RESUMO

The aim of the research was to study the pathomorphosis of syphilis under modern conditions. The morbidity, clinical and epidemiological peculiarities of syphilis were analyzed both domestically (in Ukraine) and internationally. It has been established that in the pattern of syphilis morbidity, latent forms vary between 20% and 40%. Latent syphilis is detected in almost a half of the elderly patients. There has been a marked increase in the incidence of syphilis among homosexualists, prostitutes, alcoholics and drug addicts. In Ukraine from 2008 to 2014 there was a general decrease in the incidence of all forms of syphilis by 2.5 times, but the disease patterns are also changing - the number of latent diseases, late and unspecified forms with particular damage to the nervous system, visceral organs significantly increase. In examining the immunogenetic features of syphilis in the Eastern region of Ukraine, types a, b, c, d, g, i, p of the tp gene were found. Five types of arp T. pallidum gene were identified. The main aim of T. Pallidum detection in the samples is PCR, during late and latent forms of TPHA. Penicillin drugs remain the main plank of the therapy. Thus, in the case of syphilis, nosological entity and clinical disease evidence have been blurred. To ensure the prompt diagnosis and treatment of the disease, it is necessary to consider serology, immunogenetic peculiarities, neurological, and therapeutic status of the patient.

Indices of congenital leukocyte elastase (LE) activity and adaptive immunity (a level of autoantiboies to nerve growth factor--AabNGF) in blood serum of patients with schizophrenia (attack-like, continuous and slow progressive types) and schizoaffective psychosis have been compared with clinical presentations of the disorders. A patient's state was assessed by clinico-psychopathological methods and with the Positive and Negative Syndromes Scale (PANSS). All schizophrenia types and schizoaffective psychosis were accompanied by LE activity elevation. An increase of AabNGF level was observed only in attack like and continuous schizophrenia. Correlations between AabNGF level and negative symptoms evaluated, using the PANSS, suggest a relation of autoimmune reactions against NGF to the progression of schizophrenic process. Differences in AabNGF level between schizoaffective psychosis and attack-like schizophrenia confirm nosologic independence of schizoaffective psychosis and demand for additional differential laboratory diagnosis of these disorders.

Native Chukot Peninsula residents, in contrast to Muscovites, consume a diet rich in n-3 polyunsaturated fatty acids. This dietary peculiarity is reflected in differences in plasma lipid and apolipoprotein contents. The Chukot residents have lower contents of total cholesterol, triglyceride, LDL (low density lipoprotein) cholesterol and apolipoprotein B, but higher HDL (high density lipoprotein) cholesterol levels than do Muscovites. The apolipoprotein A-I levels were identical in both groups. A higher HDL cholesterol to apolipoprotein A-I ratio was determined in the coastline Chukot residents (0.52 +/- 0.01) than in Muscovites (0.43 +/- 0.01; p less than 0.01). In contrast to Muscovites, the coastline Chukot residents also had higher n-3 and lower n-6 polyunsaturated fatty acid percentages in plasma and erythrocyte lipids, and lower phosphatidylcholine and higher sphingomyelin or phosphatidylethanolamine levels in HDL2b and HDL3. The higher HDL cholesterol levels in the plasma of the coastline Chukot residents appears to reflect the higher cholesterol-scavenging capacity of their HDL. We conclude from this study that the regular consumption of dietary n-3 polyunsaturated fatty acids by the coastline Chukot residents decreased LDL cholesterol transfer from plasma to peripheral cells, and enhanced cholesterol efflux from cellular membranes toward HDL.

Accumulation of rhodamine B isothyocyanate-conjugated low density lipoproteins (R-LDL) in cultured endothelial cells from human umbilical cord was studied with a fluorescence activated cell sorter. R-LDL uptake was blocked at 0 degrees C, inhibited by addition of excess of nonlabeled low density lipoprotein, high density lipoprotein-2 or high density lipoprotein-3. High density lipoprotein-2 was about twice as effective in inhibition of R-LDL uptake as high density lipoprotein-3. Endothelial cells that formed a contact-inhibited monolayer lost the ability to incorporate R-LDL via a receptor-mediated pathway. Using R-LDL, it was possible to distinguish cells with different levels of R-LDL incorporation.
