RESUMO

Overhead sports overload the shoulder complex due to movement repetition and the great amount of force created during the athletic motion, which may cause adaptations in the shoulder and lead to shoulder pain. However, overhead movements include the kinetic chain, and alterations in some of the structures throughout the kinetic chain may increase stress on the shoulder complex and be associated with shoulder pain. PURPOSE: To compare kinetic chain components in overhead athletes with and without shoulder pain. METHODS: Forty-one volleyball and handball athletes (21 with and 20 without shoulder pain) were included and assessed for hip internal (IR) and external rotation (ER) range of motion (ROM), hip and trunk isometric strength, trunk endurance and neuromuscular control of the lower and upper limbs (Y balance test). RESULTS: Athletes with shoulder pain showed smaller IR ROM in both hips, lower endurance time for trunk extensors and flexors, decreased reach distance in the anterior and posteromedial direction, as well as a smaller composite score in the Y balance test (p < 0.05). CONCLUSION: Volleyball and handball athletes with shoulder pain showed changes in ROM throughout the kinetic chain in addition to lower core endurance, and decreased neuromuscular control of lower limbs.

The objective of this study was to investigate how Brazilian physical therapists (PTs) use therapeutic exercises in the rehabilitation of individuals with rotator cuff (RC) tendinopathy. The study used an online survey with a mix of 62 open- and closed-ended questions divided into three sections: participant demographics, professional experience, and clinical practice in the rehabilitation of patients with RC tendinopathy. One hundred and fifty-nine Brazilian physical therapists completed the survey. Most of our sample recommended isometric exercises (69.9%) in the initial phase of rehabilitation and eccentric exercises (47.4%) in the advanced phase. However, there was a wide variability in determining the volume of exercises, particularly with isometric exercises. Most of our sample considered patient comfort and pain levels when adjusting exercise intensity, regardless of exercise type. The majority (48.40%) recommended weekly reassessment and modification of exercises. Additionally, despite pain being a key factor for discharge and the primary adverse effect of exercise, most of our sample would not discontinue exercises in case of pain during the early and late phases of rehabilitation. Despite the lack of consensus on some aspects, the clinical practice of our sample is in line with the current literature and practice in other countries. However, further research and implementation are crucial to enhance future rehabilitation outcomes, including exploring the exercise training volume, the safety and effectiveness of exercising with pain and identifying the optimal pain level for best results.

BACKGROUND AND PURPOSE: To assess the effects of a rehabilitation protocol of rotator cuff (RC) isometrics coupled with traditional shoulder exercises on patient-rated outcomes, muscle strength, and electromyographic activity in individuals with RC tendinopathy. METHODS: Eleven individuals (8 women and 3 men, 37.9 ± 5.6 years) with RC tendinopathy performed isometric RC exercises in combination with scapular muscle stretching and strengthening for 6 weeks. Treatment effects were assessed with patient-rated pain and shoulder function, isometric muscle strength, electromyographic activity during arm elevation and internal and external shoulder rotation, and pain during arm elevation before and at the end of the first session, and after 6 weeks of intervention. RESULTS: There were improvements in pain and shoulder function, increased isometric muscle strength for arm elevation and internal rotation, increased muscle activity of the infraspinatus and serratus anterior, and reduced pain during arm elevation after 6 weeks of intervention. DISCUSSION: This case report showed improvements on pain and function, increases on isometric strength of the shoulder and on electromyographic activity of the serratus anterior and infraspinatus muscles, as well as decreases on pain during arm elevation, after a 6-week intervention of RC isometric exercises associated with scapular muscle stretching and strengthening in patients with RC tendinopathy.

