RESUMO

The conceptual model of rehabilitation and suicidal behaviour prevention in dementia, considering the multiple components, is presented in the article. The developed program of the complex differentiated medical and psychological rehabilitation and suicidal behaviour prevention (MPRS BP) in patients with dementia is aimed at the preventive measures of the suicidal behaviour (SB) through the depressive symptoms reduction, patient's mind and cognitive status stabilization, self-aggression decreases, social functioning and independence increase in patients with dementia. The individual approach to the prevention of SB in patients with dementia considered the SB factors (extra- and intrapersonal), SB formation mechanisms (cognitive, depressive, and psychological), clinical psychopathological SB correlators (especially gender-based) in different types of dementia. Offered MPRS BP program is based on the complex approach to rehabilitation. It combines pharmacotherapy, psychotherapy (crisis therapy, rational psychotherapy, art therapy, family psychotherapy), psychoeducation, psychological training (cognitive, communicative and social skills training) keeping to the principles of systematicity, complexity, integrity, phasing, personal interventions, provision of assistance in time. According to the approbation results of the MPRS BP program in dementia, its high efficiency was demonstrated compared to the traditional treatment programs of this category of patients.

In this article the formation of psychopathological predictors auto-aggressive behavior in patients with a first psychotic episode were identified, which became "targets" in the framework of a comprehensive emergency suicide assistance to conduct the crisis psychotherapy. The work was done on the basis of the Sumy regional psychoneurologic dispensary, where 100 patients with a first psychotic episode were examined: 52 of them (core group) had suicidal symptoms and 48 (control group) had not. According to the test results of severity of auto-aggressive predictors (pre-suicidal syndrome) to clinicopsychopathological predictors of auto-aggressive behavior include: the narrowing of the cognitive function (p≤0,001), the avoidance of interpersonal contact (r≤0,001), the presence of affective (p≤0,001) and vegetative (p≤0,01) violations, the autoaggression of moderate severity (p≤0,001) and impulsivity (p≤0,001). Patients of the core group with the auto-aggressive behavior (n=58) completed a course of a crisis psychotherapy comprising the stages of crisis support, crisis intervention and increase the adaptation layer. After a psychotherapy course levels of aggression (6,45±0,41), auto-aggression (of 9,68±0,67), disorders in the affective sphere (18,58±0,66) and impulsivity (of 4,23±0,30) decreased, which was manifested in increasing tolerance to emotional stress factors, control over their emotions and reduce their affective valence (p≤0,001). The expansion of interpersonal interaction, the increase of patients social activity, the blood relationships establishment (of 9,23±0,40) was observed.

Spouse deadaptation is polietiologic health problem that leads to medical, psychological and social consequences that causing the need to find the key factors of its development. The research of the influence of migraine, occurring in one in five women, on the formation of deadaptation of the married couple was conducted. The author analyzes the decline of daily activity, quality of life and development of the personal characteristics of women depending on the clinical course of the disease. The interrelation was established between different variants of personality disorders in patients with migraine and types of sexual dysfunction. The most common form of sexual dysadaptation in all types of personality disorders revealed communicative. The role of men in causing interpersonal conflict in terms of disharmony of sexual relations between spouses was described. The research reveals the main factors affecting the degree of spouse deadaptation that will form the basis of psychocorrective program.

RESUMO

The high affinity and specificity of biological receptors determine the demand for and the intensive development of analytical systems based on use of these receptors. Therefore, theoretical concepts of the mechanisms of these systems, quantitative parameters of their reactions, and relationships between their characteristics and ligand-receptor interactions have become extremely important. Many mathematical models describing different bioassay formats have been proposed. However, there is almost no information on the comparative characteristics of these models, their assumptions, and predictive insights. In this review we suggested a set of criteria to classify various bioassays and reviewed classical and contemporary publications on these bioassays with special emphasis on immunochemical analysis systems as the most common and in-demand techniques. The possibilities of analytical and numerical modeling are discussed, as well as estimations of the minimum concentrations that may be detected in bioassays and recommendations for the choice of assay conditions.

RESUMO

Methods for registration of intermolecular interactions based on the phenomenon of surface plasmon resonance (SPR) have become one of the most efficient tools to solve fundamental and applied problems of analytical biochemistry. Nevertheless, capabilities of these methods are often insufficient to detect low concentrations of analytes or to screen large numbers of objects. That is why considerable efforts are directed at enhancing the sensitivity and efficiency of SPR-based measurements. This review describes the basic principles of the detection of intermolecular interactions using this method, provides a comparison of various types of SPR detectors, and classifies modern approaches to enhance sensitivity and efficiency of measurements.

RESUMO

Premises of the pyoinflammatory complications of colostomy in patients with tumorous obstructive ileus were analyzed. These were: age >60 years, concomitant diseases, extent of anemia, duration of the operative treatment more then 2,5-3 hours, mass of subcutaneous fat tissue and time before stoma opening. Highlighting of risk factors allowed prediction of inflammatory postoperative complications and take steps of its prophylaxis.

RESUMO

The article is dedicated to bacterial translocation under the conditions of acute small and large bowel obstruction, studied in experiment using bacteriological methods. The degree of contamination of internal organs, blood, and peritoneal exudate in the dynamics of the pathological process was determined. The study found that the use of allopurinol, a xanthine oxidase inhibitor, reduced the intensity of bacterial translocation.

RESUMO

We studied follow-up iliac mucosa biopsies from 11 patients with juvenile polyposis and 4 patients with familial adenomatous polyposis who had undergone total colectomy with mucosal proctoectomy with creation of straight ileoanal anastomosis. The biopsies that have been taken from the posterior wall of the terminal ileum show incomplete and focal neocolonic transformation of iliac mucosa. Nevertheless, in most cases iliac mucosa preserved its architectural and histochemical characteristics. Chronic inflammation in iliac mucosa is not typical for patients with polyposis.

RESUMO

11 patients with juvenile polyposis and 4 patients with familial adenomatous polyposis after total colectomy entered the study. Long-term follow-up with endoscopic examination and multiple biopsies in these patients showed high probability of polyps in preserved regions of the gastrointestinal tract. Therefore, regular prophylactic endoscopy, morphological examinations and biopsies of endoscopically normal mucous membrane are justified. Such policy is recommended especially for patients with familial adenomatous polyposis.
