The patient who complains of hyperhidrosis presents the physician with a diagnostic and therapeutic challenge. Patients who present with generalized hyperhidrosis are, in general, adults whose sweating occurs both during the waking and sleeping hours. Such patients require a search for a cause that may sometimes be as simple as a drug that they are taking for some medical disorder. Occasionally a systemic illness may account for the onset of hyperhidrosis and a thorough exam and appropriate testing may be necessary to identify the cause. Most patients with primary or essential hyperhidrosis present in childhood or adolescence and have a problem localized to their hands and/or feet. They have a physiologic disorder not a psychiatric or endocrinologic disease. A number of systemic, topical, surgical, and electrical remedies are available for the treatment of hyperhidrosis. Patients with hyperhidrosis of the palm or soles deserve a trial of conservative therapy, iontophoresis in particular, before aggressive surgical techniques that carry with them the risk of lifelong troublesome side effects are offered.

Eighteen subjects were treated for palmar hyperhidrosis by tap water iontophoresis. Only one hand was treated; the other served as a control. Fifteen of the 18 subjects became euhidrotic. Iontophoresis is a suitable alternative to long-term drug therapy and should be offered to patients for the control of palmar hyperhidrosis prior to surgical intervention.

Acne is a metabolic disorder of the pilosebaceous appendage in which microorganisms, hormones, inflammatory cells and fatty acids seem to contribute to the pathologic process. Through education and judicious use of therapeutic agents including benzoyl peroxide, vitamin A acid, topical and systemic antibiotics and 13 cis retinoic acid, most patients with acne can enjoy good control of their disease.
RESUMO
In a patient with a long history of classical psoriasis, a widespread pustular eruption suddenly developed. The precipitating factor appeared to be a high-dose, progesterone-containing hormone preparation. The clinical appearance and manifestations in this patient were typical of pustular psoriasis.

RESUMO
Two patients had pyoderma gangrenosum and rheumatoid arthritis. Biopsy specimens of the cutaneous ulcers in the patients showed a necrotizing vasculitis. Complement (C3) and immunoglobulins were not detected in the skin lesions. There was no important impairment of cell-mediated immunity observed.