INTRODUCTION: Rotator cuff tendinopathy is a common shoulder disorder in which the primary treatment is resistance exercises. Isometric exercises are being studied for lower limb tendinopathies but not for rotator cuff tendinopathy. This protocol for a randomized clinical trial aims to compare the effects of two types of exercise (isometric and isotonic) on shoulder pain, functioning, muscle strength, and electromyographic activity in individuals with rotator cuff tendinopathy. METHODS: Forty-six individuals (18 to 60 years old) with shoulder pain for more than three months and unilateral supraspinatus and/or infraspinatus tendinopathy will participate in this trial. Individuals will be randomized into two exercise groups: isometric or isotonic. The following outcomes will be evaluated before and after the first session and after six weeks of intervention: shoulder pain and functioning; isometric strength of shoulder elevation and lateral and medial rotation; and electromyographic activity of medial deltoid, infraspinatus, serratus anterior, and lower trapezius. Groups will perform stretching and strengthening of periscapular muscles. The isometric group will perform three sets of 32 s, at 70% of maximal isometric strength. The isotonic group will perform concentric and eccentric exercises (2 s for each phase) in three sets of eight repetitions at a load of eight repetition maximum. The total time under tension of 96 s will be equal for both groups, and load will be adjusted in weeks three and five of the protocol. Treatment effect between groups will be analyzed using linear mixed model. Objective: The purpose of this study was to evaluate relationships between the presence and number of active myofascial trigger points (MTPs) in shoulder muscles and physical and demographic characteristics, depressive symptoms, pain and function, range of motion (ROM), and strength in individuals with shoulder pain. Methods: Fifty-eight individuals were assessed for physical and demographic characteristics, depressive symptoms, shoulder pain and function, MTPs (upper and lower trapezius, infraspinatus, and supraspinatus), shoulder ROM and strength test, and pain during ROM and strength test. Relationships were verified using point-biserial (rpb), Spearman correlation test, and multiple linear regression analysis. Results: We found weak to moderate (P < .05) correlations between presence and number of MTPs and depressive symptoms (rpb, 0.28-0.32), pain during ROM (rpb, 0.36-0.40), pain during strength test (rpb, 0.29-0.38), and shoulder function (rpb, -0.29 to 0.33) and strength (rpb, 0.26-0.34). MTPs in the infraspinatus contributed 10% (R² = 0.10; P < .05) to depressive symptoms; in the upper and lower trapezius contributed 27% (R² = 0.27; P < .05) to pain during internal rotation ROM; in the upper trapezius contributed 15% (R² = 0.15; P < .01) to pain during internal rotation strength test and 14% to pain during internal rotation ROM (R² = 0.14; P < .01); and in the supraspinatus contributed 17% (R² = 0.17; P < .01) to pain during external rotation ROM. Conclusion: This study found that MTPs in individuals with shoulder pain contributed to depressive symptoms and pain during internal and external rotation ROM and internal rotation strength test.

OBJECTIVE: To investigate the correlation between functional performance and self-reported upper limb function and between these two functional measures and clinical measures of shoulder and trunk in overhead athletes with shoulder pain. DESIGN: Cross-sectional study. SETTINGS: Laboratory. PARTICIPANTS: Twenty-one volleyball and handball athletes (23.6 ± 3.7 years) with shoulder pain in the dominant limb. MAIN OUTCOME MEASURES: Self-reported dominant upper limb function using Penn Shoulder Score and Disabilities of the Arm, Shoulder, and Hand; shoulder and trunk range of motion; isometric strength of shoulder, periscapular, and trunk muscles; endurance time of trunk muscles; and functional upper limb performance using the Upper Quarter Y-Balance Test (UQYBT). RESULTS: A moderate correlation was observed (r = 0.45-0.58; p < 0.05) between UQYBT performance and strength of serratus anterior, lower trapezius, trunk rotators, and trunk flexors. No significant correlation was observed between UQYBT performance and the other clinical measures of shoulder and trunk (r = 0.01-0.39, p > 0.05). Self-reported upper limb function was neither significantly correlated with clinical measures (r = 0.01-0.40, p > 0.05) nor with UQYBT performance (r = 0.02-0.19, p > 0.05). CONCLUSION: We observed a correlation between UQYBT performance and strength of periscapular and trunk muscles.

Spine posture, mobility and strength can be associated with changes in scapular movement and shoulder pain. However, these aspects have not been assessed in healthy individuals with scapular dyskinesis without shoulder and/or back pain. Objective: To analyze spine posture, mobility and strength in healthy individuals with and without scapular dyskinesis. Method: Cross-sectional study, fifty-two college-aged individuals were divided into two groups, according to scapular dyskinesis testing: a group without scapular dyskinesis (n= 19; age= 22.95±2.86 years; BMI= 22.97±3.12 kg/m²; 10 males) and another with scapular dyskinesis (n= 33; age= 22.06±2.73 years; BMI= 22.14±3.22 kg/m²; 10 males). Posture of the head and shoulders was analyzed from photographs by a postural assessment software (PAS/SAPO). Thoracic kyphosis and range of motion of the cervical and thoracolumbar spine were measured by a digital inclinometer, and the strength of the cervical and thoracolumbar spine muscles by a hand-held dynamometer. Intergroup comparison for all the variables was conducted using analysis of variance (one-way ANOVA), considering p≤0.05 as significant. Results: No intergroup difference was found for spine posture (p= 0.18-0.99), mobility (p= 0.23-0.96) and strength (p= 0.42-0.99). Conclusion: Spine posture, mobility and strength do not differ between healthy individuals with and without scapular dyskinesis. The outcomes suggest that scapular dyskinesis may be related to interpersonal variations. Future prospective studies be conducted in order to verify if these variables can change and influence the development of shoulder pain.


