STATEMENT OF PROBLEM: Adequate plaque control is essential for the maintenance of a healthy oral mucosa. Denture cleansers are a popular method used by denture wearers for cleaning. The clinical effectiveness of a new cleanser in tablet form has not been tested in vivo. Methods used for the in vivo assessment of denture cleanser effectiveness are often not standardized. PURPOSE: The aims of this in vivo study were to develop a new method for assessing plaque levels on dentures by using digital imaging and to use this methodology, together with a validated visual scoring method, to evaluate plaque buildup on dentures. MATERIAL AND METHODS: Thirty-five subjects were randomized to 1 of 2 treatment groups: (1) dentures soaked in a new cleanser, or (2) dentures soaked in water. The treatment period lasted 14 days. Plaque was assessed at baseline and on days 2 and 14 by using digital image analysis and a visual scoring technique. Results were analyzed with ANOVA at the 95% confidence interval. RESULTS: The new cleanser showed a statistically significant reduction in mean visual plaque of 51% at day 2 (P = 0.0002) and 42% (P = 0.0014) at day 14 compared with water. Similar results were seen with digital imaging. CONCLUSION: The new denture cleanser proved to be effective at preventing plaque accumulation on dentures. The analysis of digital images gave similar results as the visual scoring method.

RESUMO

We report the deliberate ingestion of a superwarfarin product, brodifacoum, in a 39-y-old male. He presented with prothrombin and partial thromboplastin times of 150 and 113 sec, respectively. His coagulopathy was corrected by administration of blood products and phytonadione. To maintain normal clotting studies this patient required the highest maintenance dose of phytonadione, 200 mg/d, reported to date. The patient was able to tolerate this dose for 5 mo without adverse effects.

Bupropion (Wellbutrin; Burroughs Welcome Co, Research Triangle Park, NC) is a unique monocyclic antidepressant about which there is limited overdose information. A retrospective analysis of all bupropion ingestions reported to five regional poison control centers from 1989 through 1991 was conducted. There were 58 cases of bupropion ingestion and nine cases of combined bupropion and benzodiazepine ingestion. Sinus tachycardia was the only toxic cardiovascular effect noted, except for one case of hypotension in the bupropioin and benzodiazepine group. Neurological toxicity was commonly encountered and included lethargy, tremors, and seizures. Both benzodiazepines and phenytoin were efficacious in controlling seizures. Five cases of pure bupropion overdose had electrolytes reported. Serum potassium ranged from 2.6 to 4.2 mEq/L (mean, 3.3 mEq/L). In overdose, bupropion seems to lack major cardiovascular toxicity; however, it does manifest significant neurological toxicity.
