RESUMO

OBJECTIVE: The aim: To analyze the problems of PMTCT of HIV and to clarify the ways for their solving through the increasing their obstetrical and gynecological services at the primary health care level. PATIENTS AND METHODS: Materials and methods: Bibliosemantic, statistical and method of structural and logical analysis. The research materials are the data of the statistical reports for the period 2016-2020. CONCLUSION: Conclusions: The analysis of the problems of PMTCT for HIV demonstrated that in Ukraine there is insufficient availability of pregnant women for HIV testing, the excessive duration of the existing algorithm for testing pregnant women for HIV, the imperfect system of referrals of pregnant women for timely diagnosis, as a result, late and therefore less effective the prophylactic antiretroviral therapy (ART), defects in the effectiveness of standardization of health care for the prevention of mother-to-child transmission (PMTCT) of HIV with the participation of primary healthcare specialists, the main representative of which is a general practitioner - family medicine (GP-FD).

RESUMO

OBJECTIVE: The aim: To analyze the problem of family planning and highlight possible ways to solve them in the practice of a family doctor. PATIENTS AND METHODS: Materials and methods: Bibliosemantic, statistical, and method of structural-logical analysis. The materials of the study were the data of statistical reports for the period 2014-2020. CONCLUSION: Conclusions: In Ukraine, with the existing system of family planning with fairly standardized technologies, there are serious problems, which cause reproductive health disorders of the population. The integration of family planning services into the practice of the family doctor, provided that he/she is trained, will increase their availability, quality, and efficiency.

OBJECTIVE: The aim: To determine the priority directions for the integration of obstetric and gynecological care for the female population of Ukraine to the primary level of its provision. PATIENTS AND METHODS: Materials and methods: Sociological, statistical and bibliosemantic methods were used in the research. The systematic approach was the methodological basis of the study. 79 health care providers, 127 family doctors, 92 doctors of obstetrics and gynecology, and 248 women of reproductive age were interviewed as experts using a special questionnaire. RESULTS: Results: Some priority management and organizational measures that need to be carried out for the effective integration of obstetric and gynecological care to the female population of Ukraine at the primary level of its provision were identified in the research. They include, first of all, the training of family physicians and family nurses to provide medical services to the female population and the coordination of primary care and specialized obstetric and gynecological services. CONCLUSION: Conclusions: The priority of application of management and organizational measures for the effective integration of obstetric and gynecological care to the female population of Ukraine to the primary level of its provision was determined by an expert way.

RESUMO

OBJECTIVE: The aim is to determine the types of medical services that family doctors should provide to women during pregnancy and in the postpartum period and the competencies they should have to provide these services and the most acceptable ways to acquire them. PATIENTS AND METHODS: Materials and methods: During the study, the method of expert assessment, statistical method and method of structural and logical analysis were used. 50 obstetricians and gynaecologists who provide women with outpatient care and 50 family doctors acted as independent experts in the study. RESULTS: Results: The types of medical services that family doctors should provide to women during pregnancy and in the postpartum period and the competencies that they should have to ensure the provision of these services were determined by experts. The most acceptable ways to acquire the necessary competencies, as well as the benefits and risks of providing these services at the primary level of health care were designated. CONCLUSION: Conclusions: The implementation of the proposed types of medical care by family doctors will increase the availability and quality of medical care for women during pregnancy and in the postpartum period.
