[Long-term experience with glaucoma silicone implants in a multicenter study]. / Dlouhodobé zkusenosti s glaukomovým silikonovým implantátem (GSI) v multicentrické studii.

PURPOSE: To evaluate the experience with a glaucoma implant in complicated glaucoma, designed in collaboration with the Institute of Polymers in Prague. METHODS: In a multicentric retrospective study 91 eyes of 86 patients with otherwise uncontrollable glaucoma underwent GSI implantation in four departments. The implant of medical silicone consists of a tube with an internal diameter of 0.4 mm, connected to a round lens-shaped episcleral plate 169 mm2, covering the tube orifice from above. The indications were infantile glaucoma (14 eyes), traumatic glaucoma (14 eyes), various secondary glaucomas in 12, primary open-angle glaucoma in 11, neovascular and after PPV in 10 eyes each, usually after unsuccessful previous surgery. RESULTS: The average follow-up time was 20.5 months (SD +/- 19.5). Postoperative intraocular pressure at the last control was 6-22 mmHg in 56 eyes (50.95%). Life table analysis showed a cumulative probability of 494% (at least qualified success). Long-term hypotony was seen in 6 eyes. CONCLUSIONS: The GSI is comparable with similar tube shunts but is much less expensive. In vitro experiments are planned to improve the construction of the implant and its results.

[The idiopathic uveal effusion syndrome]. / Syndrom idiopatické uveální efuze.

The authors describe two cases of idiopathic uveal effusion syndrome, in one instance affecting consecutively both eyes, in the second patient the finding is unilateral so far. In the female patient with the bilateral finding reattachment of the retina and improvement of the visual acuity developed only after surgery (sclerotomy). The second patient, where the disease was detected only accidentally, is so far only under observation. The authors discuss the causes of this rare disease, the variability of clinical symptoms, diagnostic and therapeutic possibilities.

Using field data analysis for environmental decision making and subsequent remediation at two example sites.

One of the major challenges in remediating contaminated sites is having quick access to quality data on which to base remedial decisions as onsite work progresses. Case studies are presented at two Superfund sites where field screening and field analyses are used to provide these data. Emphasis is placed on the importance of high quality field data, as these data are the basis for remedial decisions prior to receipt of offsite laboratory confirmation. The decision-making processes for remediating contaminated soils and structures are presented in addition to project specifics including data quality objectives, field data collection procedures, quality assurance/quality control procedures, and comparisons of the field data with offsite laboratory results.

[Ambulatory cataract surgery]. / Ambulantní operace katarakty.

The authors present an account of the experience with the first one hundred out patient cataract surgery which were performed at the Second ophthalmological Clinic in 1993 and 1994. They found that the only absolute contraindication is the need to administer general anaesthesia and they discuss the main problems which ensue from the present state of the health services and society as a whole.
