Introduction: recent studies have suggested the use of neck circumference as a parameter capable of identifying risks of cardiometabolic complications and the accumulation of truncal fat caused by both antiretroviral therapy and the lifestyle of people with the human immunodeficiency virus (HIV).Objective: to investigate the relationship between neck circumference and anthropometric indicators and to assess cardiometabolic risk and truncal obesity through proposed cut-off points. Methods: cross-sectional study including 233 people with HIV. Demographic, socioeconomic, lifestyle and clinical data were collected using astructured questionnaire. The anthropometric evaluation included: weight, height, body mass index (BMI) measurements; waist (WC), neck (NC),arm and arm muscle circumferences; triceps and subscapular skinfolds and their sum. ROC curves were constructed to determine the accuracy of NC in predicting cardiometabolic risk in people living with HIV. Results: the sample was 57.5 % male, with a mean age of 38.4 years (95 %CI: 37.2-39.7 years). NC showed a positive and significant correlation with all anthropometric variables analyzed (p < 0.05), and a higher correlation strength with WC and BMI. The NC cut-off point selected as a predictor of risk of cardiac metabolic complications and truncal obesity in women was ≥ 32.4 cm, considering both WC and BMI. For men, the NC cut-off points were different when considering WC (≥ 39.6 cm) and BMI (≥ 38.1 cm) as a reference. It is worth noting that NC performed well in ROC curve analysis for men, while in women it was a poor performance.Conclusion: NC proved to be a promising indicator in the assessment of nutrition and health of people living with HIV, especially in men. Introducción: estudios recientes han sugerido el uso de la circunferencia del cuello como parámetro capaz de identificar los riesgos de complicaciones cardiometabólicas y la acumulación de grasa troncal causados tanto por la terapia antirretroviral como por el estilo de vida de las personas con el virus de la inmunodeficiencia humana (VIH). Objetivo: investigar la relación entre la circunferencia del cuello y los indicadores antropométricos y evaluar el riesgo cardiometabólico y la obesidad troncal a través de los puntos de corte propuestos. Métodos: estudio transversal que incluyó a 233 personas con VIH. Se recogieron datos demográficos, socioeconómicos, de estilo de vida y clínicos mediante un cuestionario estructurado. La evaluación antropométrica incluyó: medidas de peso, altura, índice de masa corporal (IMC); circunferencias de cintura (CC), cuello (CN), brazo (CA) y músculo del brazo (MCB); pliegues cutáneos del tríceps y subescapular y su suma. Se construyeron curvas ROC para determinar la precisión de la CN en la predicción del riesgo cardiometabólico en personas que viven con el VIH. Resultados: el 57,5 % de la muestra eran varones, con una edad media de 38,4 años (IC 95 %: 37,2-39,7 años). La CN mostró una correlación positiva y significativa (p < 0,05) con todas las variables antropométricas analizadas, y una mayor fuerza de correlación con la CC y el IMC. El punto de corte de la CN seleccionado como predictor de riesgo de complicaciones metabólicas cardiacas y obesidad troncular en mujeres fue ≥ 32,4 cm, considerando tanto la CC como el IMC. Objective: To test the reliability and validity of the Dutch Eating Behavior Questionnaire (DEBQ) in an online format in university students from low-income regions. Methods: We applied the questionnaire to a sample of 195 and 117 university students from a low-income region (Gini index of 0.56) to study validity and reliability, respectively. The DEBQ consists of 33 items on eating behavior in three dimensions/factors: emotional eating, restrained eating and external eating. The questionnaire was administered twice at 2-week intervals. We tested the reliability via temporal stability and internal consistency and construct validity via exploratory and confirmatory factor analysis. Results: For reliability, we identified an acceptable Spearman correlation coefficient (rho > 0.30 and p < 0.05) and Cronbach's alpha (α ≥ 0.70) for all DEBQ items. In the exploratory analysis, we identified 6 factors representing a mix of original and additional factors, with an explained variance of 69.1%. In the confirmatory analysis with structural equation modeling, we observed better global model adjustment for the 6-factor model with the Tucker-Lewis index and comparative fit index closer to one, as well as root mean square error of approximation closer to zero than the original (3-factor) model. Using generalized structural equation modeling, we also observed a better fit in latent class modeling for the 6-factor model (AIC: 16990.67; BIC. 17874.38) than for the 3-factor model (AIC: 17904.09; BIC: 18342.67). Conclusion: The online format of the DEBQ has acceptable reliability and validity for measuring eating behavior in university students from low-income regions.

