RESUMO

The COVID-19 pandemic is proving to be one of the most challenging health and social crises ever faced by humanity. Several drugs have been proposed as potential antiviral agents for the treatment of COVID-19 since the beginning of the health crisis. Among them are chloroquine (CQ), hydroxychloroquine (HCQ), ivermectin (IVM), and the combination of QC or HCQ and azithromycin (AZI). The use of these and several other drugs has grown sharply, even if there is proof of ineffectiveness in the early treatment or mild cases of COVID-19. Thus, there is great concern about the potential environmental impacts of the effluents released with the presence of these drugs. Therefore, this work aimed to carry out a literature review on wastewater treatment processes, focusing on removing these substances through advanced oxidation process. As the conventional effluent treatment processes do not have high efficiency for removal, it was concentrated in the literature that had as scope advanced and photo-mediated techniques to remove CQ, HCQ, IVM, and AZI. It is expected, with this work, to highlight the importance of conducting research that contributes to the control of pollution and contamination.

RESUMO

Acute thrombosis of the lower limb deep veins remains one of the most common vascular diseases. It is characterised by formation of thrombotic masses in the system of the deep veins of the lower extremities. A serious complication associated with deep vein thrombosis is pulmonary embolism. In rare cases, under certain conditions a massive and disseminated blockade of the venous outflow with thrombotic masses may lead to ischaemia of the extremity with the formation of superficial and deep necroses and later on to gangrene. One of the main components of management is long-term administration of anticoagulants, which in a series of cases may result in the development of gastrointestinal haemorrhage which in its turn requires discontinuation of anticoagulants. Described herein is a clinical case report concerning treatment of a woman presenting with deep vein thrombosis and the development of phlegmasia cerulean dolens complicated by massive pulmonary artery thromboembolism on the background of intestinal bleeding.

RESUMO

The article contains a review of the literature data concerning different variants of conservative treatment of patients suffering from lower limb deep vein thrombosis. This is accompanied and followed by demonstrating the manner of alterations in the views on using various anticoagulants, as well as analysing the attitude towards the place of compression therapy in treatment of patients with lower limb deep vein thrombosis.

Presented herein is a case report dealing with successive combined treatment for arteriovenous malformation of the left part of the chest, macrofistular form complicated by necroses of the left thoracic portion with arrosive haemorrhage from the area of necrosis. The authors describe diagnosis of this disease, technical peculiarities of performing transcatheter endovascular occlusion and topical treatment of wounds, underlining difficulties of verifying the pathology concerned at the prehospital stage and prevention of complications. Particular emphasis in management of these patients is placed upon therapeutic decision-making. Radical excision of tissue practically completely consisting of dilated thin-walled vessels may result in massive perfuse haemorrhage and is often impossible. Selective exclusion of feeding arteries by endovascular methods frequently leads to formation of necroses, does not differ in radical nature because of the presence of multiple arteriovenous fistulas and requires regular follow up of the patient due to a possibility of relapse. The final therapeutic decision should be made in the context of a particular patient, since there are no versatile approaches to management of the pathology concerned.

Factor XIII (FXIII) plays an important role in formation and stabilization of the fibrin clot. It is known that FXIII-A gene G163T (Val34Leu) polymorphism leads to increased activation of this factor, however participation of the 34Leu variant in thrombogenesis remains disputable. The present work was aimed at studying peculiarities of distribution of variants of FXIII-A polymorphisms in patients with early onset of venous thromboembolism (VTE), as well as revealing associative links between the carrier state of this mutation and the character of clinical course of the disease. We examined a total of 250 patients with VTE. Of these, there were 119 (47.6%) men and 131 (52.4%) women, mean age 37.42 years (range 10-45 years). In the group of patients with VTE, the proportion of homozygous carriers of allele 163T (Leu34) turned out to be more than 1.5-fold higher as compared with the corresponding parameter in the control group (OR=1.8, 95% CI: 0.9-4.0; p=0.14). Analysing FXIII-A G163T polymorphism depending on gender revealed a considerable increase in the incidence rate (IR) of 163TT genotype in women with VTE as compared to male patients (13.0% versus 5.1 %, respectively, OR=2.8, 95% CI: 1.1-7.4; p=0.047) and to the control group (OR=2.7; 95% CI: 1.2-6.1; p=0.023). Analysing gene FXIII-A polymorphism in the groups of patients with various clinical manifestations of VTE revealed a decrease in the proportion of heterozygotes in the group of deep vein thrombosis + pulmonary artery thromboembolism (PATE) and, vice versa, an increase in the proportion of homozygotes by the Leu34 variant in patients with signs of PATE. The obtained findings make it possible to consider the genotype 163TT of FXIII-A gene as a new independent risk factor for the development of VTE in young women living in the North-West region of Russia, which is observed for the first time. Additional studies are necessary.

