OBJECTIVES: We conducted a trial to assess the treatment fidelity of an individual-based oral health education intervention utilising motivational interviewing (MI) techniques and its efficacy when compared to a group-based traditional oral health education intervention (TOHE) and a standard of care group (SC) in a sample from Philadelphia during a 12-month follow-up. BACKGROUND: There is lack of information on how different types of oral health educational interventions affect older adults on non-clinical outcomes including changes in oral health-related quality of life (OHRQoL), oral health self-efficacy (SE) and oral health knowledge (OHK). MATERIALS AND METHODS: One hundred and eighty patients were randomly allocated to TOHE, MI and SC groups. Treatment fidelity was measured in 16 non-study patients. The MI intervention was administered by a public health dental hygienist (PHDH). All interviews were audio-recorded and coded by an expert using the Motivational Interviewing Treatment Integrity (MITI) Code. Multivariable longitudinal regression analyses accounting for baseline demographics and correlated errors due to repeated measures via generalised estimating equation were conducted following an intention to treat approach. RESULTS: Over the 1-year follow-up, SE and OHRQoL scores significantly improved amongst the MI group whereas both outcomes worsened amongst the SC group. During the same period, SE and OHRQoL did not change in the TOHE group. CONCLUSION: Findings from the study support the fidelity of this intervention and the improvement of all non-clinical outcomes after 12 months amongst the MI group.

BACKGROUND: Population studies of the prevalence of oral disease rely upon indices that summarize disease status. There is no universally accepted index that summarizes the burden of periodontal diseases, considering the number of teeth remaining in the mouth. METHODS: A new conceptual index was developed that includes consideration of the extent and severity of the periodontal diseases, the distribution of affected teeth, and tooth loss as a function of age. The index is referred to as the burden of periodontal diseases and tooth loss (BPT). RESULTS: A cohort of 1,097 individuals originally seen as new patients in a dental school clinic, and evaluated for undiagnosed dysglycemia, were studied. The BPT index was applied to this data set. A modifying effect of considering the number of lost teeth was observed. The distribution of scores w skewed to the left, which gradually shifted to the right when the most involved teeth (periodontal pathology, tooth loss) were weighted more heavily. This shift was not observed when missing teeth were not considered. CONCLUSIONS: This conceptual study illustrates that the extent and severity of periodontal pathology, and number and distribution of missing teeth, are important considerations when summarizing the condition of the mouth. The BPT provides a measure of the oral disease burden, as both periodontal pathology and tooth loss are associated with both functional impairment and reduced quality of life. The dental profession and dental research community must continually seek to develop new approaches to defining and summarizing the oral disease burden.

BACKGROUND: Health education has a focus on knowledge dissemination and advice giving, which seldom translates into behavioral changes. Motivational interviewing (MI) has potential for helping elderly patients. However, most oral health studies conducted to date have excluded older individuals, and the outcomes examined have been exclusively clinical. This study examines the treatment fidelity of an individual-based MI intervention and assesses baseline characteristics related to older adults' self-efficacy (SE), oral-health-related quality of life (OHRQoL), oral health knowledge, and facilitators, barriers, and confidence on achieving MI goals. METHODS: A convenience sample of 60 seniors was recruited. Participants were obtained through the flow of patients at dental clinics, and received a 45-min individual-based MI education session during a scheduled dental appointment. RESULTS: Regarding MI treatment fidelity, basic proficiency or higher was achieved in all global areas. Significant correlations were found between SE and OHRQoL scores and rating elements of the MI intervention (P = 0.0001). Lack of knowledge related to connections between poly pharmacy and caries risk, and the importance of use of fluorides were evident. CONCLUSION: Findings from the study support the reliability of this new MI intervention. Dental professionals are encouraged to adopt the spirit of MI in their one-on-one patient counseling.

OBJECTIVE: To analyse the cost-effectiveness of a screening programme and follow-up interventions for persons with dysglycemia who are identified during a dental visit. METHODS: This study is a secondary analysis utilizing data from two relevant publications. Those studies identified persons with dysglycemia who were seen in a dental school clinic for routine dental care and determined compliance with a recommendation to seek medical care. The response site was 59.4%. The Archimedes disease simulation model was utilized to simulate the effect of a weight loss programme for identified subjects on several outcomes. RESULTS: Two scenarios for weight loss programmes were considered: a 10% permanent loss in body weight and a 10% loss that decays over time. Both diabetes and prediabetes were analysed. The decay path costs $21 243 per quality adjusted life year (QALY) with 3 years required to achieve the weight reduction. This cost decreases to $6655 if only 1 year is needed to achieve the weight goal. Without decay, the cost per QALY is $15 873 with 20 years of intervention, vs $647 per QALY with 10 years of intervention. For individuals with type 2 diabetes mellitus, the cost per QALY is $48 604 to $56 207 depending on adherence. With the addition of oral medication (a sulfonylurea), the cost is three times higher. CONCLUSIONS: Under the conditions described here, identification of persons with dysglycemia in the dental office for initiating prediabetic care is a cost-effective means of identifying and treating affected individuals.

