RESUMO

OBJECTIVES: The treatment of diabetic ulceration of the lower extremities is a complicated task due to the nature of the ulcer and potential underlying comorbidities. This report describes the case of a 61-year-old male patient with Type 2 Diabetes who presented with an ulcerative leg wound. The objective of this study was to evaluate the outcome of a topical compounded treatment. METHODS: The patient applied a compounded medicine containing 2% mupirocin in a topical anhydrous silicone base containing fatty acids from pracaxi oil directly to the ulcer for 63 days, 3 times daily. This regimen was supplemented with exercise and an additional compounded medicine applied to the wound margins in order to increase circulation. RESULTS: By the end of the application period, the patient's ulcer was fully closed. CONCLUSION: A topical anhydrous silicone compounding base containing fatty acids from pracaxi oil may be useful in the treatment of patients with diabetic ulcers.

The purpose of this article was to present a description and first-stage evaluation of the impact of the Ice House Entrepreneurship Program on the learning experience of participating prerelease inmates at a Mississippi maximum-security prison and their perception of the transfer of skills learned in program into securing employment upon reentry. The Ice House Entrepreneurship Program is a 12-week program facilitated by volunteer university professors to inmates in a prerelease unit of a maximum-security prison in Mississippi. Participants' perspectives were examined through content analysis of inmates' answers to program Reflection and Response Assignments and interviews. The analyses were conducted according to the constant comparative method. Findings reveal the emergent of eight life-lessons and suggest that this is a promising approach to prison programming for prerelease inmates. This study discusses three approaches to better prepare inmates for a mindset change. The rethink, reform, and reenter approaches help break the traditional cycle of release, reoffend, and return.

RESUMO

We propose a general approach to describe large amplitude motions (LAM) with multiple degrees of freedom (DOF) in molecules or reaction intermediates, which is useful for the computation of thermochemical or kinetic data. The kinetic part of the LAM Lagrangian is derived using a Z-matrix internal coordinate representation within a new numerical procedure. This derivation is exact for a classical system, and the uncertainties on the prediction of observable quantities largely arise from uncertainties on the LAM potential energy surface (PES) itself. In order to rigorously account for these uncertainties, we present an approach based on Bayesian theory to infer a parametrized physical model of the PES using ab initio calculations. This framework allows for quantification of uncertainties associated with a PES model as well as the forward propagation of these uncertainties to the quantity of interest. A selection and generalization of some treatments accounting for the coupling of the LAM with other internal or external DOF are also presented. Finally, we discuss and validate the approach with two applications: the calculation of the partition function of 1,3-butadiene and the calculation of the high-pressure reaction rate of the CH(3) + H → CH(4) recombination.

INTRODUCTION: Although cardiovascular diseases (CVD) are the leading cause of death among Americans, significant disparities persist in CVD prevalence, morbidity, and mortality based on race and ethnicity. However, few studies have examined risk factor reduction among the poor and ethnic minorities. METHODS: Community-based participatory research (CBPR) study using a cluster randomized design--African-American church congregations are the units of randomization and individuals within the congregations are the units of analysis. Outcome variables include dietary change (Diet History Questionnaire), level of physical activity (7-Day Physical Activity Recall), lipoprotein levels, blood pressure, fasting glucose, and hemoglobin A1c. RESULTS: Eighteen (18) church congregations were randomized to either a health maintenance intervention or a control condition. Complete data were obtained on 392 African-American individuals, 18 to 70 years of age, predominantly employed women with more than a high school diploma. Treatment and intervention groups were similar at baseline on saturated fat intake, metabolic equivalent of tasks (METS) per day, and other risk factors for CVD. CONCLUSIONS: The GoodNEWS trial successfully recruited and evaluated CVD-related risk among African-American participants using a CBPR approach. Several logistical challenges resulted in extending the recruitment, preliminary training, and measurement periods. The challenges were overcome with the assistance of a local community consultant and a professional event planner. Our experience supports the need for incorporating non-traditional community-based staff into the design and operational plan of CBPR trials.

Evolutionary trajectories of codistributed taxa with comparable ecological preferences and dispersal abilities may be similarly impacted by historical landscape-level processes. Species' responses to changes in a shared biogeographic landscape may be purely concerted, completely independent, or classified as falling within an intermediate part of the continuum bounded by these two extremes. With sufficient molecular data, temporal contrasts of congruence among taxa with respect to these responses can be made. Such contrasts provide insights into the relative influence of ancient versus more recent climatic (and other) impacts on genetic structuring. Using phylogenetic, allele frequency, and genotypic data from two low-mobility, rotting-log-adapted (saproxylic) springtail species (Collembola) from an isolated 100-km-long section of the Great Dividing Range in southeastern Australia, we tested the concerted-response hypothesis over three timescales. Tests of phylogeographic, demographic, and contemporary population-genetic congruence were performed using an integrative approach that draws on both direct (pattern-based) and indirect (scenario-based) analyses. Our data revealed a general pattern of broad-scale similarities in species' responses to the interaction between Pleistocene climatic cycles and landscape setting, overlaid with some species-specific differences on local geographic and more recent temporal scales. This general pattern of phylogeographic congruence was accompanied by evidence for contemporaneous demographic incongruence indicating that, even at relatively small spatial scales, biogeographic context can exert an overarching influence on genetic structuring.

Some problems associated with unrestricted wave functions for open-shell molecules are discussed in the contest of coupled-cluster calculation of molecular properties. Particular attention is given to a phenomenon akin to the "triplet instability" of closed-shell molecules, where the approximate spin pairing of a nominal pair of electrons in the unrestricted Hartree-Fock wave function begins to give way to significant spin polarization. This problem-which gives rise to pronounced spin contamination-is discussed from the point of view of orbital instability and occupation numbers of the charge density matrix. The onset, rather than the magnitude of the spin contamination is analyzed in detail for diatomics, especially heteronuclear cases where the transition to significant spin contamination does not occur discontinuously. It is shown that the qualitative description of this phenomenon satisfactorily explains anomalous results for NO and PO, although the magnitude of spin contamination in these molecules is significantly less than in other cases where anomalous results are not observed. It appears that calculations of equilibrium molecular properties using coupled-cluster methods based on unrestricted Hartree-Fock reference should be monitored carefully for any molecule containing a multiple bond, especially when the bonded pair of atoms appear in different rows of the periodic table.
