Firefighters' occupational activity causes cancer, and the characterization of exposure during firefighting activities remains limited. This work characterizes, for the first time, firefighters' exposure to (coarse/fine/ultrafine) particulate matter (PM) bound polycyclic aromatic hydrocarbons (PAHs) and metal(loid)s during prescribed fires, Fire 1 and Fire 2 (210 min). An impactor collected 14 PM fractions, the PM levels were determined by gravimetry, and the PM-bound PAHs and metal(loid)s were determined by chromatographic and spectroscopic methodologies, respectively. Firefighters were exposed to a total PM level of 1408.3 and 342.5 µg/m3 in Fire 1 and Fire 2, respectively; fine/ultrafine PM represented more than 90% of total PM. Total PM-bound PAHs (3260.2 ng/m3 in Fire 1; 412.1 ng/m3 in Fire 2) and metal(loid)s (660.8 ng/m3 versus 262.2 ng/m3), distributed between fine/ultrafine PM, contained 4.57-24.5% and 11.7-12.6% of (possible/probable) carcinogenic PAHs and metal(loid)s, respectively. Firefighters' exposure to PM, PAHs, and metal(loid)s were below available occupational limits. The estimated carcinogenic risks associated with the inhalation of PM-bound PAHs (3.78 × 10-9 - 1.74 × 10-6) and metal(loid)s (1.50 × 10-2 - 2.37 × 10-2) were, respectively, below and 150-237 times higher than the acceptable risk level defined by the USEPA during 210 min of firefighting activity and assuming a 40-year career as a firefighter. Additional studies need to (1) explore exposure to (coarse/fine/ultrafine) PM, (2) assess health risks, (3) identify intervention needs, and (4) support regulatory agencies recommending mitigation procedures to reduce the impact of fire effluents on firefighters.

Water pollution is a global environmental issue, and the presence of pharmaceutical compounds, such as tetracyclines (TCs), in aquatic ecosystems has raised growing concerns due to the potential risks to both the environment and human health. A high surface area CeO2 was prepared via atmospheric thermal treatment of a metal-organic framework of cerium and benzene-1,3,5-tricarboxylate. The effects of calcination temperature on the morphology, structure, light absorption properties and tetracycline removal efficiency were studied. The best activity of the photocatalysts could be achieved when the heat treatment temperature is 300 °C, which enhances the photocatalytic degradation performance towards tetracycline under visible light. The resulting CeO2 particles have high capacity for adsorbing TCs from aqueous solution: 90 mg g-1 for 60 mg L-1 TCs. As a result, 98% of the initial TC can be removed under simulated sunlight irradiation. The cooperation of moderate defect concentration and disordered structure showed tetracycline removal activity about 10 times higher than the initial Ce-MOF. An embryotoxicity assessment on zebrafish revealed that treatment with CeO2 particles significantly decreased the toxicity of TC solutions.

BACKGROUND: Latin America comprises an extensive and diverse territory composed of 33 countries in the Caribbean, Central, and South America where Romance languages-languages derived from Latin are predominantly spoken. Economic disparities exist, with inequitable access to pediatric surgical care. The Latin American Surgical Outcomes Study in Pediatrics (LASOS-Peds), a multi-national collaboration, will determine safety of pediatric anesthesia and perioperative care. OBJECTIVE: Below, we provide a descriptive initiative to share how pediatric anesthesia in Brazil, Chile, and Mexico operate. Theses descriptions do not represent all of Latin America. DESCRIPTIONS AND CONCLUSIONS: Brazil an upper middle-income country, population 203 million, has a public system insufficiently resourced and a private system, resulting in inequitable safety and accessibility. Surgical complications constitute the third leading cause of mortality. Anesthesiology residency is 3 years, with required rotations in pediatric anesthesia; five hospitals offer pediatric anesthesia fellowships. Anesthesiology is a physician-only practice. A Pediatric Anesthesia Committee within the Brazilian Society of Anesthesiology offers education through seasonal courses and workshops including pediatric advanced life support. Chile is a high-income country, population 19.5 million, the majority cared for in the public system, the remainder in university, private, or military systems. Government efforts have gradually corrected the long-standing anesthesiology shortage: twenty 3-year residency programs prepare graduates for routine pediatric cases. The Chilean Society of Anesthesiology runs a 1-month program for general anesthesiologists to enhance pediatric anesthesia skills. Pediatric anesthesia fellowship training occurs in Europe, USA, and Australia, or in two 2-year Chilean university programs. Public health policies have increased the medical and surgical pediatric specialists and general anesthesiologists, but not pediatric anesthesiologists, which creates safety concerns for neonates, infants, and medically complex. Chile needs more pediatric anesthesia fellowship programs. Mexico, an upper middle-income country, with a population of about 126 million, has a five-sector healthcare system: public, social security for union workers, state for public employees, armed forces for the military, and a private "self-pay." There are inequities in safety and accessibility for children. Pediatric Anesthesiology fellowship is 2 years, after 3 years residency. A shortage of pediatric anesthesiologists limits accessibility and safety for surgical care, driven by added training at low salary and hospital under appreciation. The Mexican Society of Pediatric Anesthesiology conducts refresher courses, workshops, and case conferences. Insufficient resources and culture limits research.