A postura, mobilidade e força da coluna podem estar associadas com mudanças no movimento da escápula e dor no ombro. Objetivo: Analisar a postura da coluna vertebral, mobilidade e força em indivíduos saudáveis com e sem discinese escapular. Método: Estudo transversal com cinquenta e dois indivíduos em idade universitária foram divididos em dois grupos, de acordo com o teste de discinese escapular: um grupo sem discinese escapular (n= 19; idade= 22,95±2,86 anos; IMC= 22,97±3,12 kg/m²; 10 masculino) e outro com discinese escapular (n= 33; idade= 22,06±2,73 anos; IMC= 22,14±3,22 kg/m²; 10 masculino). A postura da cabeça e dos ombros foi analisada a partir de fotografias por um software de avaliação postural (PAS/SAPO). A cifose torácica e a amplitude de movimento da coluna cervical e torácica foram medidas por um inclinômetro digital, e a força dos músculos cervical e torác lombar por um dinamômetro portátil. A comparação intergrupo para todas as variáveis foi realizada utilizando-se a análise de variância (ANOVA unidirecional), considerando p≤0,05 como significativa. Resultados: Não foi encontrada diferença entre grupos postura (p= 0,18-0,99), mobilidade (p= 0,23-0,96) e força da coluna (p= 0,42-0,99). Conclusão: Postura, mobilidade e força da coluna vertebral não diferem entre indivíduos saudáveis com e sem discinese escapular. Os resultados sugerem que a discinese escapular pode estar relacionada a variações interpessoais. Estudos prospectivos futuros devem ser conduzidos afim de verificar se essas variáveis podem modificar e influenciar o desenvolvimento da dor no ombro.

OBJECTIVE: The purpose of this study was to determine the relative and absolute reliability of the pressure pain threshold (PPT) in the shoulder muscles of participants with and without unilateral subacromial impingement syndrome. METHODS: Study of intraday intra- and interrater and interday intrarater reliability. Fifty-two participants symptomatic for unilateral subacromial impingement syndrome were divided into 2 groups (SG1 and SG2) of 26 participants each, and 26 participants asymptomatic for shoulder pain took part in the study. Two raters assessed the PPT in 4 shoulder muscles. Each rater assessed symptomatic (SG1) and asymptomatic participants twice on the same day, and one of the raters on 2 different days (SG2). The intraclass correlation coefficient, standard error of measurement (SEM95% and SEM%), and minimum detectable change (MDC95% and MDC%) were calculated. RESULTS: Relative reliability was good or excellent for all assessments, as well as for both groups and raters (intraclass correlation coefficient: 0.87-0.98). The SEM95% values for intra- and interday intrarater measures were between 0.43 and 1.50 kgf/cm2 and SEM% between 6.76 and 12.86%, whereas MDC95% values ranged from 0.60 to 2.12 kgf/cm2 and MDC% from 9.56 to 18.18%. In interrater measures, SEM95% was between 0.58 and 0.77 kgf/cm2 and SEM% between 10.10% and 13.71%, whereas MDC95% varied from 0.82 to 1.08 kgf/cm2 and MDC% from 14.29% to 19.39%. CONCLUSION: Relative reliability was good or excellent. This study presents absolute reliability values that could be used as a reference in the clinical use of PPT.

OBJECTIVE: The aim of the study was to evaluate the effects of adding load to restrain the nonparetic lower limb during gait training on weight-bearing and temporal asymmetry after stroke. DESIGN: Thirty-eight subjects were randomized into treadmill training with load (5% of body weight) on the nonparetic limb (experimental group) and treadmill training without load (control group). Interventions lasted 30 mins/d for 2 wks (9 sessions). Both groups performed home-based exercises and were instructed to increase the use of paretic limb in daily life situations. Ground reaction force was obtained by a force plate during standing position (static) and gait (dynamic). Temporal gait parameters were assessed by a motion system analysis. Outcome measures were evaluated at baseline, posttraining, and after a 40-day follow-up. RESULTS: The experimental group increased static ground reaction force of the paretic limb at posttraining (P = 0.037) and the control group increased dynamic ground reaction force of the paretic limb at posttraining (P = 0.021), both with maintenance at follow-up. Neither group showed a change in the swing time symmetry ratio after training (P = 0.190). CONCLUSIONS: Treadmill training associated with behavioral strategies/home-based exercises seemed to be useful to minimize weight-bearing asymmetry, but not to improve temporal gait asymmetry. Load addition did not show additional benefits.

