clinical effectiveness constituted 85.2%.
