RESUMO

The brain and viscera of 19 persons who had died from acute closed craniocerebral trauma were studied histologically (survey methods). Nonspecific and universal pathology was demonstrated in the system of blood aggregation regulation (BAR) in the form of the thrombohemorrhagic syndrome (THS) on the organ-tissue level. The authors have shown the efficacy of intervention in the BAR system in the presence of such pathology and the necessity of heparinization for preventing dystrophy in the brain (the organ liberating coagulants) and organs which had not suffered injury but were involved in the thrombohemorrhagic process under compulsion on the part of the activated blood, as well as for preventing and treating edema of the brain, lungs, and other organs.

Using electron microscopy, the authors studied neurons and their synapses in the cortical foci of the brain in patients (1) with brain tumours attended by epileptic seizures, (2) with tumours accompanied by no paroxysms, and (3) with epilepsy of unknown etiology. The material was obtained in the interparoxysmal period. Morphometric study of the ultrastructure showed that morphological changes characteristic of epileptization were determined only in axodendritic (AD) and axospinal synapses in the form of their massive activation whose degree was calculated by collating the area of the "active zones" with the area of the entire synaptic surface. In epilepsy this ratio was 10-12 times higher than in tumours unattended with seizures. Activation of AD synapses should be considered as an ultrastructural manifestation of epileptization of the neuron while its extension should be seen as a criterion of the morphological diagnosis of the disease itself.

RESUMO

The work deals with the results of surgical management of 61 patients with temporal-lobe epilepsy. The indications for the use of various types of operative interventions in this pathological condition are discussed. Positive results of treatment were obtained in 90.8--80.65% of cases. Deterioration in the patients' condition was not noted. It is emphasized that indications for surgical treatment must be determined as early as possible.

RESUMO

The work discusses the results of a study of the overall electric activity of the cerebral cortex, its frequency analysis and integration in brain tumors attended with epileptic seizures. The changes in electric activity are correlated with the cortical ultrastructure and the oxygen balance in the brain tissue. The possible mechanisms responsible for the development, cessation or absence of epileptic seizures in the clinical picture of brain tumors are analysed. Data are presented in evidence of the important role of hypoxia of various degrees in these mechanisms. It is established that activation of the synaptic apparatus may be considered as an ultrastructural correlator of hyperexcitation in the epileptic focus.

RESUMO

The results of local cerebral blood flow examinations were analysed in 20 patients in the postoperative period. The dynamics of the restoration of the vascular responses to functional tests was observed. A study of the changes of the local cerebral blood flow following the administration of Rheopolyglukin demonstrated its high efficacy. The rationale of a combined employment of dehydration drugs and agents improving the rheological properties of blood is emphasized.