Chromoblastomycosis (CBM) and phaeohyphomycosis (FEO) are infections caused by melanized filamentous fungal agents, primarily found in tropical and subtropical regions. Both infections pose significant challenges for the correct identification of the causative agent due to their morphological similarity, making conventional methods of morphological analysis highly subjective. Therefore, molecular techniques are necessary for the precise determination of these species. In this regard, this study aimed to contribute to a new methodology based on PCR-RFLP for the identification of agents causing CBM and FEO. Sequences from the Internal Transcribed Spacer (ITS) region were used to identify potential restriction enzyme sites in silico, followed by in vitro validation using the selected restriction enzymes. The obtained results were compared with species identification through morphological analyses and sequencing. The results demonstrated that the PCR-RFLP applied in this study accurately identified two major agents of chromoblastomycosis, Fonsecaea pedrosoi and Fonsecaea monophora, as well as Cladophialophora bantiana and Exophiala dermatitidis, both causative agents of phaeohyphomycosis. In this context, the proposed assay can complement current methods for identifying these species, aiding in diagnosis, and contributing to the proper management of these infections.

Patients are harmed by the undertreatment of pain and suffering, transmission of disease, as well as the risk associated with impaired vigilance. Healthcare providers may be harmed by the physical and mental impact of their addictions. Healthcare systems are placed in jeopardy by the legal impact associated with illegal routes of drug release including sanction and financial liability and loss of public trust. Healthcare institutions have implemented many measures to reduce diversion from the perioperative area. These efforts include education, medical record surveillance, automated medication dispensing systems, urine drug testing, substance waste management systems, and drug diversion prevention teams. This narrative review evaluates strengths, weaknesses, and effectiveness of these systems and provides recommendations for leaders and care providers.

Oral squamous cell carcinoma (OSCC) represents ~90% of all oral cancers, being the eighth most common cancer in men. The overall 5-year survival rate is only 39% for metastatic cancers, and currently used chemotherapeutics can cause important side effects. Thus, there is an urgency in developing new and effective anti-cancer agents. As both chalcones and 1,2,3-triazoles are valuable pharmacophores/privileged structures in the search for anticancer compounds, in this work, new 1,2,3-triazole-chalcone hybrids were synthesized and evaluated against oral squamous cell carcinoma. By using different in silico, in vitro, and in vivo approaches, we demonstrated that compound 1f has great cytotoxicity and selectivity against OSCC (higher than carboplatin and doxorubicin) and other cancer cells in addition to showing minimal toxicity in mice. Furthermore, we demonstrate that induced cell death occurs by apoptosis and cell cycle arrest at the G2/M phase. Moreover, we found that 1f has a potential affinity for MDM2 protein, similar to the known ligand nutlin-3, and presents a better selectivity, pharmacological profile, and potential to be orally absorbed and is not a substrate of Pg-P when compared to nutlin-3. Therefore, we conclude that 1f is a good lead for a new chemotherapeutic drug against OSCC and possibly other types of cancers.

