RESUMO

A meeting of Interdisciplinary Expert Panel with leading specialists in the field of orthopedics/traumatology, surgery, rheumatology, and neurology was held in Moscow on February 10, 2023. The purpose of the meeting was to discuss the current status of local injection therapy (LIT) in Russia and the rationale behind the use of collagen-based products for various musculoskeletal disorders. The experts considered the following issues: (1) General contraindications to the use of medical products based on tropocollagen as well as an algorithm for actions in case of adverse events; (2) Guidelines regarding LIT in general and LIT using tropocollagen in particular, including in combination with other LIT products; (3) Particular indications and approaches to the treatment of patients with abnormal changes in appendicular joints and spine with damage to both intra-articular structures and periarticular soft tissue.

The purpose of the study was to reveal gender-related dimorphism of changes in adhesive properties of the vascular wall in the dynamics of surgical treatment of chronic periodontitis (CP). A total of 51 persons aged 24 to 39 years were examined, of which 20 were clinically healthy volunteers (10 men and 10 women) and 31 were patients with CP of moderate severity (14 men and 17 women). It has been shown that endothelial dysfunction is more pronounced in men than in women with CP. Surgical treatment of CP is accompanied by a reactive increase in the concentration of soluble forms of adhesive endothelial molecules in early postoperative period more pronounced in women. In the long-term period surgical treatment causes a decrease in the impairment of the endothelial adhesive properties in men to a lesser extent than in women.

AIM: To assess the efficacy and safety of naproxen in patients with nonspecific low back pain. MATERIAL AND METHODS: Ninety patients with nonspecific low back pain were enrolled in the study. Patients took 550 mg of naproxen twice a day. All patients were assessed with VAS, the Oswestry questionnaire, Schober's test and clinical global impression scale. The treatment lasted from 7 to 14 days depending on the pain relief (VAS ≤10 mm). RESULTS: The pain syndrome relief was observed in 77 patients (88.5%) during the first week of the treatment and in 81 (93.1%) by the end of the study. The average value of VAS was reduced by 6.2 times (by 52.9 mm) in comparison to the baseline. According to the Oswestry questionnaire the influence of pain syndrome on the life quality was reduced by 4,78 times in comparison to the baseline. Schober's test revealed an increase in the amplitude of movements during therapy by 27% in comparison to the baseline. Only 5 (5.7%) drug-related adverse reactions were observed during the whole study, 80% of those were mild. CONCLUSION: Naproxen in the dose of 550 mg twice a day demonstrates the high efficacy and safety in patients with non-specific pain in lumbosacral spine.

An important factor in the pathogenesis of chronic inflammation in periodontitis is endothelial dysfunction (ED). Adhesion molecules are markers of impaired barrier function, adhesive properties and vascular permeability. The study of the concentration of soluble adhesion molecules is very promising in the diagnosis of ED. The purpose of this research was to study changes in the concentration of soluble forms of adhesive molecules of the selectin family and the immunoglobulin superfamily when used as markers of systemic manifestations of ED in the dynamics of the treatment of chronic generalized periodontitis (CP) using surgical and therapeutic schemes. 60 patients with CP (33 women and 27 men) and 20 clinically healthy volunteers (10 men and 10 women) were examined. The state of the endothelium was assessed by the content in the serum of soluble forms of adhesive molecules - sP- and sE-selectins, intercellular adhesive molecules of type 1 (sICAM-1), vascular molecules of cell adhesion of type 1 (VCAM-1) by ELISA. The short-term results of non-surgical treatment of CP are more effective in correction of vascular wall adhesive properties disorders compared with surgical treatment. Conservative treatment of chronic hepatitis C effectively adjusted the blood content of adherent endothelium molecules in women than in men. The long-term results of applying this scheme were characterized by partial preservation of a positive effect on the adhesive properties of the endothelium, however, long-term results were inferior to short-term ones. Surgical treatment of CP in women in the early postoperative period was accompanied by reactive impairment of the adhesive properties of the endothelium. The long-term results of this treatment regimen testified to its greater effectiveness. In women, the effectiveness of treatment of CP in restoring the adhesive properties of the endothelium was higher than in men. The results obtained allow us to conclude that the systemic manifestations of ED in CP were characterized by an increase in the concentration in the blood of soluble forms of adhesion molecules. The dynamics of the concentration of adhesive molecules during the treatment of СР testified to the dependence of the sex of patients on the effectiveness of the influence of therapeutic and surgical treatment regimens on the systemic manifestations of ED.

Polyribosomes in cells usually have a certain structural organization whose significance has not yet been elucidated. The development of cryo electron tomography has provided a new approach to study polyribosome structure. New data confirm or correct observations made earlier by classical techniques of electron microscopy. The existence of circular and linear (zigzag) topology of polyribosomes was confirmed, and their relationship with the frequently observed two-row forms was clarified. Contacts between ribosomes have been identified in densely packed three-dimensional helical polyribosomes. At the same time, modern cell-free translation systems have opened the possibility of investigating polyribosomes on mRNA of a given structure to elucidate the mechanism of polyribosome structure formation, especially of circular polyribosomes. There is an increasing amount of data supporting the idea of interdependence between polyribosome structure and their translational activity. Moreover, participation of polyribosomes in mRNA transport and localization of protein synthesis in the cell has been shown. Improvement of the resolution and the development of the cryo electron tomography technique for the analysis of polyribosomes in situ will enable further progress in understanding the process of protein synthesis in cells.

