RESUMO

Congenital anomalies of respiratory system are quite diverse and not all of them are subject to surgical treatment. One example is accessory lobe of the azygos vein. This anomaly usually has no clinical manifestations and requires only follow-up, as well as attention in surgery on the right half of the chest for some other disease. This situation changes when complications occur, for example, purulent-inflammatory process. Therapy is not always effective, and lung tissue destruction requires surgical treatment. Progressive destruction complicates diagnosis and choosing surgical tactics. We present a rare case of severe purulent-inflammatory complication with abscess in accessory lobe of v. azygos. Anatomical abnormalities following this congenital pulmonary anomaly can cause difficulties in surgeries for other intra-thoracic diseases. The situation is especially relevant for thoracoscopic access. This report will be useful for radiologists, pulmonologists and thoracic surgeons.

RESUMO

A retrospective analysis of medical history of 1489 patients with spontaneous pneumothorax was made. The patients were hospitalized to the department of thoracic surgery in municipal hospital No 2. According to the data, the incidence rate of primary spontaneous pneumothorax was noted in men during last decade in Saint-Petersburg. The number of patients with secondary spontaneous pneumothorax didn't considerably increase in spite of the growth of incidence of chronic obstructive lung disease. The secondary spontaneous pneumothorax took place in female population two times more, than in male.

The specific features of lipid peroxidation (LPO) and the activity of enzymatic antioxidants in the plasma and red blood cells of patients with acute appendicitis, phlegmons, and abscesses located in the soft tissues of the shin, hip, buttocks, shoulder, and forearm were comprehensively evaluated. In these diseases, the signs of evolving oxidative stress were revealed irrespective of the site of an acute inflammatory focus. Excess accumulation of LPO products was recorded with the inhibition of antioxidative defense components, including erythrocytic glutathione-dependent enzymes. The manifestations of antioxidant deficiency were accompanied by a reciprocal increase in the concentration of the antioxidative protein ceruloplasmin and increased with more severe inflammation. The signs of imbalance in the prooxidant/antioxidant defense system slightly normalized at discharge during clinical convalescence.

RESUMO

Intravenously injected 99mTc ciprofloxacin is rapidly accumulated and washed from the septic focus. Lymphotropic injections provide targeted and long-lasting effect of the antibiotic. After injection into the interspinous ligament the drug slowly enters the inflammation area (by-passing the urinary organs and liver), where it is maximally accumulated only after 24 h, which allows to reduce the number of injections and the total dose of the antibiotic.
