RESUMO

Results of treatment of 102 patients for a variety of injuries and surgical diseases of the colon, who performed the intervention, culminating in the formation of the external colonic fistula (ECF) were analyzed. After reconstructive and restorative surgery for ECF, all patients were alive. Postoperative complications occurred in 15 (20.8%) patients, including intraabdominale--in 2 (2.8%). The optimal timing of surgical rehabilitation and volume reduction or reconstructive surgery in patients over the ECF should be determined strictly individual basis, taking into account the severity and nature of the illness or injury, which required imposition of an stoma, the severity of postoperative scar and local inflammatory processes in the abdominal cavity, in laparotomic wound and fistula. Performing simultaneous operations in the surgical rehabilitation of patients with ECF practically does not increase the risk of postoperative complications.

RESUMO

Clinical course and results of surgical treatment of 94 patients with complex forms of recurrent acute paraproctitis were analysed. Radical surgical intervention was conducted in 68 patients, of whom in 35--using the pararectal abscess opening with cryptectomy, in 17--with translocation of a distal rectum mucosa, in 16--with a ligature conduction through internal aperture of fistula. One-nine year follow-up of 89 patients is available. After radical intervention the recurrence and postoperative complications had occurred in 19%, after nonradical one--in all the patients.

After radical operation excellent and good result was noted in 78.7% of patients, poor--in 15.3%; after nonradical one--in 7.8 and 92.2% accordingly. After the internal fistula aperture closure using translocated flap of the distal rectum mucosa in 95.9% of patients an excellent and good result was achieved, poor--in 2%; after opening of abscess with cryptectomy--in 80.6 and in 17.9% accordingly; after the ligature method application--in 55.9 and in 29.5% accordingly.
