Owing to the high concentrations of crude protein and beneficial fatty acids, hempseed cake could be a high-quality feedstuff for ruminants. The aim of the present study was to evaluate the effects of hempseed cake on milk production and quality, as well as haemato-chemical parameters in Alpine goats. In total, 28 French Alpine dairy goats were evaluated over 45 days. All goats were fed meadow hay supplemented with 1.2 kg/day feed mixture. The feed mixture in control group contained soybean meal and extruded soybean. In the first experimental group, soybean meal and extruded soybean were partially replaced with 60 g/kg of hempseed cake (HC6). In the second experimental group, soybean meal and extruded soybean were partially replaced with 120 g/kg of hempseed cake (HC12). Feeding goats with HC6 and HC12 did not influence milk yield, although the concentration of milk urea was higher in the HC12 compared to control group. The HC12 decreased the linoleic acid (LA)/linolenic acid (ALA) ratio and glutathione peroxidase activity in the serum. Under the conditions evaluated in this study, the inclusion of HC6 in the diet was optimal with respect to chemical composition of milk and haemato-chemical parameters. Even though HC12 resulted in lowered LA/ALA ratio, which confers functional properties to goat milk, HC12 increased milk urea concentrations, and worsened serum antioxidant status.

INTRODUCTION: Orchiopexy for a palpable undescended testis can be approached through a traditional inguinal incision or trans-scrotally. Despite the possible advantages of the scrotal approach, including reduced postoperative pain and shorter recovery, it is not consistently advocated. OBJECTIVE: The objective of this study was to present the experience with a tailored approach to orchiopexy based on physical findings. STUDY DESIGN: This is an extended case series. MATERIALS AND METHODS: The mobility of the testis as described at examination under anesthesia informs the choice of surgical approach. If a 'low' palpable testis (defined as testis that can be manipulated to the scrotum) was found, a scrotal approach was used. In cases of 'high' palpable testis (testis that cannot be manipulated to scrotum), the inguinal approach was used. Success was defined by location and size of the testis 3 months after surgery. RESULTS: A total of 259 orchiopexies were performed in 181 boys (78 bilateral). Scrotal approach was used in 125 (48%) and inguinal in 134 (52%) orchiopexies. Operative time was significantly shorter for the scrotal approach, 25 min vs. 40 min for inguinal orchiopexy (P < 0.05). The overall success rate was 98% with no statistical difference between the groups. Three children from the inguinal group and two from the scrotal group required an additional procedure for persistent undescended testis. The rates of testicular atrophy and hypotrophic testis were higher in the inguinal group than the scrotal group (5/134 vs. 0/125; P < 0.05 and 17/134 vs. 6/126; P < 0.05, respectively). DISCUSSION: The substantial cohort of patients selected for trans-scrotal orchiopexy experienced success rates and rates of atrophic and hypotrophic testis comparable with those found in the published literature. Furthermore, trans-scrotal operative times were significantly lower than those of inguinal procedures, and less patients required re-operation in the trans-scrotal group. Limitations of this study include significantly higher age at operation in trans-scrotal patients and a difficulty accurately classifying hypotrophic testes. Furthermore, the higher atrophic rate in the inguinal group vs. the scrotal group likely reflects the vulnerability of a testis that is located higher and not the superiority of the scrotal approach. CONCLUSION: This tailored approach to a palpable undescended testis appears simple, safe, and effective, providing high success rate with marginal complications. It is considered a preference in cases of low undescended testis, whereas the standard two-incision inguinal orchiopexy may better serve those with high undescended testis.

PURPOSE: Spleen mesh wrapping is a valuable and efficacious technique for treating severely traumatized spleens. We report the morbidity of the procedure and describe its management in children. METHODS: A retrospective survey of children admitted to the Hillel Yaffe Medical Center with a traumatized spleen was carried out. RESULTS: Between 1985 and 2000, we treated 168 children for blunt spleen trauma. The spleen was the only intra-abdominal organ injured in 102 of them. Absorbable mesh wrapping was chosen for 20 children (age range 1 - 15 years), with grade III/IV lesions that would otherwise probably have required splenectomy. Hemostasis was achieved in all 20 children. All children survived with intact spleens as suggested by the absence of long-term infectious complications and by normal hematological, US and scintigraphy studies. Anti-pneumococcal vaccination was not required. Complications were diagnosed in 12 cases, the most common being prolonged fever. Perisplenic fluid, oozing, and left pleural effusion were also present. Only one intrasplenic pseudoaneurysm occurred and this resolved spontaneously. There was one splenic abscess which was successfully treated by transmesh transcutaneous computerized tomography-guided drainage. No complications were detected at the 1 year follow-up. CONCLUSIONS: Absorbable mesh wrapping is safe and effective for treating grade III/IV spleen ruptures in children. Complications are mostly minor, and resolve spontaneously. Major complications are rare and can usually be managed nonsurgically.