Introdução: O Brasil, assim como outros países, vem alterando seu perfil demográfico elevando o número de pessoas idosas, o que repercute em mudanças não só para sociedade, mas também para saúde pública. Este grupo de pacientes é mais vulnerável devido à fisiologia inerente ao envelhecimento, logo se tornam mais propensos ao uso de medicamentos que podem causar outros problemas de saúde. Essa probabilidade de risco é uma preocupação atual e levou a criação de métodos que norteiam os prescritores para adequarem suas terapêuticas neste grupo de pacientes. Um destes métodos é o critério de Beers, que é atualizado periodicamente trazendo uma lista de medicamentos potencialmente inapropriados (MPIs) para idosos. Objetivo: Avaliar a prescrição de pacientes idosos internados no Hospital Universitário da Universidade Federal de Juiz de Fora (HU-UFJF/Ebserh) quanto à prevalência do uso de MPI e polifarmácia, no período de julho a agosto de 2019. Material e Métodos: Estudo observacional descritivo e retrospectivo, cujos dados foram coletados de prontuários pacientes idosos com idade igual ou superior a 65 anos para obtenção dos resultados que foram avaliados estatisticamente. Resultados: Foram avaliados 187 prontuários, e observada prevalência de 80,2% da prescrição de MPIs, sendo os mais prevalentes omeprazol e benzodiazepínicos. A maioria dos pacientes tiveram polifarmácia (95,7%). Conclusão: Os resultados convergem com base no critério de Beers, para necessidade de adequar a terapia de pacientes idosos. É necessário também avaliar os benefícios e alternativas quanto aos MPIs mais prevalentes, além de realizar estudos observacionais sobre possíveis efeitos adversos que possam ser consequência do uso desses medicamentos, com objetivo de aperfeiçoar a terapia farmacológica e aprimorar a farmacoeconomia, melhorando assim a qualidade de vida dos pacientes idosos.


Introduction: Brazil, like other countries, has been changing its demographic profile, increasing the number of elderly people, which reflects in changes not only for society, but also for public health. This group of patients is more vulnerable due to the inherent physiology of aging, so they become more likely to use medications that can cause other health problems. This risk probability is a current concern and has led to the creation of methods that guide prescribers to adapt their therapies in this group of patients. One of these methods is the Beers criterion, which is periodically updated with a list of potentially inappropriate medications (PIM) for the elderly. Objective: To evaluate the prescription of elderly patients hospitalized at the University Hospital of Juiz de Fora (HU-UFJF/Ebserh) regarding the prevalence of the use of PIM and polypharmacy, from July to August 2019. Material and Methods: Descriptive and retrospective observational study, whose data were collected from medical records of elderly patients aged 65 years or older to obtain the results that were statistically evaluated. Results: A total of 187 medical records were evaluated, and a prevalence of 80.2% of the prescription of PIMs was observed, the most prevalent being omeprazol and benzodiazepines. Most patients had polypharmacy (95.7%). Conclusion: The results converge, based on the Beers criterion, for the need to suit the therapy of elderly patients. It is also necessary to evaluate the benefits and alternatives regarding the most prevalent PIMs, in addition to conducting observational studies on possible adverse effects that may be a consequence of the use of these medications, aiming to refine pharmacological therapy and improve pharmacoeconomics, thus improving quality of life of elderly patients.