The concentrations of respirable particulate matter (PM) and the impact on indoor air quality in occupational settings remains poorly characterized. This study assesses, for the first time, the cumulative and non-cumulative concentrations of 14 fractions of coarse (3.65-9.88 µm), fine (0.156-2.47 µm), and ultrafine (0.015-0.095 µm) PM inside the garage of heavy vehicles, firefighting personal protective equipment' storage room, bar, and a common area of seven Portuguese fire stations. Sampling campaigns were performed during a regular work week at the fire stations. Levels of daily total cumulative PM ranged from 277.4 to 413.2 µg/m3 (maximum values of 811.4 µg/m3), with the bar (370.1 µg/m3) and the PPE' storage room (361.3 µg/m3) presenting slightly increased levels (p > 0.05) than the common area (324.8 µg/m3) and the garage (339.4 µg/m3). The location of the sampling site, the proximity to local industries and commercial activities, the layout of the building, the heating system used, and indoor sources influenced the PM concentrations. Fine (193.8-301.0 µg/m3) and ultrafine (41.3-78.2 µg/m3) particles were predominant in the microenvironments of all fire stations and accounted for 71.5% and 17.8% of daily total cumulative levels, respectively; coarse particles (23.3-47.1 µg/m3) represented 10.7% of total PM. The permissible exposure limit (5.0 mg/m3) defined by the Occupational Safety and Health Organization for respirable dust was not overcome in the evaluated fire stations. Results suggest firefighters' regular exposure to fine and ultrafine PM inside fire stations which will contribute to cardiorespiratory health burden. Further studies are needed to characterize firefighters' exposure to fine and ultrafine PM inside fire stations, identify main emission sources, and evaluate the contribution of exposures at fire stations to firefighters' occupational health risks.

Acquired immune deficiency syndrome (AIDS) is an infectious disease caused by the types 1 and 2 human immunodeficiency virus (HIV-1 and HIV-2). Clinical outcomes in patients are highly varied and delineated by complex interactions between virus, host, and environment, such as with help of co-receptors, for example, the C-C chemokine receptor 5 (CCR5). This work aimed to describe the scientific evidence relating the influence of CCR5 polymorphisms in association studies for HIV-1 disease susceptibility, severity, and transmissibility. This is a systematic review of the literature on single nucleotide polymorphisms (SNPs) and the deletion [Insertion and Deletion (Indel)] Δ32 of CCR5. The search for articles was based on the ScienceDirect, PubMed, and Coordination for the Improvement of Higher Education Personnel (CAPES) databases for the period between 2001 and 2021. The final sample consisted of 32 articles. †SNP rs1799987 is one of the genetic polymorphisms most associated with the criteria of susceptibility and severity of HIV-1, having distinct consequences in genotypic, allelic, and clinical analysis in the variability of investigated populations. As for the transmission character of the disease, the G mutant allele of rs1799987 corresponds to the highest positive association. ‡Furthermore, the results on Indel Δ32 corroborate the absence and rarity of this variant in some populations. Finally, mitigating the severity of cases, SNPs rs1799988 and rs1800023 obtained significant attribution in individuals in the studied populations. It is shown that the reported polymorphisms express significant influences for the evaluation of diagnostic, therapeutic, and prophylactic measures for HIV-1 having fundamental particularities in the molecular, genetic, and transcriptional aspects of CCR5.

Occupational exposure as a firefighter is a complex activity that continuously exposes subjects to several health hazards including fire emissions during firefighting. Firefighters are exposed to polycyclic aromatic hydrocarbons (PAHs), known as toxic, mutagenic, and carcinogenic compounds, by inhalation, dermal contact, and ingestion. In this work, a literature overview of firefighters' dermal exposure to PAHs after firefighting and data retrieved from skin in vitro/in vivo studies related to their dermal absorption, bioavailability, and associated toxicological and carcinogenic effects are reviewed. The evidence demonstrates the contamination of firefighters' skin with PAHs, mainly on the neck (2.23-62.50 ng/cm2), wrists (0.37-8.30 ng/cm2), face (2.50-4.82 ng/cm2), and hands (1.59-4.69 ng/cm2). Concentrations of possible/probable carcinogens (0.82-33.69 ng/cm2), including benzopyrene isomers, were found on firefighters' skin. PAHs penetrate the skin tissues, even at low concentrations, by absorption and/or diffusion, and are locally metabolized and distributed by the blood route to other tissues/organs. Lighter PAHs presented increased dermal permeabilities and absorption rates than heavier compounds. Topical PAHs activate the aryl hydrocarbon receptor and promote the enzymatic generation of reactive intermediates that may cause protein and/or DNA adducts. Future research should include in vitro/in vivo assays to perform a more realistic health risk assessment and to explore the contribution of dermal exposure to PAHs total internal dose.