Perforation of the appendix is rare in term neonates and even more rare in prematures. In most of the reported cases it is a complication of necrotizing enterocolitis (NEC) but can also complicate Hirschsprung's disease (HD), cystic fibrosis (CF) or acute appendicitis (AA). We present a pre-term newborn, born to a mother who had bacteriaemia associated with chorioamnionitis. On his sixth day of life he had clinical and radiological signs of perforated abdominal viscus. Laparotomy and histologic findings indicated acute perforated appendicitis. The association of acute perforated appendicitis with chorioamnionitis points to the role of maternal foetal infection in the aetiology of neonatal acute appendicitis.

PURPOSE: The aim of this work was to detect nuclear parameters related to the prognosis of patients with stage III, IV or DS neuroblastomas. METHODS: Histological sections of 25 operation specimens obtained from children with advanced-stage neuroblastomas were subjected to computer-assisted image analysis. Statistical relationships between nuclear descriptors of the tumor cells and patients' clinical outcome were determined. RESULTS: The coefficient of variability of the mean nuclear area the mean nuclear elongation factor, and the mean nuclear averaged Feret diameter of the neuroblastoma cells were ascertained to be discriminators separating high-grade from low-grade tumors. CONCLUSIONS: The histomorphometrically gauged nuclear parameters may help oncologists to assess the prognosis of patients with advanced-stage neuroblastoma.

Acrylate glues used in a childrens' day care unit to close small surgical wounds were compared. In 157 children, aged 12 weeks to 3.7 years, either Histoacryl or Dermabond was used (respectively, H: Ethicon Inc., Johnson & Johnson, NJ or D: Dermabond, Braun Surgical Gmbh, Melsungen, Germany). Operations were for inguinal hernia (110 cases), hydrocele (25), undescended testis (16), umbilical hernia (13) and funiculocele (3). 1 week after surgery the wounds were evaluated in terms of integrity of closure, redness or infection, need for antibiotics, wound granuloma, and parental satisfaction with instructions and actual method of wound caring. 3 months after surgery the wound/scar was reexamined. The margins of the wounds were separated partially or completely in 8 of 85 in group H (9.4%) while in the D group, 2 wounds (2.4%) had partially opened (p < 0.05). There were no differences between the glues with regard to wound infection or cosmetic results. Parental satisfaction was higher with D (96%) than H (82%) but the difference was not statistically significant. It is convenient to use glue to close operative wounds in children after ambulatory surgery. The use of D significantly reduced wound ruptures compared to H. Long-term cosmetic results were similar.

RESUMO

A rare case of a gastrointestinal cystic duplication in a 7-day-old infant is described. The duplication diagnosed antenataly at 25 weeks of gestation was found during surgery to be separated from the gastrointestinal tract, hanging on a vascular pedicle, with no connection to the mesentery. The duplication was excised, and postoperative follow-up of 14 months was uneventful. The possible pathogenesis of this malformation is discussed.

The authors studied the effect of ligation of the femoral and saphenous veins on the evolution of lipase injection-induced subcutaneous fibrosis and inflammation in the rats' hindlimbs. The superficial muscular fascia was thickened and the number of veins was increased 3 days and 3 weeks after vein ligation; both abnormalities disappeared 6 weeks postoperatively. Vein ligation did not quantitatively affect fascial thickening or fibrosing panniculitis in hindlimbs injected with lipase 6 weeks prior to sacrifice. The results contradict the proposition that lipase-induced injury of rats' subcutaneous tissues compromised by venous stasis may lead to a persistent chronic inflammation and fibrosis of the subcutaneous-fascial complex duplicating lipodermatosclerosis in man.

RESUMO

Lymphangioma is a benign developmental anomaly of the lymphatic system located in about 40% of cases in the neck and usually completely resectable. In some cases it invades adjacent structures such as the larynx, pharynx, or tongue. In such cases it is almost impossible to resect completely, as this would endanger vital structures. We describe a baby born with a huge cervical lymphangioma which invaded the tongue, larynx, pharynx and other cervical structures. He was treated with bleomycin and aethoxysklerol. The sclerotherapy shrank the lymphangioma considerably and it became resectable. At 2 years of age the cosmetic result is good and vital function, such as swallowing and facial expression, are preserved.

In 1994-1995, central venous lines were placed in 47 children. All except 1 were of the Broviac type, with subcutaneous tunneling via the internal or external jugular vein. Ages were between 7 days and 16 years. Indications for central venous cannulation were chemotherapy (35 cases), TPN (5), prolonged parenteral antibiotics (4), and repeated blood transfusions (3). The catheter was the source of infection in 13 children (28%), 11 of whom were immunocompromised. The commonly identified bacteria were Staphylococcus aureus (4 cases), Pseudomonas aeruginosa (4), coagulase-negative Staphylococcus (2), and various gram-negative rods (3). All cases were treated with antibiotics through the catheter. The most commonly used were oxacillin (4), ceftazidime (4), and amikacin (4). In 10, treatment succeeded without having to remove the line. In 2 others, tunnel infection developed and the catheter had to be removed. 1 child forcefully removed his catheter before treatment could be started. There were no further complications in the group treated conservatively, except for a case of superior vena cava thrombosis in a girl with recurrent infection of the tunnel. In 7 out of 13 treatment was continued and completed at home. This saved 65 days of hospitalization out of 210. We conclude that the conservative approach to treatment is feasible in most cases of infection when the source is the central venous catheter itself. However, when the tunnel is infected, conservative treatment may be ineffective. Treatment can be carried out in the home, with economy in cost and in use of hospital beds, and is preferred by patients and their parents.