While sealants are more effective than fluoride varnish in reducing the development of new carious lesions on occlusal surfaces, and a course of treatment requires fewer clinical visits, they are more expensive per application. This analysis assessed which treatment is more cost-effective. We estimate the costs of sealants and fluoride varnish over a 4-year period in a school-based setting, and compare this to existing estimates of the relative benefits in terms of caries reduction to calculate the relative cost-effectiveness of these two preventive treatments. In our base case scenario, varnish is more cost-effective in preventing caries. Allowing for caries benefits to nonocclusal surfaces further improves the cost-effectiveness of varnish. Although we found that varnish is more cost-effective, the results are context specific. Sealants become equally cost-effective if a dental hygienist applies the sealants instead of a dentist, while varnish becomes increasingly cost-effective when making comparisons outside of a traditional dental clinic setting.

OBJECTIVES: The current study evaluates the results of a previously reported method for creating a core medical electronic journal collection for a new medical school library, validates the core collection created specifically to meet the needs of the new school, and identifies strategies for making cost-effective e-journal selection decisions. METHODS: Usage data were extracted for four e-journal packages (Blackwell-Synergy, Cell Press, Lippincott Williams & Wilkins, and ScienceDirect). Usage was correlated with weighted point values assigned to a core list of journal titles, and each package was evaluated for relevancy and cost-effectiveness to the Florida State University College of Medicine (FSU COM) population. RESULTS: The results indicated that the development of the core list was a valid method for creating a new twenty-first century, community-based medical school library. Thirty-seven journals are identified for addition to the FSU COM core list based on use by the COM, and areas of overlapping research interests between the university and the COM are identified based on use of specific journals by each population. CONCLUSIONS: The collection development approach that evolved at the FSU COM library was useful during the initial stages of identifying and evaluating journal selections and in assessing the relative value of a particular journal package for the FSU COM after the school was established.

Streptococcus gordonii is a commensal bacterium that colonizes the hard and soft tissues present in the human mouth and nasopharynx. The cell wall-anchored polypeptides SspA and SspB expressed by S. gordonii mediate a wide range of interactions with host proteins and other bacteria. In this article we have determined the role of SspA and SspB proteins, which are members of the streptococcal antigen I/II (AgI/II) adhesin family, in S. gordonii adherence and internalization by epithelial cells. Wild-type S. gordonii DL1 expressing AgI/II polypeptides attached to and was internalized by HEp-2 cells, whereas an isogenic AgI/II- mutant was reduced in adherence and was not internalized. Association of S. gordonii DL1 with HEp-2 cells triggered protein tyrosine phosphorylation but no significant actin rearrangement. By contrast, Streptococcus pyogenes A40 showed 50-fold higher levels of internalization and this was associated with actin polymerization and interleukin-8 upregulation. Adherence and internalization of S. gordonii by HEp-2 cells involved beta1 integrin recognition but was not fibronectin-dependent. Recombinant SspA and SspB polypeptides bound to purified human alpha5beta1 integrin through sequences present within the NAV (N-terminal) region of AgI/II polypeptide. AgI/II polypeptides blocked interactions of S. gordonii and S. pyogenes with HEp-2 cells, and S. gordonii DL1 cells expressing AgI/II proteins inhibited adherence and internalization of S. pyogenes by HEp-2 cells. Conversely, S. gordonii AgI/II- mutant cells did not inhibit internalization of S. pyogenes. The results suggest that AgI/II proteins not only promote integrin-mediated internalization of oral commensal streptococci by host cells, but also potentially influence susceptibility of host tissues to more pathogenic bacteria.

