RESUMO

The meiofaunal diversity is used as an indicator to assess the complex and diverse impacts on the coastal environment during the natural calamities. The present study evaluates the effects of Mandous cyclone on ecologically two different stations, Pattinampakkam beach and Adyar estuary, which are located on Chennai coast, Tamil Nadu, India. The impact of the Mandous cyclone on physico-chemical parameters and meiobenthic faunal composition was investigated during, prior to, and after the cyclone. Thirty-nine species of meiofauna belonging to 15 taxa were recorded in both the stations. Nematoda, Oligochaeta and Harpacticoida taxa occurred with higher diversity and abundance than other meiofaunal taxa. Among these taxa, Polygastrophora sp. of Nematoda, Grania pusilla of Oligochaeta, and Arenosetella indica of Harpacticoida were the predominant species recorded during the study period. There was a prominent decline in the population density of meiofauna due to the Mandous cyclone, and thereafter, it took three weeks for recolonization and restoration to normalcy. Statistically, significant impact of the Mandous cyclone on the diversity, density, and evenness of the meiofaunal community with abiotic factors were observed through the Ecological indices and Canonical Correspondence Analysis. The Mandous cyclone assessment with special emphasis on meiofaunal communities allowed to fill the gap with knowledge regarding the diversity, abundance, composition, and distribution of meiofauna between pre- and post-Mandous cyclone, which helped in understanding the physico-chemical changes and response of meiofauna in a sandy beach and estuary.

Carrageenan is an emerging biopolymer for wound healing and regenerative applications. In this study, reduced graphene oxide (rGO) and hydroxyapatite (HAp) nano-composites infused carrageenan bioactive membrane was fabricated. Here, hydroxyapatite was synthesized from cuttlefish bone (CF-HAp) and its properties were compared with that of chemically synthesized HAp. Crystalline Ca5(PO4)3(OH) and Ca3(PO4)2) phases were obtained in cuttlefish bone derived HAp. Reduced graphene oxide was synthesized and composites were prepared with chemical HAp and CF-HAp. FT-IR spectral analysis showed the imprints of hydroxyapatite on the membrane and also nano-structured particles were evident through morphological estimations that confirm the distribution of nano-particles on the carrageenan membrane. Nano-particulates infused carrageenan membrane showed the maximum tensile strength, in which graphene incorporated carrageenan bioactive membrane showed highest stability of 15.26 MPa. The contact angle of chemical HAp infused carrageenan membrane (CAR-HAp) showed more hydrophilic in nature (48.63° ± 7.47°) compared to control (61.77° ± 1.28°). Bio-compatibility features enunciate the optimal compatibility of fabricated bioactive membrane with fibroblast cell line; simultaneously, CAR-rGO-CF-HAp showed tremendous wound healing behavior with zebrafish model. Hence, fabricated bioactive membrane with the infusion of rGO- hydroxyapatite derived from cuttlefish bone was found to be a versatile biopolymer membrane for wound healing application.

Enarodustat is an orally available hypoxia-inducible factor-prolyl hydroxylase inhibitor which can correct the erythropoietic capacity and improve anemia in chronic kidney disease. Sevelamer carbonate, a non-calcium-based polymeric resin, is one of the commonly prescribed agents for the management of hyperphosphatemia in patients undergoing hemodialysis. This was an open-label, crossover study in healthy male subjects (N = 12) that evaluated the effect of sevelamer carbonate (2400 mg) on the bioavailability of enarodustat (25 mg) when the 2 drugs were administered together (Treatment B) or when enarodustat was administered 3 hours after (Treatment C) or 1 hour before (Treatment D) sevelamer carbonate compared to enarodustat alone (Treatment A). With coadministration of the 2 drugs (Treatment B), enarodustat Cmax and AUCinf reductions were 53% and 45%, respectively. For Treatment C, Cmax and AUCinf reductions were 11% and 6%, respectively, and for Treatment D the corresponding values were 8% and 20%. Thus, coadministration of enarodustat and sevelamer carbonate resulted in a substantial reduction (≈50%) in the oral bioavailability of enarodustat. However, the interaction was substantially mitigated by staggering the administration of enarodustat and sevelamer carbonate. Administration of 4 single oral doses of enarodustat 25 mg, with or without sevelamer carbonate, were safe and well tolerated in this study.