INTRODUCTION: Eccentric training and jump tests are widely used to recover and measure deficits in knee strength and functionality after anterior cruciate ligament (ACL) reconstruction. OBJECTIVE: To correlate knee extension and flexion torque generated by eccentric isokinetic training, with functional jump tests in subjects with reconstructed ACL. METHOD: Sixteen men with unilateral ACL reconstruction were assessed before and after 12 weeks of eccentric isokinetic training of knee flexors and extensors (3×10 MVC, 2x/week) at 30°/s for extension and flexion torque (isometric; concentric and eccentric at 30 and 120°/s) and functional jump tests (single, triple, cross and figure of 8). Inter- and intra-limb pre- and post-training mean peak torque (MPT), distance and jump test times were compared along with the correlations between these variables, considering P<0.05. RESULTS: The affected limb (AL) showed significant gain of extension and flexion torque (P<0.01) in the different test categories and velocities evaluated. In the non-affected limb (NAL), this only occurred in the eccentric category (30 and 120°/s), in the extension (P<0.01) and flexion (P<0.05 and P<0.01) torques, respectively. In the jumps, there was an increase in distance (single and triple; P<0.05) and a decrease in time (crossed and figure of 8; P<0.01), however, MPT x Jump correlations were weak (r<0.3) in the pre and post-training period in both limbs. CONCLUSION: Despite the gain in knee extension and flexion torque and jumping performance, the expected correlation was not satisfactory, suggesting that knee functionality involves other variables inherent to motor control. INTRODUÇÃO: O treino excêntrico e testes de saltos são muito utilizados para recuperar e aferir déficits de força e funcionalidade do joelho, após reconstrução do ligamento cruzado anterior (LCA). OBJETIVO: Correlacionar o torque extensor e flexor do joelho, gerados pelo treino isocinético excêntrico, com testes funcionais de saltos em sujeitos com LCA reconstruído. MÉTODO: Foram avaliados 16 homens, com reconstrução unilateral do LCA, antes e depois de 12 semanas de treino isocinético excêntrico de extensores e flexores do joelho (3 × 10 CEVM, 2x/semana) a 30°/s, quanto ao torque extensor e flexor (isométrico; concêntrico e excêntrico a 30°/s e 120°/s) e aos testes funcionais de saltos (simples, triplo, cruzado e figura em 8). As médias dos picos de torque (MPT), a distância e os tempos dos testes de salto inter e intramembros, pré e pós-treino foram comparados, além das correlações entre essas variáveis, considerando P< 0,05. RESULTADOS: O membro acometido (MA) mostrou ganho significativo de torque extensor e flexor (P< 0,01) nos modos e velocidades avaliados. No membro não acometido (MNA), isso ocorreu apenas para o modo excêntrico (30°/s e 120°/s), do torque extensor (P< 0,01) e flexor (P< 0,05 e P<0,01), respectivamente. Nos saltos, houve aumento da distância (simples e triplo; P< 0,05) e diminuição do tempo (cruzado e figura em 8; P< 0,01), porém, as correlações entre MPT e saltos mostraram-se fracas nos dois membros (r< 0,3) antes e depois do treino. CONCLUSÃO: Apesar do ganho no torque extensor e flexor do joelho e no rendimento dos saltos, a correlação esperada não foi satisfatória, sugerindo que a funcionalidade do joelho envolve outras variáveis inerentes ao controle motor. INTRODUCCIÓN: El entrenamiento excéntrico y las pruebas de saltos son muy utilizados para recuperar y medir los déficits de fuerza y funcionalidad de la rodilla, después de la reconstrucción del ligamento cruzado anterior (LCA). OBJETIVO: Correlacionar el torque extensor y flexor de la rodilla, generados por el entrenamiento isocinético excéntrico, con pruebas funcionales de saltos en sujetos con LCA reconstruido. MÉTODO: Fueron evaluados 16 hombres, con reconstrucción unilateral del LCA, antes y después de 12 semanas de entrenamiento isocinético excéntrico de extensores y flexores de la rodilla (3x10 CEVM, 2x/semana) a 30°/s, cuanto al torque extensor y flexor (isométrico, concéntrico y excéntrico a 30 y 120°/s) y a las pruebas funcionales de saltos (simple, triple, cruzado y figura en 8). Se compararon promedios de los picos de torque (MPT), distancia y tiempos de las pruebas de salto, inter e intra-miembros, pre y post-entrenamiento, además de las correlaciones entre estas variables, considerando P<0,05. RESULTADOS: El miembro acometido (MA) mostró aumento significativo del torque extensor y flexor (P<0,01) en los modos y velocidades evaluados. En el miembro no acometido (MNA), eso ocurrió sólo para el modo excéntrico (30 y 120°/s), del torque extensor (P<0,01) y flexor (P<0,05 y P<0,01), respectivamente. En los saltos, hubo aumento en la distancia (simple y triple; P<0,05) y disminución del tiempo (cruzado y figura en 8; P<0,01), sin embargo, las correlaciones MPT x saltos se mostraron débiles (r<0,3) en el pre y post-entrenamiento, en los dos miembros. OBJECTIVE: To determine inter- and intraexaminer reliability of examiners without clinical experience in identifying and classifying myofascial trigger points (MTPs) in the shoulder muscles of subjects asymptomatic and symptomatic for unilateral subacromial impact syndrome (SIS). DESIGN: Within-day inter- and intraexaminer reliability study. SETTING: Physical therapy department of a university. PARTICIPANTS: Fifty-two subjects participated in the study, 26 symptomatic and 26 asymptomatic for unilateral SIS. INTERVENTIONS: Two examiners, without experience for assessing MTPs, independent and blind to the clinical conditions of the subjects, assessed bilaterally the presence of MTPs (present or absent) in 6 shoulder muscles and classified them (latent or active) on the affected side of the symptomatic group. Each examiner performed the same assessment twice in the same day. MAIN OUTCOME MEASURES: Reliability was calculated through percentage agreement, prevalence- and bias-adjusted kappa (PABAK) statistics, and weighted kappa. RESULTS: Intraexaminer reliability in identifying MTPs for the symptomatic and asymptomatic groups was moderate to perfect (PABAK, .46-1 and .60-1, respectively). Interexaminer reliability was between moderate and almost perfect in the 2 groups (PABAK, .46-.92), except for the muscles of the symptomatic group, which were below these values. With respect to MTP classification, intraexaminer reliability was moderate to high for most muscles, but interexaminer reliability was moderate for only 1 muscle (weighted κ=.45), and between weak and reasonable for the rest (weighted κ=.06-.31). CONCLUSIONS: Intraexaminer reliability is acceptable in clinical practice to identify and classify MTPs. However, interexaminer reliability proved to be reliable only to identify MTPs, with the symptomatic side exhibiting lower values of reliability.

