Objetivo: identificar los patrones alimentarios de la población adulta Antioqueña y su asociación con variables socioeconómicas. Métodos: estudio transversal con muestra aleatoria de 1.394 adultos de 18 a 59 años, derivados del estudio Perfil alimentario y nutricional de Antioquia 2019. El consumo alimentario fue evaluado con recordatorio de consumo en 24 horas (R24h) y un segundo R24h en día no consecutivo al 25% de la muestra, se cuantificó el consumo de alimentos en gramos, los patrones alimentarios se establecieron mediante análisis factorial por Componentes principales con rotación ortogonal varimax. Para verificar la asociación entre los patrones alimentarios y las variables socioeconómicas se empleó regresión de Poisson por varianza robusta. Resultados: se definieron tres patrones alimentarios que representan el 28,1% de la varianza: "Tradicional" (9,8%), "Regional" (9,7%) y "Prudente" (8,6%). Los dos primeros se asociaron con variables demográficas, como sexo, edad y etnia; mientras que el último se asoció con variables socioeconómicas como escolaridad, ingresos familiares, régimen de seguridad social y clasificación de seguridad alimentaria. Conclusiones: Los hallazgos del presente estudio muestran que la dieta de la población estudiada, sigue unos referentes de tradición alimentaria, reflejados en los patrones Tradicional y Regional, mientras que el patrón Prudente, corresponde a la parte de la población que tiene mejores condiciones socioeconómicas y probablemente lo siguen por recomendación de salud.


Objective: to identify the eating patterns of the adult population from Antioquia, Colombia, and their association with socioeconomic variables. Methods: a cross-sectional study was performed with a random sample of 1,394 adults aged 18 to 59 years, derived from the Antioquia Food and Nutritional Profile 2019 study. Food consumption was evaluated with a 24-hour consumption recall (R24h) and a second R24h on a non-consecutive day at 25% of the sample, food consumption was quantified in grams, and food patterns were established by factorial analysis by Principal Components with varimax orthogonal rotation. To verify the association between eating patterns and socioeconomic variables, Poisson regression was used for robust variance. Results: three eating patterns were defined that represent 28.1% of the variance: "Traditional" (9.8%), "Regional" (9.7%), and "Prudent" (8.6%). The first two were associated with demographic variables, such as gender, age, and ethnicity; while the latter was associated with socioeconomic variables such as schooling, family income, social security system, and food security classification. Conclusions: The findings of this study show that the diet of the population of the study, follows some references to food tradition, reflected in the Traditional and Regional patterns, while the Prudent pattern corresponds to the part of the population that has better socioeconomic conditions, and they probably follow it for health recommendation.

Through structural equation modeling (SEM), this study aimed to evaluate the influence of commensality under the dietary pattern of Brazilian adolescents based on data from the third edition of the National Survey of School Health (PeNSE), conducted in 2015. PeNSE is a cross-sectional survey with a complex probabilistic sampling plan and with representativeness for adolescents in public and private schools in Brazil. Based on recommendations from the Food Guide for the Brazilian Population (Guia Alimentar Para a População Brasileira), we developed a theoretical model, using the SEM model, (stratified by sex) to investigate the relationship between sociodemographic characteristics, handwashing, body satisfaction, and commensality, and their influence on the eating pattern of adolescents. For males, age significantly and inversely proportionally associated with commensal practice (SDE: -0.28, p<0.01). Regarding female subjects, body dissatisfaction was shown to be negatively associated with commensality to a more significant effect when compared to males (SDE: -0.30, p<0.01). On the other hand, commensality positively associated with healthy eating patterns for males (SDE: 0.42, p<0.01) and females (SDE: 0.62, p<0.01). Through the analyses conducted in this study, a positive relationship between commensality and the Healthy Eating Pattern of Brazilian adolescents was observed. There were also different influences for each sex.

