Epizootological, serological, and molecular virological analysis of an outbreak of hemorrhagic fever with renal syndrome (HFRS) in the Egoryevsk district of Moscow region (September 1995-January 1996) has been carried out. Hantavirus (Puumala) antigen and virus-specific antibodies were isolated from bank voles captured in the endemic focus. Anti-Puumala antibodies were detected in the sera of all HFRS patients and in 2% healthy residents of the endemic focus. Analysis of nucleotide sequence (RNA from hantavirus-positive lung of a bank vole) showed that the studied hantavirus is a distinct genotype of Puumala virus. Hence, a new highly active natural focus of HFRS associated with Puumala virus, dangerous for the population, has been revealed in Moscow region.

Influenza polymer-subunit vaccine "Grippol", made up of the sterile conjugate of surface proteins of influenza viruses, groups A and B, with polyoxidonium (copolymer), was obtained at the State Scientific Centre of Research Institute of Immunology. As the result of our investigations, the coefficient of prophylactic effectiveness of vaccine "Grippol" was found to be 71%; in the presence of the 50% immune stratum in the group the coefficient of antiepidemic protection was found to be 66.4%. At the same time, cases of influenza and acute respiratory diseases were shown to take a milder form in persons immunized with vaccine "Grippol" than in the group of nonimmunized subjects. The results of this investigation did not show any side effects caused by the vaccine.

Peculiarities of modern influenza and other acute respiratory diseases in the units make medical service to execute epidemiologic supervision of these infections, especially in relation of newly formed collectives and troop contingency during there moving to other regions of the country. The medical service should apply agents of specific prophylaxis of influenza in groups of increased risk during threat of occurrence of an epidemic of influenza. It is necessary to contain a sufficient reserve of influenza vaccines for this purpose. At occurrence of pandemic strain of virus of influenza tactics and measures of prophylaxis should be revised with account of new epidemiologic situation.

Enzyme immunoassay with biotin-labeled monoclonal antibodies to Puumala virus was used for typing hantavirus antigens. The system is type-specific, that is, it detects Puumala virus only, which was demonstrated by typing 9 hantavirus antigens in infected Vero E5 cells and by epizootological analysis of an HFRS outbreak in the Yegoryevsk district of the Moscow region in November-December 1995. The suggested system may be used as an element of EIA screening of natural samples in analysis of the screening results.

Clinical trials of inactivated hepatitis A vaccine Hep-A-in-Vac demonstrated its specific safety and moderate reactogenicity, manifesting by short-term fibrillar twitching of musculus deltoideus at the site of injection. After a course of three immunizations 87.5% seronegative vaccines developed a high level of specific antibodies to hepatitis A virus with at least 100 reverse values of titers. In controls antibody titers remained seronegative in 90% cases. These data indicate evident immunologic activity of Hep-A-in-Vac vaccine.

Altogether 162 cases of tonsillitis were registered in two military units during the period of May 11-16. The disease took an acute course with short-time fever, symptoms of acute intoxication, sore throat, pronounced inflammatory changes in tonsils and swelling of regional (submaxillary and anterocervical) lymph nodes. In some of the patients (1.1%) sickness and vomiting and in 0.2% diarrhea were registered. In 6.7% of the patients scarlatiniform eruptions were observed on days 2-3 of the disease. In the course of the bacteriological examination of the patients group A streptococci, serovar T II, were isolated. The dynamic study of paired sera showed a considerable increase in the number of patients with a high level of antibodies to streptolysin 0 and group A polysaccharide. All patients were fed at the same canteen. The factor of the transmission of this infection could be butter, stored without observation of the required temperature conditions and apportioned by the soldiers of the kitchen police. Experimental study revealed that group A streptococci are capable for proliferation and accumulation in butter.
