The authors present the case of a 15-year-old boy who was infected with COVID-19 by his family and developed pneumonia severe enough to require hospitalization. He was one of several children who required hospitalization, and vaccination was considered by the parent to be "risky" despite this. This case illustrates the complexities of vaccine hesitancy and societal misinformation.

RESUMO

A 14-year-old boy presented to the emergency department with pain and inability to move the right knee after a fall while playing basketball. X-ray findings identified a Salter-Harris type II fracture of the tibial metaphysis with anterior subluxation of the proximal tibia. The case was then transferred to a pediatric orthopedic subspecialist for operative treatment. This case highlights an unusual fracture location about the proximal tibial physis without damage to the epiphysis.

The authors present the case of a patient seen for back pain in the emergency department, initially discharged home with a diagnosis of musculoskeletal pain. He returned a week later and was found to have vertebral osteomyelitis based on a thorough neurologic exam. A review of ominous causes of back pain, including the elusive nature of vertebral osteomyelitis/diskitis is discussed.

RESUMO

Spontaneous pneumothorax (SP) is an abnormal occurrence in which air enters the pleural space, increasing pressure that pushes on the lung, causing it to collapse in part or full. Diagnosis is made by physical exam and can be confirmed by chest X-ray (CXR), chest computed tomography (CT), ultrasonography, and other forms of imaging showing a collapsed lung. We present the case of a 35-year-old male cannabis user presenting with sharp sudden pains in the right chest, who presented almost 12 hours after symptom onset and was diagnosed with a 90% pneumothorax of the right lung. Symptoms, diagnosis, treatment, and risk factors are discussed.

RESUMO

Pseudotumor cerebri, or idiopathic intracranial hypertension (IIH), is a syndrome of elevated intracranial pressure (ICP) of unknown etiology that occurs predominantly in obese women of childbearing age. Pseudotumor cerebri literally means "false brain tumor". It is a "diagnosis of exclusion" therefore a complete work-up to rule out life-threatening causes for increased ICP must be performed through a comprehensive history, complete physical examination, diagnostic imaging, and cerebrospinal fluid (CSF) analysis before the diagnosis can be made. The authors present the case of a young woman with headache, and near blindness due to pseudotumor cerebri. The presentation, diagnosis, and treatment options are discussed.

RESUMO

We present a rare case of corneal abrasion with mild eyelid epitheliopathy caused by a high-voltage electrical spark. The case includes emergency department evaluation and subsequent management at the burn center with ophthalmology. The prognosis, in this case, is good, however, the potential severity of high-voltage electrical injuries can be much worse. Prevention strategies for occupational electrical injuries are discussed with an emphasis on proper personal protective equipment (PPE).

RESUMO

AHORA (Andar, Hablar, Ojos, Rostro, Ambos Brazos o Piernas) is a Spanish language tool to identify stroke symptoms. A survey of 300 primarily Spanish-speaking, non-medical professionals was conducted to assess the acceptance of the tool, specifically about ease of understanding and ability to implement it. The overwhelming majority of respondents reacted very positively to the tool, finding it quite easy to learn, teach, and understand. Respondent feedback, pitfalls, and questions for further research are presented.

BACKGROUND: Kratom is a habit-forming opioid-like substance with an acute toxidrome of various symptoms such as diaphoresis, dizziness, nausea, and vomiting. Chronic users require increasing dosages for the analgesic effects. Although kratom use dates back to the 1800s in Asia, kratom intoxication is still a novel (but increasing) toxidrome in the Western world. Here, we present a novel case of acute toxicity from overdose in a kratom-naïve patient, taking place through recommendation by a family member who chronically takes this substance. CASE PRESENTATION: We present the case of a 62-year-old woman arriving to the emergency department (ED) with a chief complaint of intractable vomiting after ingestion of kratom. After a day of yard work, she was in pain, secondary to her osteoporotic joints. She was recommended kratom from a family member, who stated he was using kratom to transition away from opioid dependence. She took two "scoops." She proceeded to have multiple episodes of vomiting at home. She came to the ED, where she required multiple rounds of anti-emetic medication for resolution of her symptoms. DISCUSSION: We present a classic case of a novel acute toxicity: kratom. A unique aspect of this case is the circumstance by which this toxicity took place: a family member who chronically takes this substance (that requires increasing dosages to remain effective) recommended a dosage to this kratom-naïve patient, leading to overdose. This opioid family alternative substance is gaining popularity across the USA in the era of the opioid crisis. Further documentation of case reports and research is required to learn the associated risks of the use of this substance.

Hydrofluoric acid burns are uncommon but unique among chemical burns in that they can cause visually mild burns with significant deep tissue injury and systemic toxicity through multiple mechanisms. We present the case of a patient who presented with bilateral hydrofluoric acid burns to his hands from aluminum brightener. The patient had been using an aluminum brightener with a hydrofluoric acid concentration of 10% for several months at work. On emergency department presentation, the patient endured significant tenderness to his hands and fingers. The patient suffered no serious complications, had no concerning lab or electrocardiographic findings, and was treated symptomatically with calcium gluconate gel. He was discharged home after successful symptom resolution with proper return precautions and instructions on how to safely use hydrofluoric acid containing products. Although not a very common cause of burns, acute care of these burns requires specific knowledge which is imperative for emergency personnel.

In this paper, we review the indications, complications, and pitfalls associated with ventriculoperitoneal (VP) shunts. As most VP shunt problems initially present to the emergency department, it is important for emergency physicians to be well-versed in managing them. In the article, the possible reasons for shunt failure are explored and summarized using an infographic. We also examine potential clinical presentations of VP shunt failure.

RESUMO

With the widespread use of serotonergic agents including many antidepressants, antiemetics, illicit drugs, and even some herbal supplements, serotonin syndrome is a condition seen more frequently. It can appear abruptly and, if untreated, can progress to a life-threatening state. Prompt recognition and treatment is imperative to avoid complications. The presentation is variable and can be confused with other conditions. The authors present a case of serotonin syndrome that was recognized early and treated promptly in the emergency department.
