This article describes a surgical procedure for rehabilitation of the severely atrophic maxillary alveolar ridge by bone grafting to the maxillary sinus and nasal floor followed by installation of implants in the grafted regions at a second operation. Five treated cases are presented. Further data are considered necessary to evaluate the procedure before it can be recommended for routine use.

The aim of the present study was to compare the effect on swelling after bilateral osteotomies of the mandible of treatment with organo-heparinoid (Hirudoid) cream and that of a placebo cream. 12 patients (7 female, 5 male), median age of 29.5 years, were included in the study. 5 patients received bilateral sagittal-split procedures for symmetrical mandibular advancement, and 7 patients received bilateral mandibular osteotomies for symmetrical mandibular set-back. Each of the 2 operated sides were randomized for treatment with active medication or placebo; 1.5 g of the active- or placebo cream were carefully rubbed into the skin above the masseteric muscle 4 times a day for 4 days. Periodic identical CT-scans were performed to evaluate swelling; patients were CT-scanned preoperatively and once or twice postoperatively. 3 variables were used to assess the swelling: (1) thickness of the subcutis; (2) maximum thickness of the masseteric muscle; (3) cross-sectional area of the soft tissues facial to the ramus of the mandible. No significant differences in postoperative swelling could be demonstrated between the 2 treatments. There were no detectable systemic effects of the treatment with organo-heparinoid.

In the present study, the recurrence rates found after treatment of keratocysts by enucleation or enucleation combined with cryotherapy are compared. Despite the relatively short follow-up period, this study indicates that there is no difference in recurrence rate between the two treatment methods.

After more than four years of postoperative follow-up, 224 patients receiving secondary bone grafting of alveolar clefts were examined. Patients were classified into three groups according to age and eruption stage of the cleft side canine tooth at surgery. Each group was then subdivided according to the diagnoses cleft lip alveolar process only, unilateral cleft lip and palate, and bilateral cleft lip and palate. The treatment results were evaluated with respect to bone level in the cleft area, gingival and periodontal condition, orthodontic treatment result, growth of the jaws after surgery, and complications. The best results were achieved when the bone grafting was performed before eruption of the cleft side canine. In half of these patients orthodontic treatment could be finished with a closed dental arch without the need for prosthodontic treatment. The sagittal growth of the jaws was unaffected by the bone grafting, whereas the anterior height of the maxilla was reduced apparently without any clinical significance.

A new surgical technique for secondary bone-grafting for closure of residual clefts in the alveolar process and hard palate is described. The technique improves the anatomy in the cleft region and, thus improves the possibility of total dental rehabilitation. The treatment results based on experiences with 293 patients will be published in a future paper, and must be estimated as promising. The technique minimizes hospitalization as well as post-operative discomfort.