METHODS: ISAD-PI was a population-based cross-sectional study that analyzed the living conditions and health status of the population residing in urban areas in the municipalities of Teresina and Picos, in Piauí. Sampling was carried out by conglomerates in two stages: Primary Sampling Units and households. To calculate the sample size, we considered the stratification of the population in both cities, according to the age of the individuals. We evaluated the "non-response" rate (NRR) and estimated the proportions according to sex and age, as well as the prevalence of social determinants of health in order to assess the compliance of the sampling plan. Analyses related to the precision according to the coefficient of proportion variation of the standard error (Cv-pˆ) and the design effect (deff). Cv-pˆ less than 20% and deff less than 1.5 were considered adequate. The total NRR of households was 38.2% in Teresina and 38.3% in Picos. We carried out twenty-four proportion estimates in relation to age and sex and 48 prevalence of social and health determinants estimates, totaling 72 estimates. Among them, 71 had Cv-pˆ less than 20% and 61 had deff less than or equal to 1.5. CONCLUSION: Data generated from the ISAD-PI may contribute to the assessment of health and morbidity conditions in the population. Furthermore, methodological aspects employed in this research may serve as a basis for studies carried out in other cities in Brazil.

A amostragem foi realizada por conglomerados, em dois estágios: Unidades Primárias de Amostragem e domicílios. Para o cálculo do tamanho da amostra, considerou-se a estratificação da população de ambas as cidades, de acordo com a idade dos indivíduos, para ambos os sexos. Para avaliação da conformidade do plano amostral, foi avaliada a taxa de "não-resposta" (TNR) e, além disso, as estimativas de proporções segundo sexo e idade, bem como as prevalências de determinantes sociais de saúde, que foram analisadas em relação à precisão por meio do coeficiente de variação da proporção do erro padrão (Cv-pˆ) e do efeito do delineamento (deff). Foram considerados adequados Cv-pˆ menores que 20%, e deff menores que 1,5. A TNR-Total dos domicílios foi de 38,2% em Teresina e de 38,3% em Picos. Foram realizadas 24 estimativas de proporção em relação à idade e ao sexo e 48 estimativas de prevalência de determinantes sociais e de saúde, totalizando 72 estimativas, das quais 71 apresentaram Cv-pˆ menor que 20% e 61 apresentaram deff menor ou igual a 1,5. OBJECTIVE To estimate the prevalence of hypertension in adults and analyze its association with socioeconomic, demographic, and lifestyle risk factors. METHODS Home-based cross-sectional epidemiological study conducted with adults of both sexes living in the cities of Teresina and Picos (PI). The prevalence of hypertension was estimated by self-reported diagnosis. The associations between hypertension and the following variables were tested: age, gender, skin color, education, family income, marital status, alcohol and cigarette consumption, physical activity, and nutritional status. RESULTS A total of 1,057 adult individuals with a mean age of 38.6 ± 11.5 years participated in the study, of which 62.3% were female. The prevalence of hypertension in the population was 16.4%. In the crude analysis, a significant association was observed between the prevalence of hypertension and factors such as older age, lower education, single marital status, brown skin color, smoking, and overweight (p < 0.05). However, after adjustments, only the associations with older age, brown skin color, sedentary lifestyle and overweight remained significant. CONCLUSION Older age, brown skin color, sedentary lifestyle, and the presence of overweight or obesity were significantly associated with hypertension. Among the modifiable risk factors, a sedentary lifestyle and overweight stand out as important for the genesis of hypertension, subject to intervention measures.

