To improve health for all in the United States by the year 2000, dental health needs must be considered a component of total health and primary care. The failure to address dental needs has reached a crisis level, particularly in the black and underserved communities throughout the nation. Data from several nationwide studies have shown that oral disease is greater in black Americans than their white counterparts. More severe periodontal disease patterns, untreated dental decay, and earlier tooth loss were observed. Key minority subgroups received less preventive care.

The challenge to our profession today is to improve the quality of oral health while overcoming both extrinsic and intrinsic factors which may adversely affect our progress toward this goal. The combined efforts of dental education, dental research, and dental practice will be needed to enable us to maintain the quality of our present system and to meet the myriad changes that will chart our new directions in the future. Our unified efforts can make a difference and, thereby, ensure a meaningful and productive future for dentistry in our country and throughout the world. Let us show the world that the future of dentistry matters to us and that we look to the future with great anticipation and optimism.

The following report was presented during the October regional meeting of the American Society for Geriatric Dentistry in Washington, DC. The needs for dentists who specialize in geriatric dentistry and dental curriculum in geriatrics are discussed, together with issues that face dental educators in geriatric dentistry.

The status of dental health in the American population has significantly improved during the past 15 years. The prevalence of dental diseases is similar in children when racial comparisons are made. However, the dental treatment needs are greater and much more severe in both black adults and black children. The most recent national dental survey included employed adults only. Therefore, true differences between races may be even more marked when jobless adults are included. This omission is especially significant for blacks, where unemployment rates are twice those of whites, and this subgroup has not been included in the most recent national survey.

Dental educational institutions, as components of university systems, must develop strategic plans for program development, resource allocation, evaluation, and continued financial support. This dynamic process will be accomplished in a competitive academic arena where program excellence and program relevance are key issues in the game of survival. This article focuses on issues and trends that form the basis for planning assumptions and initiatives into the next decade and into the 21st century. This is our challenge, this is our mission if we are to be catalysts for change in the future.