INTRODUCTION: Transient bacteraemias are frequently detected following dental manipulation. Infective endocarditis (IE) can arise in susceptible individuals and antibiotic prophylaxis is routinely performed for certain procedures considered to be "at risk" of IE. Evidence is emerging that periodontal disease may be a significant risk factor for the development of certain systemic diseases such as cardiovascular disease. These systemic conditions could be initiated or detrimentally influenced by the repeated entry of bacteria into the bloodstream. MATERIALS AND METHODS: The present study comprised a single blind parallel study of 2 weeks duration. A baseline blood sample was obtained from 30 volunteers with untreated periodontal disease following which a periodontal probing depth chart was collected. A further blood sample was taken following this procedure, and each subject was recalled 2 weeks later. A blood sample was collected, the subject carried out toothbrushing and a further blood sample taken. Full-mouth ultrasonic scaling was then performed and a final blood sample taken. Blood samples were analysed for bacteraemia using conventional microbiological culture and polymerase chain reaction (PCR) using universal bacterial primers that target the 16S ribosomal RNA gene of the vast majority of bacteria. RESULTS: Using culture methods, the incidence of bacteraemias was as follows: following ultrasonic scaling (13%), periodontal probing (20%) and toothbrushing (3%). PCR analysis revealed bacteraemia incidences following ultrasonic scaling, periodontal probing and toothbrushing of 23%, 16% and 13%, respectively. CONCLUSION: These findings suggest that detectable dental bacteraemias induced by periodontal procedures are at a lower level than previously reported.

INTRODUCTION: This study was undertaken to test the hypothesis that male, non-smokers, aged 25-50 years, with a proven track record of developing gingivitis would provide suitable subjects for experimental gingivitis studies because the inter-subject variation would be greatly reduced. MATERIALS AND METHODS: Subjects were required to be men aged between 25 and 50 years, in good general health with at least four sound teeth in one posterior quadrant of the mandible. Subjects who were smokers, taking antibiotics, anti-inflammatory drugs or had any other medical, surgical or social condition making participation in the study inadvisable, were excluded. Subjects had previously participated in a 21-day experimental gingivitis study. Subjects with the most gingivitis at the end of the previous study were invited to participate first, moving down the list until the required number of subjects had been achieved. Subjects were randomly assigned to one of three toothpastes: Crest Gum Care, Mentadent P or a placebo (UK Signal). Subjects were asked to brush their teeth twice daily for 1 min., with the tooth-shield in place and containing 1 ml of their assigned toothpaste. RESULTS: Thirty-five subjects completed the study using the placebo, 36 used Mentadent P and 39 completed the study using Crest Gum Care. Both toothpastes containing active gum health ingredients (Triclosan and Zinc Citrate in Mentadent P and Stannous Fluoride in Crest Gum Care) resulted in statistically significant reductions in gingival bleeding and inflammation. Although a directional reduction, there was no statistically significant reduction in plaque scores. No significant differences between the two active groups were established. CONCLUSION: The factors that were controlled or measured and analyzed in these studies were: gender, smoking status, age and chronic gingivitis level. The results add support to the hypothesis that male subjects show less variation, however there is limited evidence linking the female menstrual cycles to variations in gingivitis status. Further research in this area is required before definitive conclusions can be made. Selecting a subject population in order to decrease variation must be undertaken with caution. A subject population selected for particular attributes can be extremely valuable when assessing the anti-gingivitis potential of new agents, but this population would be as biased if used for large efficacy clinical trials. The data obtained will not necessarily be able to be generalized, that is, applicable to the whole population. However there is sufficient evidence to justify balancing for (or excluding) smokers from subject populations in experimental gingivitis studies.

The Florida State University (FSU) College of Medicine Medical Library is the first academic medical library to be established since the Web's dramatic appearance during the 1990s. A large customer base for electronic medical information resources is both comfortable with and eager to migrate to the electronic format completely, and vendors are designing radical pricing models that make print journal cancellations economically advantageous. In this (almost) post-print environment, the new FSU Medical Library is being created and will continue to evolve. By analyzing print journal subscription lists of eighteen academic medical libraries with similar missions to the community-based FSU College of Medicine and by entering these and selected quality indicators into a Microsoft Access database, a core list was created. This list serves as a selection guide, as a point for discussion with faculty and curriculum leaders when creating budgets, and for financial negotiations in a broader university environment. After journal titles specific to allied health sciences, veterinary medicine, dentistry, pharmacy, library science, and nursing were eliminated from the list, 4,225 unique journal titles emerged. Based on a ten-point scale including SERHOLD holdings and DOCLINE borrowing activity, a list of 449 core titles is identified. The core list has been saved in spreadsheet format for easy sorting by a number of parameters.

RESUMO

This article introduces a series of articles examining the state of the medical library profession as practiced in the clinical context. It is widely understood that many changes across the spectrum of medical librarianship practice have been brought about by both technological advances and economic realities. These changes have created strains felt by many in the profession. Discussions of evolving roles for medical librarians that have gone on for years have taken on a new sense of urgency, not just because support of library services is at stake, but also because new opportunities, which many are eager to explore, await librarians. In June 2000, an editorial appearing in a mainstream medical journal proposed a reinvention of clinical librarianship that, if designed as presented in the editorial, would have a dramatic effect on current hospital-based library practice. This series of articles was developed in an effort to provide thoughtful consideration of the "informationist" model and to present new ways to look at the core competencies that define the profession.