This study aimed to compare muscle activity in individuals with isolated acromioclavicular osteoarthritis (ACO), ACO associated with rotator cuff disease (ACO+RCD), and controls. Seventy-four participants (23 isolated ACO, 25 ACO+RCD, 26 controls) took part in this study. Disability was assessed with the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire. Muscle activity of the upper trapezius (UT), lower trapezius (LT), serratus anterior (SA), and anterior deltoid (AD) was collected during arm elevation in the sagittal and scapular planes. Pain during motion was assessed with the numerical pain rating scale. Analysis of the DASH, pain and kinematics were reported in part 1 of this study. For each muscle, separate 2-way linear mixed-model ANOVAs were performed to compare groups. ACO+RCD group had more UT and AD activity than the the isolated ACO and control other groups, more AD activity than the isolated ACO group during the ascending phase, and more AD activity than the ACO and control groups during the descending phase in both planes. Isolated ACO group had less SA activity than the control group only in the sagittal plane. Alterations in shoulder muscle activity are present in individuals with isolated ACO and with ACO+RCD and should be considered in rehabilitation.

This study investigated the effects of the isokinetic eccentric training (IET) on the knee extensor and flexor torque and kinematic gait parameters in individuals with ACL reconstruction. Sixteen men with ACL reconstructed (ACLr) whose torque and the gait were evaluated, before and after 12 weeks of IET, was compared to a control group (14 individuals). Student t, MANOVA and ANOVA tests were performed with 5% of significance. The training increased the isometric, concentric at 30 and 120º/s (p < .05) and eccentric at 30º/s (p < .01) extensor torque on the affected limb (AL), and eccentric at 30 and 120º/s (p < .01), on the non-affected limb (NAL). In the flexors, there was an increase on the torque: isometric, concentric at 30º/s and eccentric at 30 and 120º/s (p < .01) in AL and in eccentric at 30 (p < .05) and 120º/s (p < .01) in NAL. With respect to the angular and spatio-temporal variables gait, there was no difference between pre-and post-training in LCAr group. Compared to control group, the cycle time, in two members, was lower in LCAr group, and stride length and cadence were higher in the AL of the LCAr (p < .05). Moreover, the knee flexion-extension angles (minimum and maximum) remained lower in LCAr, pre- and post-training (p < .01). The torque gain associated with eccentric isokinetic training did not affect the kinematic parameters of gait in patients undergoing ACL reconstruction...