In this study, Haematococcus pluvialis and Coelastrella saipanensis were evaluated for heterotrophic nutrition potential in dairy waste medium by blocking the PSII using DCMU. The study was done by four sets of experiments. In the first set, in the different concentrations DCMU-treatments, 20µL showed pronounced effect in H. pluvialis and C. saipanensis as 89 % and 83% decrease in cells (>30 and > 250 cells/mL) compared to control (536 ± 12.35 × 104 and 1167 ± 15.35 × 104 cells/mL, respectively). Damage to the PS II by DCMU interrupted the growth, which in turn produced a significant drop in the number of cells. In the second round of experiment, growth of algae in various dairy waste concentrations suggest that dairy wastewater (DWW) provides enough nutrients to produce 35.71 % and 64.74 % more cells in H. pluvialis and C. saipanensis, respectively compared to the control. In the third set, high DCMU concentration was added to microalgae cultures in DWW to assess the heterotrophic nutrition potential. Growth in cell number 34.4 ± 19 and 617.46 ± 60.44 cells/mL was recorded in H. pluvialis and C. saipanensis when grown control medium whereas addition of DCMU reduced the cell number to 1.53 ± 0.75 and 55.13 ± 0.75 cells/mL on 15th day, respectively. This shows cells in cultures treated with DCMU reveal that algae can sustain their metabolic activity by utilizing the nutrients of dairy waste inhibiting photosystem. Fourth round of experiments found that microalgae could resume their growth and productivity by adapting to heterotrophic nutritional behaviour when DCMU given in mild dose at different time interval. This study conclude as C. saipanensis grows more readily by absorbing dairy waste nutrients than H. pluvialis. Therefore, C. saipanensis is an excellent choice for wastewater treatment through sustainable environmentally benign process after scale-up investigation. These results provide useful information to advance to molecular study for measuring microalgae's capability for bioremediation application.

The drug interaction potential of enarodustat (doses: 25, 50 mg) on the activity of cytochrome P450 (CYP) 1A2, 2C9, 2C19, 2D6, and 3A4 was evaluated after once-daily administration for 15 days in a phase 1 multiple-ascending-dose study in healthy subjects. Probe substrates specific for the enzymes, i.e., caffeine (CYP1A2), tolbutamide (CYP2C9), omeprazole (CYP2C19), dextromethorphan (CYP2D6), and midazolam (CYP3A4), were administered orally as a cocktail with (day 15) and without (day -3) enarodustat. Drug interaction was based on geometric mean maximum plasma concentration (Cmax ) and area under the plasma concentration-time curve from the time of dosing to infinity (AUCinf ) ratios (day 15/day -3) for CYP1A2, 2C9, 2C19, 2D6, 3A4, and urinary excretion of dextromethorphan metabolite dextrorphan for CYP2D6. At the 2 enarodustat doses, for caffeine, the geometric mean ratios (range) for Cmax and AUCinf were 0.99-1.06 and 1.61-1.63, respectively. The ratios for peak concentrations and total exposures were 0.98-1.07 and 0.71-1.78 for tolbutamide and omeprazole, respectively. For dextrorphan the Cmax and AUCinf ratios were 0.83-0.90 and 1.02-1.04, respectively. The mean dextrorphan cumulative amount excreted into the urine from the time of dosing to 24 hours values on day -3 and day 15 were 8.25 mg and 8.20 mg at the lower dose, and 9.40 mg and 9.51 mg at the higher dose. The ratios for midazolam Cmax and AUCinf were 1.42-1.63. Overall, there was a lack of enarodustat dose dependency regarding the geometric mean ratios and 90% confidence intervals and urinary excretion of dextrorphan. There were some cases where the 90% confidence intervals at the 2 enarodustat doses were outside the 0.80-1.25 range, but changes in the geometric mean ratios were all <2-fold.