Resumo: Introdução: Os estudantes de Medicina vivem experiências de adoecimento e dificuldades em saúde mental frequentes e, muitas vezes, graves. Essas experiências se acentuaram durante a pandemia de doença por coronavírus (Covid-19). Nesse contexto, o treinamento em atenção plena e regulação emocional pode ser uma ferramenta útil de promoção da saúde mental nessa população. Relato de Experiência: Com o objetivo de disponibilizar esse treinamento para os estudantes de Medicina de uma instituição de ensino de Minas Gerais, foi ofertada uma disciplina sobre atenção plena e equilíbrio emocional. Ofereceu-se essa disciplina entre setembro e dezembro de 2020, com 13 aulas síncronas remotas semanais, com duas horas de duração, para 16 estudantes. Após o transcurso da disciplina, aplicou-se um questionário on-line com perguntas abertas e fechadas com três seções: informações sociodemográficas, avaliação da disciplina baseada na Escala de Satisfação com a Experiência Acadêmica (ESEA) e avaliação de impactos no bem-estar subjetivo por meio do Questionário de Saúde Geral 12 (General Health Questionnaire 12 - GHQ-12). Treze estudantes responderam ao questionário da pesquisa. A disciplina foi considerada satisfatória pela maioria dos estudantes em todos os itens avaliados. Nos 12 aspectos de saúde mental pesquisados, a concordância com os impactos positivos da disciplina variou de oito a 12 respondentes. Discussão: Em consonância com as revisões sistemáticas sobre o tema, a disciplina "Atenção plena e equilíbrio emocional" parece ter impactado de forma positiva a saúde mental, a sociabilidade e a autoimagem dos estudantes. Todos os aspectos pedagógicos avaliados foram considerados satisfatórios por mais de 80% dos participantes, com exceção dos que avaliavam especificamente o formato remoto. O caráter optativo da oferta da disciplina pode ter contribuído de forma relevante para esse resultado. Conclusão: Considerando as particularidades do contexto pandêmico, foi possível disponibilizar uma proposta original de disciplina sobre atenção plena e equilíbrio emocional positivamente avaliada pelos estudantes. Novas pesquisas são necessárias para confirmar a associação entre a participação na disciplina e a promoção do bem-estar mental.


Abstract: Introduction: Medical students experience frequent and often severe mental illness and difficulties in mental health. These experiences were accentuated during the covid 19 pandemic. In this context, training in mindfulness and emotional balance can represent a useful tool for promoting mental health in this student group. Experience Report: In order to make this training available to medical students at a university in Minas Gerais, a course on Mindfulness and Emotional Balance was offered between September and December 2020 with 13 synchronous remote weekly classes, lasting two hours. After the course, the 16 participant students were invited to answer an online questionnaire with three sections: sociodemographic information, evaluation of the course based on the Satisfaction with Academic Experience Scale (ESEA); and assessment of impacts on subjective well-being based on the General Health Questionnaire 12 (General Health Questionnaire 12 - GHQ-12). Thirteen students answered the survey questionnaire. The discipline was considered satisfactory in all evaluated items. Within the 12 aspects of mental health surveyed, agreement with the impacts of the discipline ranged between 8 and 12 respondents. Discussion: In line with systematic reviews on the topic, the course of mindfulness and emotional balance seems to have positively impacted the students' mental health, sociability and self-image. All pedagogical aspects evaluated were considered satisfactory by more than 80% of the participants, with the exception of those who specifically evaluated the remote format. The optional nature of the course may have significantly contributed to this result. Conclusion: Considering the particular circumstances of the pandemic, it was possible to provide an original proposal for a discipline on mindfulness and emotional balance that was positively evaluated by students. Further research is needed to confirm the association between participation in the discipline and the promotion of mental well-being.