"Efeitos do treinamento isocinético excêntrico de extensores de joelho e torque flexor na marcha de indivíduos com reconstrução do LCA: Um ensaio clínico controlado." Este estudo investigou os efeitos do treinamento isocinético excêntrico (TIE) sobre o torque extensor e flexor do joelho e parâmetros cinemáticos da marcha de indivíduos com reconstrução do LCA. Dezesseis homens com LCA reconstruído (LCAr), foram avaliados quanto ao torque e marcha, antes e após 12 semanas de TIE e comparados com um grupo controle (14 indivíduos). Testes t Student, MANOVA e ANOVA foram realizados com 5% de significância. O treinamento aumentou o torque extensor isométrico, concêntrico a 30 e 120º/s (p < 0,05) e excêntrico a 30º/s (p < 0,01) no membro afetado (MA), e excêntrico a 30 e 120º/s (p < 0,01), no membro não afetado (MNA). Nos flexores, houve um aumento no torque: isométrico, concêntrico a 30º/s e excêntrico a 30 e 120º/s (p < 0,01) no MA, e excêntrico a 30 (p < 0,05) e 120º/s (p < 0,01) no MNA. Com relação às variáveis espaço-temporais e angulares da marcha, não houve diferença entre as avaliações pré e pós-treino no grupo LCAr. Comparado ao controle, a duração do ciclo, nos dois membros, foi menor no LCAr, e comprimento da passada e cadência foram maiores no MA do grupo LCAr (p < 0,05). Além disso, os ângulos (mínimo e máximo) de flexão-extensão do joelho permaneceram menores no LCAr, pré e pós-treino (p < 0,01). O ganho de torque associado ao treinamento isocinético excêntrico não modificou os parâmetros cinemáticos da marcha nos indivíduos submetidos à reconstrução do LCA...


"Efectos del entrenamiento excéntrico isocinético en extensor de la rodilla y el par flexor y sobre la marcha de las personas con reconstrucción ACL: Un ensayo clínico controlado." Este estudio investigó los efectos del entrenamiento isocinético excéntrico (EIE) en el torque del extensor y del flexor de la rodilla y parámetros cinemáticos de la marcha de personas con la reconstrucción del LCA. Dieciséis hombres con LCA reconstruido (LCAr), fueron evaluados para el par y la marcha antes y después de 12 semanas de EIE y se compararon con un grupo control (14 personas). Prueba t Student, ANOVA y MANOVA se realizaron con 5 % de significación. La formación aumentó extensor torque isométrico, concéntrico 30 y 120°/s (p < 0,05) y la excéntrica 30°/s (p < 0,01) en el miembro afectado (MA), y la excéntrica 30 y 120°/s (p < 0,01) en el miembro no afectado (MNA). Flexor, hubo un aumento en el par motor: isométrica , concéntrica 30°/s excéntrica 30 y 120°/s (p < 0,01) en MA, excéntrico y 30 (p < 0,05 ) y 120°/ s (p < 0,01) en el MNA. Con respecto a las variables angulares y espacio-temporal de andar, no hubo diferencia entre pre y post-entrenamiento en grupo LCAr. En comparación con el grupo control, el tiempo de ciclo, em los dos miembros, fue menor en LCAr, y la longitud del paso y cadencia fueron mayores en el LCAr del MA (p < 0,05). Por otra parte, los ángulos de flexión-extensión de la rodilla (mínimo y máximo) se mantuvieron bajos en LCAr, pre y post-entrenamiento (p < 0,01). El aumento del torque asociado con el entrenamiento isocinético excéntrico no afectó los parámetros cinemáticos de la marcha en las personas sometidas a la reconstrucción del LCA...

This study described the three-dimensional shoulder motion during the arm elevation in individuals with isolated acromioclavicular osteoarthritis (ACO) and ACO associated with rotator cuff disease (RCD), as compared to controls. Seventy-four participants (ACO=23, ACO+RCD=25, Controls=26) took part of this study. Disability was assessed with the DASH, three-dimensional kinematics were collected during arm elevation in the sagittal and scapular planes, and pain was assessed with the 11-point numeric pain rating scale. For each kinematic variable and demographic variables, separate linear mixed-model 2-way ANOVAs were performed to compare groups. Both ACO groups had higher DASH and pain scores. At the scapulothoracic joint, the isolated ACO group had greater internal rotation than control, and the ACO+RCD group had greater upward rotation than both other groups. At the sternoclavicular joint, both groups with ACO had less retraction, and the isolated ACO group had less elevation and posterior rotation. At the acromioclavicular joint, the isolated ACO group had greater upward rotation, and both ACO groups had greater posterior tilting. Patients with ACO had altered shoulder kinematics, which may represent compensatory responses to reduce pain and facilitate arm motion during arm elevation and lowering.