AIM: To study an effect of neuromidin on the changes in electroneuromyographic (ENMG) parameters in patients with carpal tunnel syndrome in the conditions of artificial decompression of a forearm. MATERIAL AND METHODS: Sixty-four 64 patients, 46 women and 18 men, mean age 50,7±4,5years, matched for disease severity were studied. The patients were randomized into neuromidin group and control group. Neuromidin was introduced using subcutaneous local perineural injections. Characteristics of neuromidin effect on the changes in ENMG parameters of the median nerve were studied in a model of additional artificial decompression of the median nerve. RESULTS AND CONCLUSION: There was the significant depression of the nerve function and worsening of ENMG parameters during the additional decompression as well as the aggravation of conduction block along nerve fibers (motor and sensory). In patients treated with ipidacrine, there was an increase in the amplitude of sensory and (to a less extent) of M-response that indicated the activation of axonal transport and reservation of functioning of the compressed nerve.

We studied the influence of the induced strain and applied electric field on the ground state of ferroelectric Ba0.7Sr0.3TiO3 thin films, deposited on the cubic (0 0 1) substrate. The dependence of the pyroelectric coefficient on the applied field is calculated for the different values of the induced strain. We found that tuning of the misfit strain in the film under the dielectric bolometer mode by the proper selection of substrate makes it possible to create the structures with very large values of the pyroelectric coefficient.

AIM: To develop the algorithm of early diagnosis of carpal tunnel syndrome (CTS) at the stage of functional neurological disturbances by expanding diagnostic possibilities of electroneuromyography using artificial compression test. MATERIAL AND METHODS: Parameters of conductivity of the median nerve in 54 patients with finger numbness were analyzed during 3 months before and after compression of the forearm (blood pressure was measured for 1 min). RESULTS AND CONCLUSION: An increase in the latency in motor fibers and a decrease in the amplitude of sensory response were identified in patients with CTS signs and normal electroneuromyographical parameters at baseline. The use of additional electroneuromyographical monitoring during the provocative artificial compression test expands the possibilities of this method and improves early diagnosis of CTS.

A phenomenological thermodynamic theory of uniaxial relaxor strontium barium niobate [Formula: see text] is developed using the Landau-Devonshire approach with two order parameters. The fourth-order thermodynamic potential allowed to explain the shape of the polarization hysteresis loops experimentally observed at different temperatures. We show that the broad maximum of the dielectric permittivity is not related to the phase transition and arise due to the coupling between polarization and true order parameter which has antiferroelectric nature. We found that the phase transition temperature is much higher than the maximum of the dielectric permittivity and very likely corresponds to so-called Burn's temperature. True order parameter has no simple relation with polar modes.

Anion ordering in the structure of cubic perovskite has been investigated by the group-theoretical method. The possibility of the existence of 261 ordered low-symmetry structures, each with a unique space-group symmetry, is established. These results include five binary and 14 ternary anion superstructures. The 261 idealized anion-ordered perovskite structures are considered as aristotypes, giving rise to different derivatives. The structures of these derivatives are formed by tilting of BO6 octahedra, distortions caused by the cooperative Jahn-Teller effect and other physical effects. Some derivatives of aristotypes exist as real substances, and some as virtual ones. A classification of aristotypes of anion superstructures in perovskite is proposed: the AX class (the simultaneous ordering of A cations and anions in cubic perovskite structure), the BX class (the simultaneous ordering of B cations and anions) and the X class (the ordering of anions only in cubic perovskite structure). In most perovskites anion ordering is accompanied by cation ordering. Therefore, the main classes of anion order in perovskites are the AX and BX classes. The calculated structures of some anion superstructures are reported. Comparison of predictions and experimentally investigated anion superstructures shows coherency of theoretical and experimental results.

RESUMO

The native population of the Arctic char in the Lake Mushtalampi was studied. The dwarf form of the Arctic char inhabiting the lake was revealed. The fish is characterized by extremely low growth rate and by early maturation. Twelve parasite species were found in the arctic char. The parasite fauna is typical of representatives of Salmonidae. Distinguishing features of this fauna include the absence of narrowly specialized char parasite species. High intensity of infection with nematodes Philonema oncorhynchi allows estimating the epizootic state of the studied char population as insufficient.