Introdução: A linhaça dourada apresenta importantes quantidades de ácido αlinolênico, fibras solúveis e lignanas, podendo trazer benefícios para a saúde. Objetivo: Desenvolver paçoquinha adicionada de farinha de linhaça dourada (FLD) em diferentes proporções, bem como determinar sua composição centesimal e aceitação sensorial. Materiais e métodos: Foram elaboradas paçoquinhas com adição de 10%, 15% e 20% de FLD. A composição centesimal foi determinada e, após, foram aplicados testes de aceitação (escala hedônica). Os dados foram analisados através de teste de Kruskal Wallis e ANOVA, adotando-se nível de significância de 5%. Resultado: As paçoquinhas padrão e adicionadas de FLD não apresentaram diferença no teor de umidade (17,1 a 18,5%) e lipídeos (30,8 a 34,2%). Quanto ao teor de cinzas, apenas as formulações com 15 e 20% de FLD tiveram maior percentual, 1,94% e 2,03%, respectivamente. O teor de proteínas foi significativamente menor nas formulações com adição de linhaça (15,4 a 18,5%) e a quantidade de fibras alimentares aumentou nas formulações com FLD. Em relação à análise sensorial, as paçoquinhas adicionadas de FLD obtiveram boa aceitabilidade, não sendo observadas diferenças significativas com relação aos atributos avaliados em comparação com a paçoquinha padrão. Conclusão: Os dados obtidos permitiram verificar que é possível a substituição parcial do amendoim em paçoquinhas por farinha de linhaça dourada, pois os novos produtos apresentaram características sensoriais que agradaram aos consumidores em todos os atributos avaliados, além de apresentarem valor nutricional superior no que se refere ao incremento de fibras e teor de minerais. Introduction: Golden flaxseed has important amounts of α-linolenic acid, soluble fibers and lignans, which can bring benefits to health. Objective: To prepare peanut candy added with golden flaxseed flour (GFF) in different proportions and determine its proximate composition and sensory acceptance. Materials and methods: Peanut candies were prepared with the addition of 10%, 15% and 20% of GFF. The proximate composition was determined and then acceptance tests (hedonic scale) were applied. Data were analyzed using the Kruskal Wallis test and ANOVA, adopting a significance level of 5%. Result: Regular peanut candy and a preparation added with GFF showed no difference in moisture (17.1 to 18.5%) and lipid (30.8 to 34.2%) content. As for ash content, only formulations with 15 and 20% of GFF had the highest percentage, 1.94% and 2.03%, respectively. The protein content was significantly lower (15.4 to 18.5%) and the amount of dietary fiber increased in formulations with GFF. In relation to sensory analysis, the candies added with GFF obtained good acceptability, with no significant differences regarding the attributes evaluated in comparison with the regular peanut candy. Conclusion: The data obtained allowed us to see that it is possible to partially replace peanuts with golden flaxseed flour in the preparation of candies, as the new products presented sensory characteristics that pleased consumers in all the evaluated attributes, in addition to superior nutritional value with regard to increased fiber and mineral content. SUMMARY OBJECTIVE To estimate the prevalence of hypertension in adults and analyze its association with socioeconomic, demographic, and lifestyle risk factors. METHODS Home-based cross-sectional epidemiological study conducted with adults of both sexes living in the cities of Teresina and Picos (PI). The prevalence of hypertension was estimated by self-reported diagnosis. The associations between hypertension and the following variables were tested: age, gender, skin color, education, family income, marital status, alcohol and cigarette consumption, physical activity, and nutritional status. RESULTS A total of 1,057 adult individuals with a mean age of 38.6 ± 11.5 years participated in the study, of which 62.3% were female. The prevalence of hypertension in the population was 16.4%. In the crude analysis, a significant association was observed between the prevalence of hypertension and factors such as older age, lower education, single marital status, brown skin color, smoking, and overweight (p < 0.05). However, after adjustments, only the associations with older age, brown skin color, sedentary lifestyle and overweight remained significant. CONCLUSION Older age, brown skin color, sedentary lifestyle, and the presence of overweight or obesity were significantly associated with hypertension. Among the modifiable risk factors, a sedentary lifestyle and overweight stand out as important for the genesis of hypertension, subject to intervention measures.


