RESUMO

PURPOSE: To investigate measurement accuracy in terms of precision and inter-rater variability in the simultaneous volumetric assessment of lung, liver and lymph node metastasis size change over time in comparison to RECIST 1.1. MATERIALS AND METHODS: Three independent readers evaluated multislice CT data from clinical follow-up studies (chest/abdomen) in 50 patients with metastases. A total of 117 lung, 77 liver and 97 lymph node metastases were assessed manually (RECIST 1.1) and by volumetry with semi-automated software. The quality of segmentation and need for manual adjustments were recorded. Volumes were converted to effective diameters to allow comparison to RECIST. For statistical assessment of precision and interobserver agreement, the Wilcoxon-signed rank test and Bland-Altman plots were utilized. RESULTS: The quality of segmentation after manual correction was acceptable to excellent in 95 % of lesions and manual corrections were applied in 21 - 36 % of all lesions, most predominantly in lymph nodes. Mean precision was 2.6 - 6.3 % (manual) with 0.2 - 1.5 % (effective) relative measurement deviation (p <.001). Inter-reader median variation coefficients ranged from 9.4 - 12.8 % (manual) and 2.9 - 8.2 % (volumetric) for different lesion types (p < .001). The limits of agreement were ± 9.8 to ± 11.2 % for volumetric assessment. CONCLUSION: Superior precision and inter-rater variability of volumetric over manual measurement of lesion change over time was demonstrated in a whole body setting.

PURPOSE: Quantification of tumour burden in oncology requires accurate and reproducible image evaluation. The current standard is one-dimensional measurement (e.g. RECIST) with inherent disadvantages. Volumetric analysis is discussed as an alternative for therapy monitoring of lung and liver metastases. The aim of this study was to investigate the accuracy of semi-automated volumetric analysis of artificial lymph node metastases in a phantom study. MATERIALS AND METHODS: Fifty artificial lymph nodes were produced in a size range from 10 to 55mm; some of them enhanced using iodine contrast media. All nodules were placed in an artificial chest phantom (artiCHEST®) within different surrounding tissues. MDCT was performed using different collimations (1-5 mm) at varying reconstruction kernels (B20f, B40f, B60f). Volume and RECIST measurements were performed using Oncology Software (Siemens Healthcare, Forchheim, Germany) and were compared to reference volume and diameter by calculating absolute percentage errors. RESULTS: The software performance allowed a robust volumetric analysis in a phantom setting. Unsatisfying segmentation results were frequently found for native nodules within surrounding muscle. The absolute percentage error (APE) for volumetric analysis varied between 0.01 and 225%. No significant differences were seen between different reconstruction kernels. The most unsatisfactory segmentation results occurred in higher slice thickness (4 and 5 mm). Contrast enhanced lymph nodes showed better segmentation results by trend. CONCLUSION: The semi-automated 3D-volumetric analysis software tool allows a reliable and convenient segmentation of artificial lymph nodes in a phantom setting. Lymph nodes adjacent to tissue of similar density cause segmentation problems. For volumetric analysis of lymph node metastases in clinical routine a slice thickness of ≤3mm and a medium soft reconstruction kernel (e.g. B40f for Siemens scan systems) may be a suitable compromise for semi-automated volumetric analysis.

Preoperative examination intended to detect multifocality, multicentricity and bilaterality-once considered the strongest indication of breast magnetic resonance imaging (MRI)-is currently being strongly questioned in medical literature. This paper aims at evaluating, based on our experience at Clínica Alemana, Santiago, Chile, breast MRI ability to improve preoperative radiological tumour staging by conventional methods, as well as to determine the proportion of patients in which this diagnostic procedure generated changes in the surgical management. We retrospectively reviewed preoperative MRI studies carried out between January 2009 and June 2010. Classification: Group 1: MRI provided no new information. Group 2: by detecting additional lesions, MRI improved radiological staging without changing the type of surgery planned. Group 3: MRI showed new benign lesions and caused unnecessary surgery. Group 4: MRI successfully changed the type of surgery planned based on conventional studies. A total of 419 breast MRI scans were performed during a 18-month period; 39 percent of them were carried out preoperatively. For the analysis, 128 patients were enrolled and distributed in the following categories: Group 1 (66 percent), Group 2 (20 percent), Group 3 (2 percent) and Group 4 (12 percent). In 95.3 percent of the patients, a single surgery with clear margins was performed. This work demonstrated the usefulness of preoperative MRI in our practice, i.e., it allowed for a better radiological staging in one third of the patients and even successfully changed the surgical approach in 12 percent of cases.