The aim of the study was to compare static postural balance of healthy individuals of three age groups in different conditions of support and vision. Seventy one individuals, divided into 3 groups, were analyzed: young group (YG: 22.2 ± 2.1 years), middle aged group (MAG: 50.7 ± 5.7 years) and older individuals group (EG: 66.8 ± 5.4 years). Their balance was tested on a force platform, under 3 support and 3 visual conditions. Measures included: total (TD), anterior-posterior (APD) and mediolateral displacement (MLD) of the center of pressure (CoP). ANOVA revealed significant differences for interactions between group X support conditions and group X visual conditions for the 3 variables (p<0.01), with greater displacements for the MAG and EG groups during single-leg stance with partial and occluded vision (p<0.05). Static postural balance decreased over time in healthy individuals, and conditions of support and visual negatively affected balance with the increment of age.


O objetivo do estudo foi comparar o equilíbrio postural estático de indivíduos saudáveis de três faixas etárias em diferentes condições de apoio e de visão. Foram analisados 71 sujeitos, divididos em 3 grupos: jovens (GJ: 22,2±2,1 anos); meia idade (GMI: 50,7±5,7 anos) e idosos (GI: 66,8±5,4 anos). O equilíbrio foi testado numa plataforma de força, em 3 condições de apoio e 3 condições de visão, avaliando-se o deslocamento total (DT), ântero-posterior (DAP) e médio-lateral (DML) do centro de pressão (CP). ANOVA mostrou interação grupo X condições de apoio e grupo X condições visuais, nas 3 variáveis estudadas (p<0,01), com maiores deslocamentos no GMI e GI no apoio unipodal com visão parcial e ausente (p<0,05). O equilíbrio postural estático em indivíduos saudáveis diminuiu ao longo do tempo e que as condições de apoio e de visão podem afetá-lo negativamente com o avançar da idade.


El objetivo del estudio fue comparar el equilibrio postural estático de individuos sanos de tres grupos de edad em diferentes condiciones de apoyo y visón. Fueran analizados 71 sujetos, divididos en 3 grupos: jóvenes (GJ: 22,2±2,1 años), de mediana edad (GME: 50,7±5,7 años) y ancianos (GA: 66,8±5,4 años). El equilibrio fue probado en una plataforma de fuerza, em 3 condiciones de apoyo y 3 condiciones de visión, evaluando el desplazamiento total (DT), anteroposterior (DAP) y mediolateral (DML) del centro de presión (CP). ANOVA mostró interacción grupo X condiciones de apoyo y grupo X condiciones visuales, en las 3 variables (p<0,01), con grandes desplazamientos en GME y GA en apoyo unipodal con visión parcial y ausente (p<0,05). El equilibrio postural estático disminuyó en individuos sanos con el tiempo y que las condiciones de apoyo y de visión pueden afectarle negativamente con la edad.

Introdução: O processo de envelhecimento é acompanhado pela diminuição do controle da coordenação e do equilíbrio, constituindo fator de risco para quedas e perda da independência. No entanto, a prática de atividade física moderada, como a desenvolvida na Escolade Posturas, tem se mostrado benéfi ca para a prevenção, manutenção e retardo das deficiências funcionais do envelhecimento. Objetivo: Verificar os efeitos de um programa de Escola de Posturas sobre oequilíbrio e a coordenação de idosos não institucionalizados. Métodos:Doze indivíduos (67 ± 6,8 anos) participaram de um programa de Escola de Posturas (16 aulas teórico-práticas e 4 sobre o processode envelhecimento), realizado duas vezes por semana durante 10 semanas. Para avaliar o equilíbrio e a coordenação foi utilizada aAvaliação da Mobilidade Orientada pelo Desempenho da Marchae Equilíbio - POMA, de Tinetti (adaptado), aplicada antes e apóso término do programa. Para a comparação das variáveis entre as avaliações foi aplicado o teste não-paramétrico de Wilcoxon, comum nível de signifi cância de 5 por cento. Resultados: Houve melhora no equilíbrio e na coordenação (p < 0,01) após o término do programa. Conclusão: A forma intervencionista teórico-prática do programade Escola de Posturas melhorou o equilíbrio e a coordenação dosidosos.


