RESUMO
The new surgery thinking, in matter of hand loss, is made of two ways: without hand, there is no more functional abilities; homograft or allograft of hand is the best actual treatment. This is not true and we are able to get new functional abilities by other ways: without any treatment, we are able of spontaneous functional recovery. There are other treatments: the old operation of Krukenberg or new bionic prosthesis. This work is a recall of obvious or forgotten facts.

More than thirty years after its description, the cross finger flap is still widely in use. Its main technical data are reviewed. The follow-up show that this procedure remains indicated in deep losses of substance of the palmar aspect of the basal or middle phalanx. Its use on the terminal phalanx remains controversial, but it is still useful when elaborated microsurgical procedures are contraindicated or not available.

The technique proposed is an advancement-rotation plasty for volar coverage, as is Hueston's technique. It differs by the fact that the neurovascular bundle on the side opposite the cutaneous base is advanced as well. While this modification leads to limited possibilities of advancement, it seems that tactile sensibility might be improved.

Twelve cases of median nerve compression at the elbow are presented. More than half of them showed only an anterior interosseous nerve syndrome (Kiloh-Nevin). Nevertheless, surgical exploration was performed from the brachial tunnel to the superficialis arch. When the neurolysis occurred reasonably early, uniform good results were obtained.