El estudio preoperatorio en búsqueda de multifocalidad, multicentricidad y bilateralidad -antes considerada la indicación más sólida de la resonancia magnética (RM) mamaria- hoy se encuentra fuertemente cuestionada en la literatura. En este trabajo nos propusimos evaluar la capacidad de la RM mamaria en nuestro centro para mejorar la etapificación radiológica preoperatoria realizada por métodos convencionales y determinar la proporción de las pacientes en que genera cambio en el enfoque quirúrgico. Hemos revisado retrospectivamente las RM preoperatorias entre enero de 2009 y junio de 2010. Clasificación: Grupo1: la RM no aportó información nueva. Grupo 2: al detectar lesiones adicionales, mejoró la etapificación radiológica, sin cambiar el tipo de la cirugía planificada. Grupo3: demostró nuevas lesiones no malignas y causó cirugía inútil. Grupo 4: cambió correctamente el tipo de cirugía planeada en base a los estudios convencionales. En los 18 meses se realizaron 419 RM mamarias, el 39 por ciento de ellas en preoperatorio. Para el análisis se han reclutado 128 pacientes con la siguiente distribución en los grupos predeterminados: Grupo 1(66 por ciento), Grupo 2(20 por ciento), Grupo 3(2 por ciento) y Grupo 4(12 por ciento). En el 95,3 por ciento de las pacientes se logró realizar una sola cirugía con márgenes libres. Este trabajo demostró la utilidad de la RM preoperatoria en nuestra práctica: permite una mejor etapificación radiológica en el tercio de las pacientes e incluso cambia correctamente el enfoque quirúrgico en el 12 por ciento de los casos.

Las tecnologías incorporadas en el último tiempo en resonancia magnética, han impactado considerablemente en la resolución temporal y espacial. Los avances más relevantes son; tecnología multicanal, adquisición paralela y bobinas multireceptoras. La incorporación y asimilación de estas nuevas herramientas permite optimizar la calidad y tiempos de exámenes, respondiendo a las exigencias diagnósticas y asistenciales de esta técnica por imágenes. Interiorizarse de la tecnología que se dispone y evaluar en que medida se puede participar de estos avances, permitirá trabajar con un máximo de rendimiento, para cada centro en particular.

El objetivo de este trabajo de investigación fue el de evaluar la relación entre caries y fluorosis dental, en los escolares residentes de 4 a 11 años, en la localidad termal de Chaqui, (Departamento de Potosí- Bolivia), población en la que hasta hace cuatro años aproximadamente su única fuente de consumo de agua, era exclusivamente aguas termales, en la actualidad cuentan con agua natural de otras fuentes no termales para el consumo humano. Para este propósito se utilizó en la observación clínica, los índices de caries dental CPO-D y ceo, el índice de fluorosis dental de Dean, asimismo se indagó sobre los hábitos higiénicos alimenticios. Los resultados demuestran que existe mayor incidencia de caries dental en piezas temporarias ceo (4.4), en relación a las piezas permanentes CPO-D (0.8). Inversamente los índices de fluorosis dental se ubicaron en la dentición permanente (0.9) en relación a la temporaria (0.02), valores de mucha importancia en Salud Pública. Se concluye en relación a un estudio anterior de hace una década, actualmente se presenta mayor incidencia de caries dental en niños menores de siete años y mayor incidencia de fluorosis dental en los escolares de mayor edad.


The research work had the goal to evaluate the current behaviour that is present in the cavities and dental fluorosis in the resident schoolboys from 4 a 11 years old, in the thermal locality of Chaqui, department Potosí-Bolivia town in which approximately four years ago its unique source of water consumption was exclusively thermal sources for their consumption. For this propose the index of dental cavities CPO-O and ceo, Oean's index of dental fluorosis was used, also asked about the hygienic alimentary habits. The results demonstrate that exist more incidence od dental cavities in temporary pieces ceo (4.4) in relation ship with the permanent piece CPO-O (0.8). Inverse in the index of dental fluorosis (Oean) they placed in the permanent dentition (0.9) in relationship to the temporary one (0.02), values of great importance in public health. To conclude in relationship with a before study one decade ago, nowadays there is more incidence of dental cavities in children younger than seven years old and more incidence of dental fluorosis in older schoolboys.