Group-theoretical and thermodynamic methods of the Landau theory of phase transitions are used to investigate the hyper-kagome atomic order in structures of ordered spinels and a spinel-like Na4Ir3O8 crystal. The formation of an atom hyper-kagome sublattice in Na4Ir3O8 is described theoretically on the basis of the archetype (hypothetical parent structure/phase) concept. The archetype structure of Na4Ir3O8 has a spinel-like structure (space group Fd\bar 3m) and composition [Na1/2Ir3/2](16d)[Na3/2](16c)O(32e)4. The critical order parameter which induces hypothetical phase transition has been stated. It is shown that the derived structure of Na4Ir3O8 is formed as a result of the displacements of Na, Ir and O atoms, and ordering of Na, Ir and O atoms, ordering dxy, dxz, dyz orbitals as well. Ordering of all atoms takes place according to the type 1:3. Ir and Na atoms form an intriguing atom order: a network of corner-shared Ir triangles called a hyper-kagome lattice. The Ir atoms form nanoclusters which are named decagons. The existence of hyper-kagome lattices in six types of ordered spinel structures is predicted theoretically. The structure mechanisms of the formation of the predicted hyper-kagome atom order in some ordered spinel phases are established. For a number of cases typical diagrams of possible crystal phase states are built in the framework of the Landau theory of phase transitions. Thermodynamical conditions of hyper-kagome order formation are discussed by means of these diagrams. The proposed theory is in accordance with experimental data.

This review presents the advanced data about the role endothelial dysfunction in the pathogenesis of inflammatory periodontal diseases. This article describes the mechanisms of endothelial cell damage. Special attention is paid to the role of endothelial dysfunction in the pathogenesis of local microcirculatory changes at inflammatory periodontal diseases. The modern notions of systemic endothelial dysfunction at periodontal tissues inflammation as a risk factor for several diseases.

Group-theoretical methods of the Landau theory of phase transitions are used to investigate the structures of ordered spinels. The possibility of the existence is determined of 305 phases with different types of order in Wyckoff position 8a (including seven binary and seven ternary cation substructures), 537 phases in Wyckoff position 16d (including eight binary and 11 ternary cation substructures), 595 phases in Wyckoff position 32e (including seven binary and four ternary anion substructures) and 549 phases with simultaneous ordering in Wyckoff positions 8a and 16d (including five substructures with binary order in tetrahedral and octahedral sublattices, two substructures with ternary order in both spinel sublattices, and nine substructures with different combined types of binary and ternary order). Theoretical results and experimental data are compared. Calculated structures of the spread types of ordered low-symmetry spinel modifications are given.

RESUMO

The review presents current data concerning the functional role of endothelial cell adhesion molecules belonging to different structural families: integrins, selectins, cadherins, and the immunoglobulin super-family. In this manuscript the regulatory mechanisms and factors of adhesion molecules expression and distribution on the surface of endothelial cells are discussed. The data presented reveal the importance of adhesion molecules in the regulation of structural and functional state of endothelial cells in normal conditions and in pathology. Particular attention is paid to the importance of these molecules in the processes of physiological and pathological angiogenesis, regulation of permeability of the endothelial barrier and cell transmigration.

Nine patients were operated on with the use of robotic complex. In 3 cases the nonorganic retroperitoneal pelvic tumor was removed, the 4 full-layer urinary bladder and ureteral orifice resections and simultaneous ureteral plasty together with ileal-obturatory lymphadenectomy (2 - cancer of the urinary bladder cervix, 1 - pheochromocytoma of the urinary bladder) and 2 ureterocele with complete ureter duplication and megaureter were performed. Technical aspects of the robotic layerwise resection of the neck of the urinary bladder were worked out. The operation time varied from 1 hour 45 minutes to 4 hours 45 minutes. The maximal blood loss was 100 ml. One patient developed the acute respiratory failure. The were no other postoperative complications. The postoperative hospital stay was 7-12 days.

In the process of protein synthesis, the translating ribosomes of eukaryotic cells form polyribosomes that are found to be multiplex functional complexes possessing elements of ordered spatial organization. As revealed by a number of electron microscopy studies, the predominant visible configurations of the eukaryotic polyribosomes are circles (circular polyribosomes) and two-stranded formations (so-called double-row polyribosomes). The "long" (i.e. heavy loaded) polyribosomes are usually represented by double-row structures, which can be interpreted as either topologically circular ("collapsed rings"), or topologically linear (zigzags or helices). In the present work we have analyzed the mRNA path within the eukaryotic polyribosomes, isolated from a wheat germ cell-free translation system, by integrating two approaches: the visualization of mRNA ends in polyribosomes by marking them with gold nanoparticles (3'-end) and initiating 40S subunits (5'-end), as well as by the cryoelectron tomography. Examination of the location of the mRNA markers in polyribosomes and mutual orientation of ribosomes in them has shown that the double-row polyribosomes of the same sample can have both circular and linear arrangements of their mRNA.

RESUMO

Perivascular epithelioid cell tumors is a group of rare tumors composed of histologically and immunohistochemically distinctive perivascular epithelioid cells. There are no specific imaging sighs. In most cases, the final diagnosis is possible only after the immunihistochemical study. These tumors are extremely rare. We present imaging findings of two cases of perivascular epithelioid cell tumors of the liver.