Recently, the infectious disease COVID-19 remains to have a catastrophic effect on the lives of human beings all over the world. To combat this deadliest disease, it is essential to screen the affected people quickly and least inexpensively. Radiological examination is considered the most feasible step toward attaining this objective; however, chest X-ray (CXR) and computed tomography (CT) are the most easily accessible and inexpensive options. This paper proposes a novel ensemble deep learning-based solution to predict the COVID-19-positive patients using CXR and CT images. The main aim of the proposed model is to provide an effective COVID-19 prediction model with a robust diagnosis and increase the prediction performance. Initially, pre-processing, like image resizing and noise removal, is employed using image scaling and median filtering techniques to enhance the input data for further processing. Various data augmentation styles, such as flipping and rotation, are applied to capable the model to learn the variations during training and attain better results on a small dataset. Finally, a new ensemble deep honey architecture (EDHA) model is introduced to effectively classify the COVID-19-positive and -negative cases. EDHA combines three pre-trained architectures like ShuffleNet, SqueezeNet, and DenseNet-201, to detect the class value. Moreover, a new optimization algorithm, the honey badger algorithm (HBA), is adapted in EDHA to determine the best values for the hyper-parameters of the proposed model. The proposed EDHA is implemented in the Python platform and evaluates the performance in terms of accuracy, sensitivity, specificity, precision, f1-score, AUC, and MCC. The proposed model has utilized the publicly available CXR and CT datasets to test the solution's efficiency. As a result, the simulated outcomes showed that the proposed EDHA had achieved better performance than the existing techniques in terms of Accuracy, Sensitivity, Specificity, Precision, F1-Score, MCC, AUC, and Computation time are 99.1%, 99%, 98.6%, 99.6%, 98.9%, 99.2%, 0.98, and 820 s using the CXR dataset.

Aims & objectives: To assess the functional outcomes of the unstable intertrochanteric fractures in the osteoporotic individuals treated by primary cemented hemiarthroplasty by early mobilization and immediate weight-bearing, restoration of limb length, and gait Assessment. Materials and methods: It is a prospective study of 17 elderly patients with comminuted intertrochanteric fractures managed with cemented hemiarthroplasty attending to the Department of Orthopaedics, Government General Hospital, Rangaraya medical college, Kakinada. Results: 17 cases of unstable intertrochanteric fractures were treated with cemented bipolar hemiarthroplasty. The average follow-up period was 12 months. The average age was 72.75 years with a standard deviation of 5.56. Most (82.3%) of the patients presented with Boyd and Griffin type 2 fracture. The functional assessment by HHS showed good to excellent outcomes in most of the patients (64.7%). 2 patients had abductor weakness, 1 patient had superficial surgical site infection, one had hypotension, and 1 patient had bedsore. Conclusions: Primary cemented bipolar prosthetic replacement in comminuted intertrochanteric fractures in the elderly patients provide s painless, stable, and mobile hip joint so that we can institute early postoperative mobilization which can avoid the consequences of prolonged recumbency. Hemiarthroplasty also avoids a second procedure in such elderly patients with multiple comorbidities as there is a high risk of failure of internal fixation in patients with poor bone quality, subjecting them to a second surgery and anaesthesia being a risky proposition.

Orbital complications of rhino-sinusitis was one of the most common extracranial complications in the pre-antibiotic era. However, the incidence of intra-orbital complications secondary to rhinosinusitis has significantly decreased in recent times with the advertent usage of broad-spectrum antibiotics. The subperiosteal abscess is one of the most common intraorbital complications of acute rhinosinusitis. This is a case report of a 14-year-old girl who presented with diminished vision and ophthalmoplegia which on evaluation was diagnosed to be a case of a subperiosteal abscess. The patient was treated by endoscopic sinus surgery and with complete post-operative recovery to normal vision and ocular movements. This report aims at describing the presentation and management of the condition.

The pharmacokinetics of enarodustat were elucidated in healthy subjects and in patients with end-stage renal disease (ESRD) on hemodialysis in phase 1 studies conducted in the United States and Japan. In healthy non-Japanese and Japanese subjects, following single oral administration up to 400 mg, enarodustat was rapidly absorbed. Maximum plasma concentration and area under the plasma concentration-time curve from the time of dosing to infinity were dose-dependent, renal excretion of unchanged enarodustat was substantial (on average ≈45% of dose), and mean t1/2 of <10 hours indicated negligible accumulation with once-daily dosing. In general, with daily dosing (25, 50 mg), accumulation at steady-state was ≈1.5-fold (t1/2(eff) ≈15 hours), presumably due to a decrease in renal drug excretion which is not clinically relevant in patients with ESRD. In the single- and multiple-dose studies, plasma clearance (CL/F) was lower in healthy Japanese subjects. In non-Japanese patients with ESRD on hemodialysis, following once-daily dosing (2-15 mg), enarodustat was rapidly absorbed, steady-state maximum plasma concentration and area under the plasma concentration-time curve during the dosing interval were dose-dependent, and interindividual variability in the exposure parameters was low-to-moderate (coefficient of variation, 27%-39%). Steady-state CL/F was similar across doses, renal drug excretion was not significant (<10% of dose), mean t1/2 and t1/2(eff) were similar (overall, 8.97-11.6 hours), and accumulation was minimal (≈20%), demonstrating predictable pharmacokinetics. Japanese patients with ESRD on hemodialysis (15 mg, single dose) exhibited similar pharmacokinetics with mean t1/2 of 11.3 hours and low interindividual variability in the exposure parameters, albeit with lower CL/F versus non-Japanese patients. Body weight-adjusted clearance values were generally similar in non-Japanese and Japanese healthy subjects and also in patients with ESRD on hemodialysis.