Patients with congenital diaphragmatic hernias (CDH) usually present soon after birth with respiratory distress. Occasionally presentation is delayed. Twelve patients with delayed presentation (outside the neonatal period) of a total of 91 with CHD were seen over 17 years (1977-1994). Case records were reviewed to identify reasons for late presentation, evidence of morbidity, treatment and outcome. Age at presentation ranged from 6 weeks to 30 months (mean 8 months). Ten patients were aged 6 months or younger and there was an equal number of boys and girls. Seven cases were left-sided, 4 right-sided and 1 was anteriorly placed and bilateral. Ten patients presented acutely, 5 with small-bowel obstruction and 5 with respiratory distress, but all of the latter had a history of previous recurrent chest infection. Diagnosis was confirmed by radiographic examination of the chest in 10, barium meal in 1 and a 5-month-old patient had an inappropriate laparotomy and gastrojejunostomy for duodenal obstruction at an outlying hospital. Two had documented normal chest radiographs 3 and 6 months prior to diagnosis. Patients who presented with gastro-intestinal symptoms were older (mean age 1 year) compared with those with predominantly respiratory symptoms (mean age 19 weeks). None had any evidence of bowel strangulation. All were repaired via an abdominal approach. A hernial sac was found in half the cases (4/7 left and 2/5 right). Five had non-rotation of bowel. Primary repair without patch was carried out in all. One child with lung hypoplasia and multiple cardiac abnormalities died; the others recovered well. Delay in presentation of CDH is not uncommon (14% in this series). Most patients will have respiratory symptoms. A normal previous chest radiograph does not exclude the diagnosis. All patients with recurrent chest infection should have a contrast meal investigation early on. Long-term prognosis is favourable and postoperative morbidity is minimal, despite late presentation.

Fifteen cases of meconium ileus (MI) were treated between 1986 and 1995; 7 responded to conservative treatment. Eight were operated upon, and comprise the study group. Six of the operated babies had no complications; 1 had meconium peritonitis with a pseudocyst and small-bowel atresia, and 1 had a volvulus of a small-bowel segment with necrosis. In all 8 cases a T-tube (TT) was left via an enterotomy; in the complicated cases the enterotomy was pre-anastomotic. The obstruction was relieved in all the babies, without any stoma or bowel resection in the uncomplicated cases. Two complications occurred: 1 patient died of respiratory failure 1 month following surgery and another required a relaparotomy for lysis of adhesions. We conclude that TT ileostomy is an effective and safe procedure for uncomplicated cases of MI that do not respond to conservative therapy, as well as for complicated cases that need an anastomosis.

RESUMO

Starch peritonitis (SP), is a relatively rare surgical complication. This together with a puzzling acute clinical pattern, makes diagnosis more difficult. We describe a child who developed a full blown SP, mimicking gastric outlet obstruction, 3 weeks after an antireflux procedure. We would like to stress the importance of making the correct preoperative diagnosis, in order to spare the patient an unnecessary operation. This may be difficult.

OBJECTIVE: To evaluate the diagnostic value and potential risk of fine needle aspiration biopsy (FNAB) of the spleen in patients with diffuse splenomegaly due to an undetermined cause. STUDY DESIGN: Retrospective evaluation of the medical records and cytologic material from 58 patients on whom FNAB was performed between 1967 and 1993. RESULTS: Sixty-five FNABs were performed on 58 patients. Lymphoproliferative diseases were found in six; in four additional cases, metastatic adenocarcinoma, sarcoidosis, Candida albicans and enterococcal infection were demonstrated. The remaining 55 biopsies either showed normal splenic tissue or were nondiagnostic. No complications were recorded. CONCLUSION: Splenic FNAB may be useful in some patients with diffuse splenomegaly. The diagnostic yield is rather low. The procedure, however, is simple, safe and well tolerated.

Primary pancreatic hydatid disease is rare. The diagnosis may be difficult when the presentation is that of an unexplained epigastric mass, despite suggestive radiological and ultrasonic features. We describe a 12-year-old girl in whom the definitive diagnosis was only made at repeat operation, when appropriate surgical therapy was effected.

Combined penetrating trauma of the rectum and urinary bladder is rare, and constitutes a diagnostic and therapeutic challenge. Most of the literature discusses trauma to either urinary bladder or rectum alone. The combination of penetrating trauma to both rectum and the urinary system is associated with high morbidity and mortality. There is controversy regarding the preferred approach to this type of combined trauma.

RESUMO

A 10-year-old girl had hemobilia and a biliocutaneous fistula after blunt abdominal trauma. Embolization of the right hepatic artery occluded the hepatic artery aneurysm, but the cutaneous biliary fistula persisted despite prolonged (8 months) conservative management including sphincter decompression with an endoscopically placed biliary stent. Roux-en-Y fistuloenterostomy eventually cured the bile leak.
