Lower limb overuse injuries are common among people who are exposed to physical stress. Orthotic shoe insoles are widely used to prevent lower limb overuse injuries. Here, we conducted a randomized-controlled study to examine whether the use of orthotic insoles prevents lower limb overuse injuries. Participants (n=228) were randomly assigned to use (n=73) or not to use (n=147) orthotic insoles. The insoles were molded to the shape of the foot to provide support during physical activity. The main outcome measure in the present study was the physician-diagnosed lower limb overuse injury. Thirty-four (46.6%) subjects in the insole group were diagnosed with a lower limb overuse injury compared with 56 (38.1%) in the control group (P=0.29) during the 6-month study period. When body mass index and the results of a 12-min running test and muscle strength were adjusted in a Cox's regression model, the hazard ratio for lower limb overuse injury in the insole group was 1.3 (95% confidence intervals: 0.8-2.1) compared with the control group. Use of orthotic insoles was not associated with a decrease in lower limb overuse injuries. Our findings suggest that routine use of orthotic insoles does not prevent physical-stress-related lower limb injuries in healthy young male adults.

Superoxide dismutases play a key role in the detoxification of superoxide radicals and thus protect cells from damage induced by free radicals. Within mitochondria manganese superoxide dismutase (MnSOD) provides a major defence against oxidative damage by reactive oxygen species. Polymorphism in the mitochondrial targeting sequence of MnSOD has recently been associated with risk of breast cancer. We examined this in a study population consisting of 483 breast cancer cases and 482 controls, all of Finnish Caucasian origin. Odds ratios (OR) and 95% confidence intervals (95% CIs) were estimated by unconditional logistic regression. MnSOD genotypes containing the variant A allele were found to be associated with a 1.5-fold (95% CI 1.1-2.0) increased risk of breast cancer compared with those with the homozygous wild-type genotype (MnSOD VV). This finding supports the proposal that MnSOD genotypes may modify individual breast cancer risk.

The main stressors in work organizations have been determined in the research on mental stress. This has prompted occupational health personnel to actively look for new tools in reducing stress. However, only a few workplaces have implemented action models for health promotion by reducing stressors. The aim of this project was to investigate the applicability of survey feedback for an occupational health method of stress management. The survey feedback process, which has been one of the main approaches in organization development, was applied for stressor reduction. The employee's commitment to the programme was confirmed by participation. The occupational health personnel were responsible for carrying out the programme. The project was carried out in selected departments of one factory of an international paper company. On the basis of the survey feedback, the departments made changes in their action models, environment and instruction and guidance systems. According to the follow-up in one department, the variability of work increased, and overall mental and physical strenousness decreased. The OH personnel shifted their working model towards more active co-operation with the work units. Today the survey feedback is a routine method of the occupational health service of the company.

The incidence of microbes in the nasal cavities of workers in three paper and board mills was investigated. A total of 234 persons exposed to microbial aerosols and splashes from paper machine wires and debarker drums formed the exposed group. The control group consisted of 294 workers from the dry working areas: the winding and packing sections. Chi-square analysis was used to test the differences in the frequency of microbial incidence and various symptoms between the exposed and control groups. The nasal cavities of many workers, particularly workers in the debarkers, proved to be contaminated by Klebsiella pneumoniae, other coliforms, yeasts, and molds; usually only one microbe was involved, but sometimes two or several species were found. Nasal bacteria and yeasts were largely derived from the mill and debarker air; the microbes in the air came mainly from process waters. Lack of association of nasopharyngeal symptoms with either exposure to aerosols or nasal microbial contamination was interpreted as an indication of host defenses that were adequate to protect workers from harmful microbial colonization in paper mill environments.