RESUMO

Creating hybrid operating rooms (OR) is a new stage of the development of present-day cardiovascular surgery, with the need for these operating suites having evolved resulting from rapid implementation of novel techniques of treating cardiovascular diseases, combining elements of «open¼ and endovascular surgery. Despite widespread interest in this problem, there are virtually no systematized works properly describing the specificity of designing and organization of the hybrid OR's work. The present work was aimed at compiling the literature data and sharing own experience gained in organization of the hybrid OR, designation of indispensable technical and organizational requirements for performing hybrid interventions, studying the basic problems related to designing and building such operating rooms, as well as organization and management of the personnel.

RESUMO

The year of 1896 became the starting point in the development of cardiovascular surgery, since it was just then that Ludwig Rhen was the first in the world to have performed a successful closure of a right-ventricle wound, thus opening up the era of active surgical policy in heart wounds. However, tangible progress in surgical management of intrathoracic vessels was made later on. One of the important stages of this trend was a successful closure of an ascending-aorta wound performed by I.I. Dzhanelidze in 1913. It was not only the world's first successful operation performed in a traumatic lesion of the ascending aorta but also apf intervention giving rise to further development of surgery in this field. The present-day notions concerning surgical policy in traumatic lesions of intrathoracic vessels are based on the experience of prominent and outstanding surgeons of the 20th century. The material for this publication was I.I. Dzhanelidze's article entitled "A case of ascending- aorta wound closure" and published in 1916.

RESUMO

The authors analyzed results of treatment of 127 patients. A new direction in the endovascular treatment of critical ischemia of the lower extremities is substantiated on the basis of a combination of methods of subintimal and intraluminal angioplasty. For the first time all the patients with critical ischemia of lower extremities, and having no irreversible changes of the extremity requiring amputation at the level of the low leg or femur, must be considered as good candidates for translumbar balloon angioplasty.

OBJECTIVE OF THE STUDY: To evaluate efficiency of transluminal balloon angioplasty (TLBAP) in treatment of lower limb critical ischaemia (LLCI). MATERIALS AND METHODS: Over the period from November 2004 to April 2008, we performed a total of 89 TLBAPs in eighty-two patients presenting with LLCI. The patients' age averagely amounted to 67.8 +/- 9.8 years. Fifty (61%) patients had an ischaemic ulcer on the foot, seventeen (20.7%) had gangrene, and fifteen (18.3%) patients suffered from ischaemic pain at rest. Forty-five (54.9%) patients suffered from diabetes mellitus, including thirty-three (40.2%) subjects with insulin dependent DM. Sixty (73.2%) patients were diagnosed as having coronary heart disease (CHD), sixty-five (79.3%) had arterial hypertension, twenty-five (30.5%) - cerebrovascular disease, and thirteen (15.9%) - chronic renal insufficiency. Six (7.3%) patients were on chronic haemodialysis. The distribution of the patients by the level of the lesion were as follows: iliac 4 (3.6%), femoropopliteal 55 (49.5%), crural artery 52 (46.8%) Type-C and type-D lesions (TASC) were encountered in 89.2% of cases. Occlusions were observed in 82% of all lesions of the crural artery. 42.2% of the patients were treated by means of subintimal angioplasty. Stenting was used in fourteen (7.3%) cases. RESULTS: The angiographic and clinical success of the procedure was achieved in seventy-five (91.5%) patients. No LLCI was observed either after one or three years in 79.4 and 52.9% of the patients, respectively.