Heteroresistance, defined as the occurrence of apparently homogeneous subpopulations of microbial cells showing different levels of antimicrobial susceptibility is a problem that has been associated with therapeutical failure in cryptococcosis. The purpose of the study was an investigation on the level of heteroresistance to fluconazole (LHF) as observed in clinical and environmental C. neoformans/C. gattii complex species isolates from Amazonas State (AM), Brazil. A total of 45 isolates and 9 type strains were analyzed. The assessments comprised testing for minimal inhibitory concentrations (MICs), for LHFs, for the strains' capacity of adaptation to high fluconazole (FLC) concentrations above the LHF, and for the stability of the heteroresistance phenomenon. The mean MICs for clinical isolates of C. gattii (6.4 µg/ml) were higher than those observed for environmental C. gattii strains (1.7 µg/ml) and clinical (3.7 µg/ml) as well as environmental (1.5 µg/ml) C. neoformans isolates. The phenomenon of heteroresistance to FLC was recorded for all isolates. On average, the LHF (8-256 µg/ml) of the isolates was 16 times higher than the FLC MICs (0.5-16 µg/ml) and a proportion of 85% isolates showed LHFs ≥ 16 µg/ml, 40% even ≥ 32 µg/ml. According to the adaptation assay, a considerable number of isolates (58%) showed the capacity of adaptation to MICs even higher than the initially recorded LHF. After the adaptation experiment, the adaptative-LHF values (8-512 µg/ml) were about 60 times higher than the original MIC values. After nine subsequent passages in drug-free broth, the isolates had their adaptative-LHF reduced. However, the LHF did not revert to the initially measured level. Our findings challenge the clinical interpretation of the antifungal MIC testing and motivate future studies correlating the levels of heteroresistance and parameters like LHF and adaptative-LHF with cryptococcosis-associated morbidity and mortality. LAY SUMMARY: Cryptococcosis affects many people and is caused by fungi of the Cryptococcus neoformans/Cryptococcus gattii complexes. These agents appear to become more resistant to antifungals when exposed to increasing concentrations of antifungals due to a phenomenon called heteroresistance.

BACKGROUND: The emergence of diseases such as dyslipidemia, systemic arterial hypertension, insulin resistance and metabolic syndrome in children and adolescents has brought about a change in the epidemiologic profile of the pediatric population. As action to promote health in the school environment is a useful tool for changing the pattern of health/disease in the young population, the present study aimed to identify schools that promote healthy eating and physical activity and to study the relationship between these practices and the prevalence of overweight, hypertension, insulin resistance and hypercholesterolemia in adolescents. METHODS: A cross-sectional population-based study was conducted with 2400 adolescents aged from 12 to 17 years old and participating in the "Study of Cardiovascular Risk in Adolescents" (ERICA - Estudo de Riscos Cardiovasculares em Adolescente). The association between dependent (overweight, insulin resistance, hypertension and dyslipidemia) and independent variables (implementation of health promoting initiative in schools) was investigated using the chi-square test and prevalence ratio (PR) with a confidence index (CI) of 95%. RESULTS: The unsatisfactory implementation of a "health promoting environment" (PR = 1.02; CI 95%: 1.0; 1.04) and "partnerships with the health sector" (PR = 1.03; CI 95%: 1.01; 1.05) were linked to a high prevalence of overweight in adolescents. Hypercholesterolemia was found to be higher in the schools with unsatisfactory implementation of "healthy eating and health on the scholar curriculum" (PR = 1.71; CI 95%: 1.22; 2.44) and those lacking a "healthy-eating promoting environment" (PR = 1.29; CI 95%: 1.10; 1.54). Schools with unsatisfactory implementation of a "health-eating promoting environment" (PR = 1.36; CI 95%: 1.04; 1.79) and those lacking "partnership with the health sector" (PR = 2.12; CI 95%: 1.38; 3.24) had more adolescents with insulin resistance. There was no association between hypertension and any other component studied. CONCLUSION: Schools which have implemented adequate health promotion in their curriculums showed a lower prevalence of overweight, insulin resistance and hypercholesterolemia in adolescents.