RESUMO OBJETIVO Estimar a prevalência de hipertensão arterial em adultos e analisar sua associação com fatores de risco socioeconômicos, demográficos e de estilo de vida. MÉTODOS Estudo epidemiológico transversal, de base domiciliar, realizado com adultos de ambos os sexos residentes nas cidades de Teresina e Picos (PI). A prevalência de hipertensão arterial foi estimada por meio de diagnóstico autorreferido. Foram testadas as associações entre a hipertensão arterial e as variáveis: idade, sexo, cor da pele, escolaridade, renda familiar, situação conjugal, consumo de álcool e cigarro, prática de atividade física e estado nutricional. RESULTADOS Participaram do estudo 1.057 indivíduos adultos com média de idade de 38,6 ± 11,5 anos, sendo 62,3% do sexo feminino. A prevalência de hipertensão arterial na população foi de 16,4%. Na análise bruta, observou-se associação significativa entre a prevalência de hipertensão e fatores como maior idade, menor escolaridade, estado civil solteiro, cor da pele parda, tabagismo e excesso de peso (p<0,05). Entretanto, após ajustes, apenas as associações com a maior idade, cor da pele parda, estilo de vida sedentário e excesso de peso mantiveram-se significativas. CONCLUSÃO A idade mais avançada, cor da pele parda, sedentarismo e a presença de sobrepeso ou obesidade apresentaram-se significativamente associados à hipertensão. Dentre os fatores de risco modificáveis, destaca-se o sedentarismo e o excesso de peso como importantes para a gênese de hipertensão e passíveis de medidas de intervenção.

Leishmania braziliensis is an intracellular parasite that resides mostly in macrophages. Both the parasite genome and the clinical disease manifestations show considerable polymorphism. Clinical syndromes caused by L. braziliensis include localized cutaneous (CL), mucosal (ML), and disseminated leishmaniasis (DL). Our prior studies showed that genetically distinct L. braziliensis clades associate with different clinical types. Herein, we hypothesized that: (1) L. braziliensis induces changes in macrophage gene expression that facilitates infection; (2) infection of macrophages with strains associated with CL (clade B), ML (clade C), or DL (clade A) will differentially affect host cell gene expression, reflecting their different pathogenic mechanisms; and (3) differences between the strains will be reflected by differences in macrophage gene expression after initial exposure to the parasite. Human monocyte derived macrophages were infected with L. braziliensis isolates from clades A, B, or C. Patterns of gene expression were compared using Affymetrix DNA microarrays. Many transcripts were significantly decreased by infection with all isolates. The most dramatically decreased transcripts encoded proteins involved in signaling pathways, apoptosis, or mitochondrial oxidative phosphorylation. Some transcripts encoding stress response proteins were up-regulated. Differences between L. braziliensis clades were observed in the magnitude of change, rather than the identity of transcripts. Isolates from subjects with metastatic disease (ML and DL) induced a greater magnitude of change than isolates from CL. We conclude that L. braziliensis enhances its intracellular survival by inhibiting macrophage pathways leading to microbicidal activity. Parasite strains destined for dissemination may exert a more profound suppression than less invasive L. braziliensis strains that remain near the cutaneous site of inoculation.

BACKGROUND: American Tegumentary Leishmaniasis (ATL) caused by Leishmania braziliensis is endemic in Corte de Pedra, Northeast Brazil. Most L. braziliensis infections manifest as localized cutaneous leishmaniasis (CL). Disseminated manifestations include mucosal leishmaniasis (ML), present at a low constant level for several decades, and newly emerging disseminated leishmaniasis (DL). Surprisingly, DL has recently surpassed ML in its spatial distribution. This led us to hypothesize that distinct forms of ATL might spread in different patterns through affected regions. METHODOLOGY/PRINCIPAL FINDINGS: We explored the incidence and geographic dispersion of the three clinical types of ATL over a span of nearly two decades in Corte de Pedra. We obtained the geographic coordinates of the homes of patients with ATL during 1992-1996, 1999-2003 and 2008-2011. The progressive dispersion of ML or DL in each time period was compared to that of CL in 2008-2011 with the Cusick and Edward's geostatistical test. To evaluate whether ATL occurred as clusters, we compared each new case in 2008-2011 with the frequency of and distance from cases in the previous 3 to 12 months. The study revealed that DL, ML and CL actively spread within that region, but in distinct patterns. Whereas CL and DL propagated in clusters, ML occurred as sporadic cases. DL had a wider distribution than ML until 2003, but by 2011 both forms were distributed equally in Corte de Pedra. The incidence of ML fluctuated over time at a rate that was distinct from those of CL and DL. CONCLUSIONS/SIGNIFICANCE: These findings suggest that CL and DL maintain endemic levels through successive outbreaks of cases. The sporadic pattern of ML cases may reflect the long and variable latency before infected patients develop clinically detectable mucosal involvement. Intimate knowledge of the geographic distribution of leishmaniasis and how it propagates within foci of active transmission may guide approaches to disease control.