Diffusion tensor imaging can be used to localize major white matter tracts within the human brain. For surgery of tumors near eloquent brain areas such as the pyramidal tract this information is of importance to achieve an optimal resection while avoiding post-operative neurological deficits. However, due to the small bandwidth of echo planar imaging, diffusion tensor images suffer from susceptibility artifacts resulting in positional shifts and distortion. As a consequence, the fiber tracts computed from echo planar imaging data are spatially distorted. We present an approach based on non-linear registration using Bézier functions to efficiently correct distortions due to susceptibility artifacts. The approach makes extensive use of graphics hardware to accelerate the non-linear registration procedure. An improvement presented in this paper is a more robust and efficient optimization strategy based on simultaneous perturbation stochastic approximation (SPSA). Since the accuracy of non-linear registration is crucial for the value of the presented correction method, two techniques were applied in order to prove the quality of the proposed framework. First, the registration accuracy was evaluated by recovering a known transformation with non-linear registration. Second, landmark-based evaluation of the registration method for anatomical and diffusion tensor data was performed. The registration was then applied to patients with lesions adjacent to the pyramidal tract in order to compensate for susceptibility artifacts. The effect of the correction on the pyramidal tract was then quantified by measuring the position of the tract before and after registration. As a result, the distortions observed in phase encoding direction were most prominent at the cortex and the brainstem. The presented approach allows correcting fiber tract distortions which is an important prerequisite when tractography data are integrated into a stereotactic setup for intra-operative guidance.

Background: The family of lectins known as galectins (galectins 1-14) are involved in the regulation of the immune system and in oncogenesis. During a search for antigens recognized by antibodies produced by a patient with systemic lupus erythematosus (SLE) we found reactivity against galectin-8, for which autoantibodies have not been previously described. Aim: To determine the frequency of autoantibodies against galectin-8 in lupus patients compared with healthy controls. Patients and Methods: Galectin-8 was purified from a bacterial expression system and used in immunoblot assays as antigen to screen the sera of 55 SLE patients and matched controls. Disease activity was evaluated using the Mexican Modification of the Systemic Lupus Erythematosus Disease Activity Index (MEX-SLEDAI). Results: Reactivity against galectin-8 was detected in 30% of SLE patients, compared to 7% of controls (p=0.003). We could not detect any particular SLE manifestation associated to the presence of these autoantibodies. Conclusions: This is the first description of autoantibodies against galectin-8. Its higher frequency in patients with SLE suggests a pathogenic role. Further studies are needed to determine their clinical relevance.

Background: There are a few studies examining the etiology of community acquired pneumonia (CAP) in Chile. Aim: To evaluate the etiology of CAP in hospitalized adults. Method: We prospectively studied 130 patients (mean age +/- SD: 68 +/- 18 y.o.; overall hospital mortality: 6.2 percent), over a 16 month period. Microbiological evaluation included blood and sputum cultures for bacteria; serology for C. pneumoniae, C. psittaci and M. pneumoniae; urine antigen for L. pneumophila; and nasopharyngeal swab for respiratory viruses. Results: Etiology was identified from 64 (49 percent) patients (two or more pathogens in 6). The most frequent microorganisms were S. pneumoniae (34 percent), Parainfluenza types 1 to 3 (22 percent), Influenza A or B (14 percent ), C. pneumoniae (6 percent), M. pneumoniae (6 percent), H. influenzae (5 percent) and S. marcescens (5 percent). Twenty-five of 27 (93 percent) respiratory viruses were identified in autumn or winter. Pneumococcal pneumonia patients (19) compared to those infected with respiratory virus (23) were younger (59 +/- 18 versus 72 +/- 17 y.o.; p = 0.021) and had less comorbidities (47 versus 87 percent; p = 0.0001). No patients with bacteremia (13 of 121: 11 percent) died. Conclusions: S. pneumoniae remains the most important pathogen to cover by initial antibiotic therapy; the second most frequent etiological agents were respiratory viruses followed by "atypical pathogens". Recommendations for the management of patients infected with these two last categories of agents should be included in future national guidelines.