ABSTRACT People with Down Syndrome (DS) present metabolic differences when compared to the general population, which ends up at a higher prevalence of overweight and lower rates of aerobic fitness. The aim of this study was analysing the association of aerobic fitness and body composition in individuals with DS. Thirty individuals with DS from the city of Londrina / Brazil with a mean age of 18.00±3.66 years were evaluated. The variables body mass index, body fat, and aerobic fitness (VO²peak) were investigated. Men were taller than women, presented a higher percentage of lean body mass and higher VO²peak, while women showed higher means for android, gynoid and total fat. Aerobic fitness was positively associated with bone mineral density and inversely associated with nutritional status, lean body mass and body fat.


RESUMO Pessoas com Síndrome de Down (SD) apresentam diferenças metabólicas quando comparadas à população em geral, o que resulta em maior prevalência de sobrepeso e menores índices de aptidão aeróbia. O objetivo deste estudo foi analisar a associação da aptidão aeróbia com a composição corporal em indivíduos com SD. Foram avaliados 30 indivíduos com SD da cidade de Londrina / Brasil com média de idade 18,00±3,66 anos. Foram investigadas as variáveis índice de massa corporal, gordura corporal, e aptidão aeróbia (VO²pico). Os homens foram mais altos que as mulheres, apresentaram maior percentual de massa magra e maior VO²pico, enquanto as mulheres apresentaram maiores médias para gordura andróide, ginóide e total. A aptidão aeróbia associou-se inversamente ao índice de massa corporal, massa magra e gordura corporal.

Astrocyte reactivity in the spinal cord may occur after peripheral neural damage. However, there is no data to report such reactivity after Achilles tendon injury. We investigate whether changes occur in the spinal cord, mechanical sensitivity and gait in two phases of repair after Achilles tendon injury. Wistar rats were divided into groups: control (CTRL, without rupture), 2 days post-injury (RUP2) and 21 days post-injury (RUP21). Functional and mechanical sensitivity tests were performed at 2 and 21 days post-injury (dpi). The spinal cords were processed, cryosectioned and activated astrocytes were immunostained by GFAP at 21 dpi. Astrocyte reactivity was observed in the L5 segment of the spinal cord with predominance in the white matter regions and decrease in the mechanical threshold of the ipsilateral paw only in RUP2. However, there was gait impairment in both RUP2 and RUP21. We conclude that during the acute phase of Achilles tendon repairment, there was astrocyte reactivity in the spinal cord and impairment of mechanical sensitivity and gait, whereas in the chronic phase only gait remains compromised.

Leishmania infantum is a protozoan parasite that causes a vector borne infectious disease in humans known as visceral leishmaniasis (VL). This pathology, also caused by L. donovani, presently impacts the health of 500,000 people worldwide, and is treated with outdated anti-parasitic drugs that suffer from poor treatment regimens, severe side effects, high cost and/or emergence of resistant parasites. In previous works we have disclosed the anti-Leishmania activity of (-)-Epigallocatechin 3-O-gallate (EGCG), a flavonoid compound present in green tea leaves. To date, the mechanism of action of EGCG against Leishmania remains unknown. This work aims to shed new light into the leishmanicidal mode of action of EGCG. Towards this goal, we first confirmed that EGCG inhibits L. infantum promastigote proliferation in a concentration-dependent manner. Second, we established that the leishmanicidal effect of EGCG was associated with i) mitochondria depolarization and ii) decreased concentration of intracellular ATP, and iii) increased concentration of intracellular H2O2. Third, we found that the leishmanicidal effect and the elevated H2O2 levels induced by of EGCG can be abolished by PEG-catalase, strongly suggesting that this flavonoid kills L. infantum promastigotes by disturbing their intracellular redox balance. Finally, we gathered in silico and in vitro evidence that EGCG binds to trypanothione reductase (TR), a central enzyme of the redox homeostasis of Leishmania, acting as a competitive inhibitor of its trypanothione substrate.

