RESUMO

A new method of objective diagnostic express-monitoring of a wound healing course has been developed for prognosis and evaluation of effectiveness of the treatment for various purulent wounds. The method is based on measurement of SAWV in the skin integument surrounding festered wound. Portable device-acoustic skin analyser (ASA) was used for the measurement of SAWW. The following results were obtained in 273 patients: in phase I of healing process in the first period SAWV was 35.0-49.9 m/s; in the second period of phase I of wound healing it was 50.0-90.0 m/s; in phase II--35.0-49.0 m/s; in phase III--25.0-34.9 m/s. SAWV in healthy skin is 18-24.9 m/sec. These measurements of SAWV completely correlated with cytological and hystological examination data.

RESUMO

The purpose of this work is to summarize the experience in treatment of patients with acute destructive pancreatitis and to carry out comparative analysis of the results of "open" and "closed" types of the treatment for this disease. 233 patients of the study group underwent surgery which demanded parietal deperitonization and mobilization of the pancreas from the retroperitoneal space, drainage of all parts of retropancreatic bat and drainage of biliary tracts, total continuous retroperitoneal neuro-vegetative blockade, local hypothermia and omentobursostomy with further regular elective pancreosequesf8p4omies and sanation of the cavity of the omental bursae with local sorbtion--dehydration therapy. The number of the days of inhospital stay in the study group made up 43.5 +/- 3.3, and in the control group--64.2 +/- 4.1 (p < 0.05). The level of postoperative complications in the study group made up 36.6%, in control group--85.1%, lethality being 18.2 and 50.0%, respectively. In the study group long-term unfavourable follow-up results were obtained only in 2.9% of patients, whereas in control group--in 31.6%.

RESUMO

From 1991 to 1994 years 52 patients with extensive pancreonecrosis underwent operations, final stage of which being creation of omentoburso-pancreatic stoma. Cardinal stages of the operation were: broad exposure of the bursa omentalis, cutting of pancreatic capsula and drainage of retropancreatic space, interventions on biliary ducts. Presence of omentobursopancreatic stoma enables regular control and sanation of omental bursa in postoperative period, to perform sequestrectomy from pancreatic gland and parapancreatic cellular tissue, to apply medical media of ointment type on aqual soluble base and on draining sorbents on the region of pathological process. In case of such complication as arrosion hemorrhage it provides control through omentobursopancreatic stoma. Postoperative mortality rate made up 25%.