Fundamento: Hay escasos estudios que examinen la etiología de la neumonía adquirida en la comunidad (NAC) en población adulta chilena. Objetivo: Identificar la etiología de la NAC en adultos inmunocompetentes hospitalizados. Método: Estudiamos, prospectiva y consecutivamente durante 16 meses, a 130 pacientes (edad promedio +/ - DS: 68 +/ - 18 años; letalidad en el hospital: 6,2 por ciento). La evaluación microbiológica incluyó cultivo de expectoración y hemocultivos para bacterias; Financiamiento: Fondo de Investigación de la Sociedad Chilena de Enfermedades Respiratorias (2002) y fondo de la Dirección de Investigación de la Pontificia Universidad Católica de Chile (DIPUC 2003/10E). Serología para C. pneumoniae, C. psittaci, M. pneumoniae; antígeno urinario para L. pneumophila e hisopado nasofaríngeo para virus respiratorios. Resultados: Se identificó la etiología en 64 (49 por ciento) pacientes (dos o más patógenos en 6). Los principales microorganismos fueron: S. pneumoniae (34 por ciento), virus Parainfluenza 1 a 3 (22 por ciento), virus Influenza A o B (14 por ciento), C. pneumoniae (6 por ciento), M. pneumoniae (6 por ciento), H. influenzae (5 por ciento) y S. marcescens (5 por ciento). El 93 por ciento (25/27) de los virus respiratorios se identificaron en otoño-invierno. Los pacientes con neumonía neumocócica (19) comparados con aquéllos infectados por virus respiratorios (23) eran más jóvenes (59 + /- 18 versus 72 +/ - 17 años; p = 0,021) y tenían menos comorbilidades (47 por ciento versus 87 por ciento; p = 0,0001). Ninguno de los 13 (11 por ciento) pacientes con bacteremia falleció en el hospital. Conclusiones: S. pneumoniae sigue siendo el principal patógeno a cubrir por el tratamiento antibiótico empírico; los virus respiratorios y los agentes atípicos fueron los que siguieron en frecuencia. Las futuras guías clínicas nacionales deberían incluir recomendaciones para el manejo de los pacientes infectados por estos dos últimos grupos de...

Reliable elasticity parameters describing the behavior of a given material are an important issue in the context of physically-based simulation. In this paper we introduce a method for the determination of the mechanical properties of brain tissue. Elasticity parameters Young's modulus E and Poisson's ratio nu are estimated in an iterative framework coupling a finite element simulation with image registration. Within this framework, the outcome of the simulation is parameterized with both elasticity moduli that are automatically varied until optimal image correspondence between the simulated and the intraoperative data is achieved. We calculated optimal mechanical properties of brain tissue in six cases. The statistical analysis of the obtained values showed a good correlation of the results, thus proving the value of the method. An approach combining simulation and registration for the determination of the mechanical brain tissue properties is presented. This contributes to performing reliable physically-based simulation of soft tissue movement.

El plexo braquial es una estructura nerviosa compleja que incluye desde las raíces cervicales hasta los nervios periféricos de la axila. Su valoración mediante métodos de imagen ha sido difícil, hasta la aparición de la RM. Es importante contar con un equipo de alto campo y bobinas de superficie que permitan obtener imágenes de alta resolución, debiendo siempre realizarse un examen en los tres planos del espacio. Este método ha probado ser útil en el diagnóstico de avulsión, neuromas, tumores, neuritis y fibrosis actínica entre las patologías más frecuentes

A clinicoepidemiological study was undertaken to determine if enterohemorrhagic Escherichia coli (EHEC) was associated with hemolytic-uremic syndrome (HUS) in children in Santiago, Valdivia, and Temuco, Chile. Prospective surveillance detected 20 hospitalized cases of HUS in children less than 4 years of age in these cities from March 1988 to March 1989. Each HUS patient was matched (by sex and age) with two control children (hospitalized elective-surgery patients). To detect EHEC, DNA from stool culture isolates of E. coli was detected by hybridization with biotin-labelled DNA probes specific for the EHEC virulence plasmid, Shiga-like toxin I (SLT-I) or SLT-II. Stool cultures from 6 of 20 cases (30%) and from 2 of 38 controls (5.3%) yielded EHEC (P = 0.0158). EHEC isolates from all HUS cases hybridized with the EHEC plasmid probe and with probes for SLT-I or -II (or both). The serogroups of the isolates included O157, O26, and O111. EHEC causes HUS in Chile, and the biotinylated gene probes are practical diagnostic tools for epidemiologic studies.

We evaluated immunofluorescence techniques to investigate the presence of S pneumoniae, N meningitidis, H influenzae types a and b and L monocytogenes in 85 samples of CSF fluid: 60 were taken from patients suffering meningitis and 25 from a control group. Results were compared to conventional bacteriologic methods. There were no false positive results in the control group. In patients with meningitis, 28 were positive by both methods. Nine additional patients were positive only to immunofluorescence which allowed identification of S pneumoniae in 6, N meningitidis in 2 and H influenzae in 1. 37 samples were positive by immunofluorescence and 28 of them were positive to conventional bacteriology. There was only 1 case of Group B streptococcus identified by bacteriology which was not diagnosed by immunofluorescence. Thus, immunofluorescence increases the ability to make a bacteriologic diagnosis in patients with meningitis.