BACKGROUND: Traditionally, percutaneous transluminal angioplasty (PTA) is a first-line approach for stenosed dialysis accesses and has been performed through the non-thrombosed vein segment. For thrombosed accesses, thrombectomy (whether open or percutaneous) is a standard approach. The primary objective of our study is to determine the clinical and technical outcomes of the trans-radial approach of PTA among thrombosed dialysis accesses, in terms of safety and feasibility, technical and clinical aspects and factors influencing them, as well as assisted primary patency, secondary patency at 6 and 12 months. METHODS: This is a single-center retrospective study that included 150 patients over 3 years. About 123 patients underwent successful percutaneous balloon angioplasty through the radial access. RESULTS: We report an overall technical and clinical success rate of 82%, assisted primary patency rate of about 90.25% at 3 months, 82.93% at 6 months, 73.18% at 1 year, and secondary patency rate of 94% at 1 year. Twenty-seven patients were referred for surgical revisions/creation of a new fistula for reasons like inability to pass wire (6 patients), unfavorable anatomical variations like aneurysms at the proximal segments (5 patients), inability to cross the fistula (5 patients), and persistent fistula dysfunction with no flow after initial balloon dilatation (11 patients). Three patients had hematoma at the radial access site (2.5%) while two patients had the AV fistula segment rupture and were successfully treated conservatively. CONCLUSION: We conclude that PTA through the trans-radial approach to completely thrombosed hemodialysis accesses is a good alternative to transvenous access and has a very good assisted primary patency and secondary patency at 1 year without major complications.

Enarodustat (JTZ-951) is a hypoxia-inducible factor prolyl hydroxylase inhibitor that has been approved and marketed in Japan for patients with anemia with chronic kidney disease (CKD). The pharmacometric approach was applied to assess the relationship between plasma concentrations of enarodustat and hemoglobin (Hb) levels, and to provide information regarding the optimal use of enarodustat in clinical practice by simulations based on the pharmacokinetic and pharmacodynamic (PK/PD) model that was developed. The PK/PD data of enarodusat obtained from phase 2 and phase 3 studies in Japanese patients with CKD were well described by the models: a 1-compartment model with first-order absorption and elimination for PK, and a semimechanistic model based on transit compartment model for PD. Although several factors were identified as statistically significant covariates on the PK/PD of enarodustat, model-based simulations showed that none of them had clinically relevant impacts on the treatment effect (ie, Hb levels) of enarodustat. Hence, enarodustat treatment provides the stable Hb control with the initial dose (hemodialysis-dependent CKD: 4 mg/day, non-dialysis-dependent CKD: 2 mg/day) and maintenance dose (1-8 mg/day) to the patients with varied demographic characteristics.