The aim of the study was to examine the potential of perftoran for the treatment of patients with critical lower limb ischemia, in whom surgical revascularization appeared not feasible in view of the type of lesion or severity of concomitant pathology. The study accrued 85 patients distributed into the basic and control groups. The control group patients (n=40) received the basic conservative therapy including rheological, vasoactive drugs, anticoagulants and aggregation inhibitors. Patients entering the basic group (n=45) were administered perftoran infusions together with the basic therapy protocol. We examined the time course of changes in the painful syndrome, ascertained the degree of ischemia, the extent of trophic disorders, the parameters of the peripheral flow in the pedal arteries by ultrasound Doppler (USD), and the intensity of the manifestations of the systemic inflammatory response. It has been proven that the use of perftoran is justified and that the preparation may be recommended for use in multimodality treatment of the patient group under consideration.

RESUMO

Results of creating the home ELISA test-system for diagnostics of enzootic encephalomyelitis (Teschen disease) has been presented. Biological components of diagnosticums has been created and described, reaction stages have been optimized. Sensitivity of the test-system for identification of antibodies to PTV-1 averaged 93%, specificity--100%. In the "sandwich"-variant the strains may be classified as serotype 1.

RESUMO

Optimal methods of purification and concentration of teschevirus strain of the pigs of the first serotype "Chernigivsky-2372" have been chosen for development of the set of Teschen pig discase diagnosticums. It has been established that differential centrifugation with following ultracentrifugation in the gradient of saccharose density permits to deprive viruses from host cell proteins, empty virus capsides and defect virus particles. That is the method fit for purification of virus preparations for the solid-phase immunoenzyme analysis.

RESUMO

Strains of porcine enteroviruses (PEV) (n = 50), have been isolated from different animal species and environment objects: from pigs--19, mice--16, cats--3, rabbits--3, poultry--2, from objects of cattle-breeding premises--7. This testifies to PEV capacity to reproduction in the organism of hosts which are not characteristic of them. Some PEV strains cause cytopathogenic effect in human cell culture. No antigenic relations between 16 investigated strains of porcine enteroviruses and polimyelitis viruses of serotypes 1-3 and Coxsackie B5 were established.

There is an ever-increasing risk of the pregnant women's developing a specific lesion of the pleura as evidenced by high infection rates (80%) among the adult population. In registering women for pregnancy, taking a history of a contact with a TB person is mandatory as is of a 3-year exposure to those illnesses presenting with paraspecific signs that characterize the pregnant women's having had atypical tuberculous pleuritis (subfebrile condition, the act of breathing-related pain in the chest, unmotivated shortness of breath). In all such cases phthisiological advice is to be applied for with writing it down in the pregnant women's card for adequate (and in some cases preventive) measures to be instituted after labor.

Several surgical procedures are available for the treatment of Hirschsprung's disease (HD) the choice often depends on the surgeon's preference. The procedure that is most convenient for the surgeon is not necessarily the optimal one for the patient, however, and complications and mortality need to be considered. We reviewed our series of 275 children operated upon for HD by endorectal pull-through after Soave-Boley dissection of the seromuscular cuff followed by a hand-sutured (HSA) (210) or stapled (SA) (65) primary perineal colorectal anastomosis. Early postoperative complications were documented in 13 children with a HSA and 5 with a SA Follow-up was from 6 months to 15 years for HSA patients with and from 1 month to 3 years after SA. Results were good or satisfactory; there were no unsatisfactory results. On long-term follow-up the complication rate after SA (13.8%) was comparable to that after HSA (20.5%). The overall complication rate on long-term follow-up was 2.91%, which is comparable to results published in the literature. The SA is particularly convenient for surgeon, as it allows a simpler and shorter operation than with the traditional HSA.

RESUMO

A retrospective analysis of treatment of 450 injured with traumas of the extremity vessels was made. These patients were subjected to reconstructive operations. In a considerable part of the patients there appeared thrombosis of the operated vessel. Experiments in dogs have shown that prophylactics of temporary prosthesis and injured artery should be based on the method of active treatment in order to eliminate the posttraumatic spasm of the arteries.

The cell adhesion molecules NCAM and L1 are considered to play key roles in neuronal development and plasticity. L1 has been shown to interact with NCAM, possibly through NCAM binding to oligomannosidic glycans present in L1. We investigated the effect of recombinant immunoglobulin (Ig) modules of NCAM involved in homophilic NCAM binding, on L1 induced neurite outgrowth from PC12-E2 cells and found a complete inhibition of L1 induced neurite outgrowth after addition of Ig-modules 1, 2 and 3 of NCAM, suggesting that the ligation state of NCAM is crucial for normal L1 signaling.