BACKGROUND: The prevalence of Substance Use Disorders (SUD) and acceptance of drug testing among anesthetists in Brazil has not been determined. METHODS: An internet-based survey was performed to investigate the prevalence of SUD among anesthetists in Brazil, to explore the attitudes of anesthetists regarding whether SUD jeopardizes the health of an impaired provider or their patient, and to determine the provider's perspective regarding acceptance and effectiveness of drug testing to reduce SUD. The questionnaire was distributed via social media. REDCap was utilized to capture data. A sample size of 350 to achieve a confidence level of 95% and confidence interval of 5 was estimated. Study report was based on STROBE and CHERRIES statements. RESULTS: The survey was returned from 1,295 individuals. Most individuals knew an anesthesia provider with a SUD (82.07%), while 23% admitted personal use. The most common identified substances of abuse were opioids (67.05%). Very few respondents worked in a setting that performs drug testing (n = 17, 1.33%). Most individuals believed that drug testing could improve personal safety (82.83%) or the safety of patients (85.41%). Individuals with a personal history of SUD were less likely to believe in the effectiveness of drug testing to reduce one's own risk (74.92% vs. 85.18%, p < 0.0001) or improve the safety of patients (76.27% vs. 88.13%, p < 0.001). CONCLUSIONS: SUDs are common among anesthetists in Brazil. Drug testing would be accepted as a viable means to reduce the incidence although a larger study should be performed to investigate the logistical feasibility.

AIM: To evaluate the potential use of REBOA in a Brazilian hospital. METHODS: We performed a retrospective analysis of all requests for emergency surgical evaluation for patients with suspected massive hemorrhage, hospitalized in a private Brazilian general hospital, from April 1, 2017 to March 31, 2018. Inclusion criteria for REBOA eligibility were: suspected abdominal and/or pelvic bleeding, hemorrhagic shock and older than 18 years of age. Exclusion criteria were: older than 70 years of age, and pre-existing terminal disease or significant comorbidities. RESULTS: 90 evaluations were requested during the study period. However, according to our inclusion/exclusion criteria, only on 14 occasions (15.6%) there was a recommendation for the use of REBOA. Gynecological/obstetric conditions were the indication in 11 cases (78.6%) and elective oncologic surgery in three cases (21.4%). CONCLUSIONS: The use of REBOA is not common in our country, but it can be an essential tool, and perhaps the last resource in extremely severe conditions until the definitive surgical, endovascular or endoscopic treatment is performed.


OBJETIVO: Este estudo objetivou analisar o uso potencial Balão de Oclusão Endovascular da Aorta para Ressuscitação (REBOA) em um hospital brasileiro. MÉTODOS: Foi realizada uma revisão da literatura e uma análise retrospectiva de todas as avaliações cirúrgicas de emergência para pacientes com suspeita de hemorragia maciça internados em um hospital brasileiro, de 1 de abril de 2017 a 31 de março de 2018. Os critérios de elegibilidade do REBOA foram: origem abdominal e/ou pélvica, choque hemorrágico e acima de 18 anos de idade. Os critérios de exclusão foram: acima de 70 anos e doença terminal pré-existente ou comorbidades significativas. RESULTADOS: No período, foram solicitadas 90 avaliações. Em 14 ocasiões (15,6%) havia indicação para o uso do REBOA. Os casos em que isso foi possível foram devidos a causas ginecológicas/obstétricas em 11 casos (78,6%) e cirurgia oncológica eletiva em três casos (21,4%). CONCLUSÕES: O REBOA é ainda pouco utilizado em nosso país, mas pode ser uma ferramenta de extrema importância, e talvez o último recurso em pacientes extremamente graves, até que o tratamento definitivo, cirúrgico, endovascular ou endoscópico seja efetuado.

BACKGROUND: Wild capuchin monkeys (Sapajus libidinosus) usually are found in conserved forests near the zoo and the urban areas of Brasília city, Brazil. In this study, some capuchin monkeys were captured using traps, followed by safe biological procedures for their overall health analysis, based on specific haematological and biochemical tests of blood samples. METHODS: Blood was collected from a total of 17 monkeys for the determination of parameters, namely packed cell volume (PCV), leucocytes, erythrocytes, platelets and triglycerides. Statistical analyses for average values, median, standard deviation and range were performed. RESULTS: These parameters were set based on the minimum and maximum values obtained from the blood tests. Data are presented in tabulated form. CONCLUSIONS: Capture procedures were based on animal safety analysis for free-living animals and would help future studies on wild animals. The collected samples used in this study suggested the animals to be apparently healthy in their habitat.
