RESUMO

PURPOSE: This retrospective cohort study was to assess the prognostic value of preoperative geriatric nutritional risk index (GNRI) on survival outcomes for patients with locally advanced oral squamous cell carcinoma (LAOSCC). METHODS: Patients with LAOSCC receiving upfront radical surgery at a single institute from January 2007 to February 2017 were enrolled. The primary outcomes in the study were 5-year overall survival (OS) and cancer-specific survival (CSS) rates, and a nomogram based on GNRI and other clinical-pathological factors was established for individualized OS prediction. RESULTS: There were 343 patients enrolled in this study. The optimal cut-off value of GNRI was observed to be 97.8. Patients in the high-GNRI group (GNRI ≥97.8) had statistically significantly better outcomes in 5-year OS (74.7% vs. 57.2%, p = 0.001) and CSS (82.2% vs. 68.9%, p = 0.005) when compared with the low-GNRI group (GNRI <97.8). In Cox models, low GNRI remained an independent negative prognosticator of OS (HR: 1.6; 95% CI: 1.124-2.277; p = 0.009) and CSS (HR: 1.907; 95% CI: 1.219-2.984; p = 0.005). The c-index of the proposed nomogram, incorporating assorted clinicopathological factors and GNRI, had a statistically significant increase compared with the predictive nomogram constructed by the TNM staging system alone (0.692 vs. 0.637, p < 0.001)." CONCLUSION: Preoperative GNRI is an independent prognostic factor of OS and CSS in patients with LAOSCC. A multivariate nomogram that includes GNRI may better help us to accurately estimate individual survival outcomes.

RESUMO

The oxidative cleavage of C═C bonds with molecular oxygen was promoted effectively by a catalytic amount of a decatungstate photocatalyst using black light irradiation (365 nm). Not only aromatic ketones but also aliphatic ketones were obtained by the photocatalytic protocol. The continuous flow reaction of α-methylstyrene using a high-power ultraviolet light-emitting diode (365 nm) dramatically decreased the reaction time.

Spatially organized molecular interactions are fundamental features underlying many biochemical processes in cells. These spatially defined reactions are essential to ensure high signaling specificity and are indispensable for maintaining cell functions. The construction of synthetic cell models that can resemble such properties is thus important yet less investigated. In this study, we present a reliable method for the rapid production of highly uniform phase-separated liposomes as synthetic cell models. Specifically, a microfluidics-based strategy coupled with custom reagents for generating size-tunable liposomes with various lipid compositions is presented. In addition, an important cell signaling interacting pair, the pleckstrin homology (PH) domain and PIP2 lipid, is used to demonstrate the controlled molecular assembly inside these liposomes. The result shows that PIP2 on phase-separated domains successfully recruits the PH domains to realize spatially defined molecular interactions. Such a system is versatile and can be expanded to synthesize other proteins for realizing multiplexed molecular interactions in the same liposome. Phase-separated lipid domains can also be used to recruit targeted proteins to initiate localized reactions, thus paving the way for organizing a complex signaling cascade in the synthetic cell.

RESUMO

Unprecedented chemo- and regioselective synthesis of benzo[a]fluorenes and naphthamide-substituted benzo[a]fluorenes were constructed from the reaction of (E)-2-aroyl-3-(2-(arylalkynes/alkenes)aryl)acrylonitrile scaffolds under metal-free conditions via the activation of nitriles and alkenes, respectively. A tentative reaction mechanism was proposed for this homofunctionalization of nitriles. Control experiments showed that the reaction proceeds via selective nitrile or alkene protonation, depending upon the substrates. Additionally, we demonstrated an alternative expeditious route for the synthesis of disubstituted benzo[a]fluorenes in the presence of TfOH alone.

RESUMO

Surgical emphysema is an uncommon but potentially serious complication of third molar removal and other oral procedures. The purpose of this case report is to remind dentists of the risk of surgical emphysema. Surgical emphysema developed in a 32-year-old female after removal of a mandibular third molar using a high-speed dental hand-piece. Because of the extent of the swelling, the patient was admitted for observation. Because of progressive swelling, a submandibular surgical incision and drainage was performed, which led to rapid resolution. The diagnosis and differential diagnosis of surgical emphysema are discussed to draw dentists' attention to the management and prevention of this complication.