Introduction: The aging process is characterized by reductionof coordination and balance control, which may constitute a riskfactor for falls and loss of independence. However, the practiceof moderate physical activity, as developed at Back School, hasshown beneficial for prevention, maintenance, and delayed ofaging functional defi cits. Objectives: To verify the effects of a Back School program on balance and coordination of non institutionalizedelderly people. Methods: Twelve subjects (67 ± 6.8 years)participated of a Back School program (16 theoretic-practical and4 about the aging process lessons) that was performed twice a weekduring 10 weeks. For the balance and coordination evaluations,the “Performance-Oriented Mobility Assessment of Gait and Balance”- POMA, of Tinetti (adapted), was applied before and afterthe program. For comparison of variables between the evaluations, non-parametric test (Wilcoxon) was used and adopted a significance level of 5 percent. Results: There was an improvement in the balance andin the coordination (p < 0.01) after the program. Conclusion: The theoretic-practical intervention of Back School program improved balance and coordination of elderly people.

O objetivo deste estudo foi comparar a atividade eletromiográfica (EMG) dos músculos reto femoral, bíceps femoral, tibial anterior e sóleo no agachamento, associando a posição de tronco ereto com 2 ângulos de flexão do joelho (40° e 60°) e a posição de tronco fletido a 45° com 3 ângulos de flexão do joelho (40°, 60° e 90°). Todas as combinações foram realizadas com e sem acréscimo de carga (10kg). A amostra foi composta por 12 indivíduos saudáveis com idade de 21,1 ± 2,5 anos e massa corporal de 62,8 ± 7,4kg. O EMG dos músculos citados foi registrado, isometricamente, em 10 posições de agachamento. Para a análise estatística foi aplicada ANOVA Two-Way de Friedman e o teste Post-Hoc de Newman-Keuls. Os resultados mostraram co-ativação entre os músculos reto femoral e bíceps femoral nas posições de tronco fletido e joelho em flexão de 40º e, entre os músculos reto femoral e sóleo, nas demais posições (p < 0,05). Houve co-ativação entre o tibial anterior e bíceps femoral com o joelho a 40º, com o tronco ereto e fletido e, entre o tibial anterior e sóleo, nas demais posições (p < 0,05). Quanto à ativação muscular isolada, a maior flexão do joelho no agachamento foi um fator determinante para maior ativação dos músculos, exceto para o sóleo. A posição do tronco e a carga adicional de 10kg influenciaram a ativação muscular do reto femoral, a 60° de flexão de joelho, na qual o tronco ereto proporcionou maior ativação. E para o bíceps femoral, a 40° de flexão de joelho, na qual o tronco flexionado proporcionou maior ativação. A co-ativação entre o reto femoral e o bíceps femoral na posição de tronco fletido, e entre o reto femoral e o sóleo nas demais posições, apontam para novas possibilidades de exercícios na reabilitação.


The aim of this study was to compare the electromyographic (EMG) activity of the femoris rectus, femoris biceps, tibialis anterior and soleus muscles in squatting, associating the trunk in erect position with two angles of knee flexion (40° and 60°) and the trunk at 45° flexion with three angles of knee flexion (40°, 60° and 90°). All associations were performed with and without additional load (10 kg). The sample was composed of 12 healthy individuals with mean age of 21.1 ± 2.5 years and weight of 62.8 ± 7.4 kg. The EMG of the cited muscles was isometrically registered in 10 squatting positions. For statistical analysis, Friedman Two-Way ANOVA and the Newman-Keuls Post-Hoc test were used. The results showed co-activation between the femoris rectus and femoris biceps muscles with the trunk in flexion and at 40° of knee flexion and between the femoris rectus and soleus muscles in the other positions considered (p < 0.05). It was also possible to observe co-activation between tibialis anterior and femoris biceps muscles with knee at 40° and 60° of flexion, with the trunk erect and in flexion and between the tibialis anterior and soleus muscles in the other positions (p < 0.05). Concerning isolated muscular activation, higher knee flexion in squatting was an important factor to greater muscles activation, except for the soleus. Trunk position and the additional load of 10 kg have influenced in the muscular activation of the femoris rectus at 60° of knee flexion, in which the erect trunk provided more activation. The femoris biceps presented greater activation when the knee was in 40° of flexion and the trunk flexioned. The co-activation between the femoris rectus and biceps with the trunk in flexion, and between the femoris rectus and soleus in the other positions, lead to new possibilities of exercises in rehabilitation.
