RESUMO
The main tendencies in the development of Pathophysiology as a science and teaching discipline are analyzed in the light of last 6th ISP-2010 Congress. The witnesses for the growing integration between Pathophysiology and other basic medical sciences are given, along with the preserved specifics of the pathophysiological knowledge on the etiology, pathogenesis and models of the diseases and pathological processes. The current role and place of Pathophysiology as Systemic Pathobiology within the structure of medical sciences are discussed.

A review of world literature, including authors' original papers, establishes the clinical pathophysiological hypothesis, initially coined in by authors in 2002, about regular transition of adolescent hypothalamic syndrome (obesity with rose striae) with age into early metabolic syndrome, complicated by autoimmune thyroiditis. Some evidences are obtained, that witness for marfanoid phenotype and chronic disequilibrium between local, autacoid-mediated and systemic, hormone-mediated regulation, typical for inherited connective tissue disorders, may promote this transition (6 fig., 3 tables, bibl.: 92 ref.).

Lipid metabolism and lipid peroxidation (LPO) were studied in residents of St. Petersburg (healthy subjects without atherosclerosis history, healthy relatives of atherosclerotic patients, postmyocardial infarction patients, post-apoplectic patients and coronary heart disease sufferers) versus matched subjects living in rural area. Altogether 215 patients were examined. Besides genotype factors, lipid metabolism and LPO were found responsive to environmental factors. These were especially potent in changing the activity of superoxide dismutase, glutathione peroxidase, catalase. In those living in the country myeloperoxidase, superoxide dismutase and catalase activity was higher than in city population. The latter exhibited, though, higher activity of glutathione peroxidase. It is evident that more advantageous ecological conditions have distinct antiatherogenic action on lipid metabolism and LPO, especially, suggesting possible treatment of atherosclerosis by moving to more healthy locality as regards environmental pollution.

Lipid metabolism and the blood lipid peroxidation system were examined in 56 military males living in rather similar conditions. The parameters in question were compared in the following groups: (1) control subjects, including healthy individuals without a family history of atherosclerotic vascular abnormalities; (2) healthy subjects with a family history of atherosclerosis; (3) patients with coronary heart diseases. There were significant differences only in single cases between the groups. The application of a system of grids setting upright the distribution curves for the parameters under study proved to be effective in finding significant differences between the groups, showing the value of the hereditary factors in the development of atherogenic lipid changes.

Great toe nail matrix capillaries were examined in 102 diabetes mellitus patients using the TM-I capillaroscope manufactured in the USSR. Changes of the capillaries and blood flow were revealed in 85.3% of the patients, of them 65.7% had aneurysmatic distention of the capillary walls. The expression and frequency of capillary pathology grew with age, period of disease and concomitant coronary heart disease, and arterial hypertension.

The method of thermovision was used for examination of lower extremities of 350 patients with diabetes mellitus. In 100 patients with diabetes mellitus the potencies of thermovision were assessed before and after cooling the lower extremities and in 38 patients with diabetes mellitus thermovision was used in the control of treatment of disturbances of the peripheral blood circulation. The data obtained have shown great diagnostic significance of thermovision.