Leishmania (Viannia) braziliensis causes three main types of American tegumentary leishmaniasis (ATL), localized cutaneous leishmaniasis (CL), mucosal leishmaniasis (ML), and disseminated leishmaniasis (DL). All forms are observed among individuals of Corte de Pedra, Brazil. We previously used random amplified markers to identify a multiclonal population among L. (V.) braziliensis isolates from ATL patients, defining parasite clades associated with different clinical syndromes. Herein we compared sequences of random amplified markers to identify genotypes of L. (V.) braziliensis recovered from lesions of CL, ML, and DL patients. Six polymorphic genomic loci were sequenced from 35 parasite isolates. Single-nucleotide polymorphisms (SNPs) and insertions-deletions (indels) at each locus allowed us to segregate the L. (V.) braziliensis population according to haplotypes. Several SNPs, indels, and haplotypes were significantly associated with an increased risk of DL. Molecular genotyping may provide markers to identify L. (V.) braziliensis strains likely to cause this emerging, hard-to-treat form of ATL.

Correlations between the genetic diversity of Leishmania infantum (syn. L. chagasi) isolates and their respective geographic origins support the theoretic assumption that visceral leishmaniasis probably originated in the Old World. Because dogs are widely considered to be the main reservoir of this disease, the present study aimed to investigate the degree of genetic divergence among 44 leishmanial canine isolates from two Brazilian cities, Jequié and Campo Grande, located approximately 2,028 km from each other. We hypothesized that a low degree of genetic divergence would be observed among these isolates. In fact, statistical analyses found no significant differences between the isolates using both random amplified polymorphic DNA and multilocus microsatellite typing genotyping techniques with three and seven markers, respectively. These findings provide support for the recent introduction of L. infantum into the New World.

BACKGROUND: Cutaneous leishmaniasis (CL) is treated with parenteral drugs for decades with decreasing rate cures. Miltefosine is an oral medication with anti-leishmania activity and may increase the cure rates and improve compliance. METHODOLOGY/PRINCIPAL FINDINGS: This study is a randomized, open-label, controlled clinical trial aimed to evaluate the efficacy and safety of miltefosine versus pentavalent antimony (Sb(v)) in the treatment of patients with CL caused by Leishmania braziliensis in Bahia, Brazil. A total of 90 patients were enrolled in the trial; 60 were assigned to receive miltefosine and 30 to receive Sb(v). Six months after treatment, in the intention-to-treat analyses, the definitive cure rate was 53.3% in the Sb(v) group and 75% in the miltefosine group (difference of 21.7%, 95% CI 0.08% to 42.7%, p = 0.04). Miltefosine was more effective than Sb(v) in the age group of 13-65 years-old compared to 2-12 years-old group (78.9% versus 45% p = 0.02; 68.2% versus 70% p = 1.0, respectively). The incidence of adverse events was similar in the Sb(v) and miltefosine groups (76.7% vs. 78.3%). Vomiting (41.7%), nausea (40%), and abdominal pain (23.3%) were significantly more frequent in the miltefosine group while arthralgias (20.7%), mialgias (20.7%) and fever (23.3%) were significantly more frequent in the Sb(v) group. CONCLUSIONS: This study demonstrates that miltefosine therapy is more effective than standard Sb(v) and safe for the treatment of CL caused by Leishmania braziliensis in Bahia, Brazil.
