RESUMO

The article presents the proportion of different categories of patients with schizophrenia performing socially dangerous actions (SDA) in the total number of registered patients. It describes features of the disease, sex, age specific for clinical presentation background. Complex evaluation of factors, including premorbid of personality traits and environmental phenomena is considered to be very important because of its prognostic significance. In cases with less acute and less progressive course of the disease significance of social and psychological factors increases. Difficulties in up-to-date detection of patients, inclined to SDA, are analysed.

RESUMO

A comparative epidemiological study covered two groups of patients (n = 4811), aged 55 years and over, diametrically differing with regard to the origin of the disease: (1) schizophrenic patients and patients with mental disturbances in the long-term period of craniocerebral trauma. The authors consider age-specific peculiarities of the rate and structure of antisocial actions, as well as their relationship with some clinical, social, and demographic characteristics of the patients.

RESUMO

The earlier detected factor determining the disadaptive behaviour of mental patients formed a basis for grouping children with a high risk of pathological behavioural disorders. The authors conducted a clinico-psychological and sociological study of one such high risk group composed of young children with ante-and perinatal damage to the brain who came from socially disadaptive families. Thirty children and their parents were studied. A pathoplastic effect of a disharmonic familial setting and a distorted nature of the interaction in the mother-child system on the clinical picture including manifestations of encephalopathy, retardation of mental development, specific disorders of communication and the first signs of behavioural disturbances was elucidated. A conclusion was drawn about the advisability of psychotherapeutic, psychocorrective and therapeutic-pedagogical work with such children in a special children institution.

RESUMO

An epidemiological examination of a representative group of 334 patients with late sequelae of craniocerebral injuries was carried out. The patients were selected from those registered at psychoneurological dispensaries of a large city. On the basis of the examination results various types of the course of the post-traumatic psychic disorders were specified, and so were dominating syndromes (asthenic, psychopathic, deficit-type and endomorphous). The peculiarities of their clinical picture formation and time course are shown with indication to factors playing the pathogenetic role. The importance of some psychopathological complexes in the genesis and prognosis of the patients antisocial behaviour is noted. Recommendations concerning medical and social prophylactic and therapeutic measures are substantiated.

Data obtained from epidemiological studies carried out in some large administrative regions have shown that among the causes of socially dangerous actions committed by schizophrenics the syndromes with prevalent delusional and hallucinatory-delusional disorders take the second (after psychopathy-like states), and, as regards acts of violence, the first place. Practically all the phenomena of a delusional or a hallucinatory-delusional syndrome may be the motives of the socially dangerous actions. However, the occurrence and character of those actions are different, these differences being associated with particular psychopathological features of the hallucinatory-delusional syndromes. The knowledge of those peculiarities may contribute to early recognition of such patients and to taking adequate curative and rehabilitation measures aimed at preventing the dangerous actions.

On the basis of an epidemiological multifactorial comparative study of schizphrenic patients who committed and did not commit socially dangerous acts, in several large administrative territories it was possible to detect a distinct correlation of different types of dangerous acts with clinical symptoms of the disease. The study contains a list of syndromes and their variants conjugated with different frequencies of dangerous acts. It was established that schizophrenic patients most frequently commit dangerous acts when there is a predominance of psychopathlike disorders in the clinical picture, mainly of a polymorphic and explosive character. It was demonstrated that this circumstance should be taken into consideration along with other factors, in a comprehensive assessment of the risk of a commitment of socially dangerous acts by patients.

RESUMO

A statistical analysis of a multifactorial comparative epidemiological study of schizophrenic patients performing and not performing dangerous actions during the period of the disease detected the following: psychological signs of schizophrenia were significantly more frequently seen at different stages of the disease in these patients who performed dangerous acts. There was a certain correlation between specific clinical signs and the age and sex structure, "prepsychotic" personality properties, exogenous influences. It was also possible to demonstrate a certain correlation between the frequency of dangerous acts and clinical conditions.

RESUMO

The report is based on a statistical analysis of a reregistration of mental patients on the territory of a large city. The authors systematized and analyzed (with the aid of a computer) data concerning the age and sex structure and the special traits of a socio-working adaptation of schizophrenic patients who committed socially dangerous acts during the period of their illness. These data were correlated with similar ones in respect to all the registered schizophrenic patients. It was demonstrated that among the patients who committed dangerous acts in the past there was a prevalence of males (on the average 4.3 times more frequently), while in the general population of schizophrenic patients there is only a 23.8% increase among the males, as compared to females. Those who committed such acts had a lower educational census and worse indices of working capacity which have their specificity in different age groups for males and females. The indices of working capacity correlate (a feed back) with the frequency of dangerous acts. The authors are of the opinion that the obtained information may be used for a wide-range evaluation of the possible risk of dangerous acts committed by schizophrenic patients.
