RESUMO

Introduction: Temporomandibular disorders may be influenced by a lot of factors. Most patients with joint problems and especially in the facial muscles present some level of psychological pathology. Materials and Methods: A prospective study was carried out to evaluate whether depression and anxiety influence the prognosis of 108 arthrocentesis in patients with temporomandibular disorders after 6-months follow-up. Results: Patients with depression and anxiety were worse than patients without psychological comorbidities, after six months postarthrocentesis. Both factors: anxiety and depression were significantly related to presenting degenerative joint pathology before and after arthrocentesis. Conclusions: Diagnosis and treatment of psychological factors are important to improve the results in the treatment of temporomandibular disorders.

INTRODUCTION: Mandibles fractures are one of the most frequent pathologies treated in an Oral and Maxillofacial Department and represent a serious public health problem. MATERIALS AND METHODS: We present a retrospective study of patients treated for mandible fractures by the Oral and Maxillofacial Surgery Department in a Spanish tertiary hospital during 2010-2012. RESULTS: One hundred and thirty-nine patients with 201 mandible fracture sites were assisted in our department; 15% were female and 85% were male, with a male-to-female ratio of 5.5:1. The observed mean age was 35 years with a range between 15 and 89 years. The most frequent etiology of fractures was the assault (43%) followed by falls (32%). The most common fracture site was the mandibular angle (35%), followed by the parasymphysis (30%). Concerning combined fractures (60%), the most repeated association was the angle and the parasymphysis. The principal imaging test for diagnosis was the orthopantomography. The intermaxillary fixation was performed in the 25% of cases, and the rest of mandible fractures were fixed by osteosynthesis. The surgical treatment had an average of 4.2 days after the trauma, and the mean time of hospitalization was 6.5 days. CONCLUSION: The principal aim of the treatment of mandible fractures is to restore the function of the patient occlusion. A malocclusion after surgery may decrease the patient quality of life, so a correct fracture reduction could shrink health spending. After the result shown in the present study, the social education should be improved in the developed countries with the objective of decline in the amount of aggressiveness.

PURPOSE: We conducted a review of orbital fractures treated in our hospital over a period of 4 years. We reviewed several complications of such fractures, especially the relationship of postsurgical diplopia with different presurgical variables: age, gender, type of fracture, fracture area, coronal and sagittal diameter of fracture, fractured floor area, time to surgery, presence of muscle herniation, and presence of muscle entrapment. PATIENTS AND METHODS: Fractures involving the orbital floor remain a controversial issue in terms of surgical treatment and the time from trauma to surgery. Surgical indications are divided into esthetic and functional, and they greatly differ from one medical center to another. We observed that the variables that influence postoperative complications varied in the different studies reviewed. Postsurgical diplopia is one of the most important complications, and its relationship with presurgical variables was the focus of this study. For the purpose of this study, we performed bivariate and multivariate analyses, accepting P < .05 as significant. RESULTS: According to the multivariate analysis, postsurgical diplopia was only associated with trapdoor fractures, regardless of all other variables, especially if these fractures were operated on after 48 hours. In addition, the bivariate analysis showed that fractures involving muscle herniation resulted in less diplopia and a better prognosis if operated on before 48 hours. However, these results were not statistically significant. CONCLUSIONS: Surgery performed within 48 hours of trauma statistically improves fractures with true muscle entrapment and also, fractures with muscle hernation (although in this fractures, the improvement is not statistically significant).

RESUMO

Objetivo: Reportar casos de linfadenoma sebáceo, un tumor raro, localizado principalmente en la glándula parótida, con muy pocos casos descritos en la literatura. Casos clínicos: Presentamos 2 casos de esta rara afección, tratados en el Hospital Universitario Miguel Servet de Zaragoza, de enero de 2010 a diciembre de 2014. El diagnóstico, tanto por imagen como citológico, es difícil debido a su baja prevalencia y a la gran variedad de tumores de glándula parótida que existen. En los 2 casos presentados ha sido necesaria la exéresis para llegar al diagnóstico. El tratamiento curativo consiste en la exéresis completa de la tumoración.


Aim: To report a rare parotid tumour: sebaceous lymphadenoma, with very few cases reported in the literature. Cases report: We present 2 cases of lymphadenoma sebaceous of the parotid gland treated by surgery at University Hospital Miguel Servet of Zaragoza between January 2010 and December 2014. The diagnosis, both radiological and cytological, is difficult because of their low prevalence and the great variety of parotid gland tumors existent. In our 2 cases described, excision has been required for diagnosis. A complete excision of the tumor is required for curative treatment.