BACKGROUND: Over the past years, polysaccharide-based scaffolds have emerged as the most promising material for tissue engineering. In the present study, carrageenan, an injectable scaffold has been used owing to its advantage and superior property. Cissus quadrangularis, a natural agent was incorporated into the carrageenan scaffold. Therefore, the present study aimed to assess the antioxidant activity and biocompatibility of this novel material. METHODS: The present in vitro study comprised of four study groups each constituting a sample of 15 with a total sample size of sixty (n = 60). The carrageenan hydrogel devoid of Cissus quadrangularis acted as the control group (Group-I). Based on the concentration of aqueous extract of Cissus quadrangularis (10% w/v, 20% w/v and 30% w/v) in carrageenan hydrogel, respective study groups namely II, III and IV were considered. Antioxidant activity was assessed using a 1,1-diphenyl-2-picrylhydrazyl radical scavenging assay, whereas the biocompatibility test was performed using a brine shrimp lethality assay. The microstructure and surface morphology of the hydrogel samples containing different concentrations of Cissus quadrangularis aqueous extract was investigated using SEM. One-way ANOVA with the post hoc tukey test was performed using SPSS software v22. RESULTS: A significant difference (P < 0.05) in the antioxidant activity was observed among the study groups. Group III reported the highest activity, whereas the control group showed the least antioxidant activity. Additionally, a significant (P < 0.01) drop in the antioxidant activity was observed in group IV when compared with group III. While assessing the biocompatibility, a significant (P < 0.001) dose-dependent increase in biocompatibility was observed with the increasing concentration of aqueous extract of Cissus quadrangularis. SEM analysis in group III showed even distribution throughout the hydrogel although the particles are close and densely arranged. Reduced antioxidant activity in group IV was probably due to clumping of the particles, thus reducing the active surface area. CONCLUSION: Keeping the limitations of in vitro study, it can be assumed that a carrageenan based injectable hydrogel scaffold incorporated with 20% w/v Cissus quadrangularis can provide a favourable micro-environment as it is biocompatible and possess better antioxidant property.

This study evaluated the effect of enarodustat on cardiac repolarization in healthy subjects. Enarodustat (20 and 150 mg [supratherapeutic dose]), placebo, and moxifloxacin (positive control, 400 mg) were administered orally to males and females (N = 54) in a crossover fashion. Continuous 12-lead Holter electrocardiogram (ECG) data were obtained before and after dosing, and blood samples were obtained for pharmacokinetic assessments of enarodustat, its circulating metabolite (R)-M2, and moxifloxacin. Central tendency analysis was performed for relevant ECG parameters, the relationship between individual-corrected interval from beginning of the QRS complex to end of the T wave in the frontal plane (QTcI, the primary end point) and plasma concentrations of enarodustat and (R)-M2 were assessed, and ECG waveforms were evaluated for morphological changes. The supratherapeutic dose resulted in 7- and 9-fold higher geometric mean maximum concentrations for enarodustat and (R)-M2, respectively, than the 20 mg dose. Based on time point analysis, the upper bound of the 2-sided 90% confidence interval (CI) for QTcI did not exceed 10 milliseconds at any of the time points for either dose. Based on QTcI-concentration analysis, the slopes for enarodustat and (R)-M2 were not statistically different than 0, and the upper bounds of the 2-sided 90% CI for QTcI at the geometric mean maximum concentrations for the supratherapeutic dose were 1.97 and 1.68 milliseconds for enarodustat and (R)-M2, respectively. The lower bound of the 2-sided 90% CI for moxifloxacin was ≥5 milliseconds, demonstrating assay sensitivity. The study demonstrated no clinically relevant effect of enarodustat and (R)-M2 on cardiac repolarization. There was no evidence of any clinically significant effect on the PR interval and QRS duration, and ECG waveforms showed no new clinically relevant morphological changes.

RESUMO

The dialysis clearance of enarodustat (JTZ-951) was determined in patients (N = 6) with end-stage renal disease on hemodialysis. Enarodustat (5 mg PO) was administered before (day 1) and after hemodialysis (day 8) with pharmacokinetic assessments on the 2 occasions. Dialysis clearance was based on plasma and dialysate enarodustat concentrations. Fraction of administered dose recovered in dialysate, total predialyzer and postdialyzer plasma enarodustat concentrations, and total and unbound venous plasma concentrations were determined. Hemodialysis did not significantly affect overall total concentrations with similar mean area under the plasma concentration-time curve from time 0 to infinity (coefficient of variation) of 3350 (26.4%) and 3640 (20.9%) ng · h/mL on days 1 and 8, respectively, and mean terminal half-life was 9.35 (11.9%) and 9.96 (18.7%) hours on the 2 occasions. Mean maximum concentration was somewhat lower on day 1 compared to day 8 (404 vs 559 ng/mL); the difference did not significantly affect total exposure (area under the plasma concentration-time curve from time 0 to infinity). Plasma protein binding was high (>99%) with similar binding on the 2 occasions, and total pre- and postdialyzer enarodustat concentrations were similar. Plasma unbound enarodustat concentrations decreased during dialysis, with a postdialysis rebound presumably due to re-equilibration with peripheral tissues. Mean unbound area under the plasma concentration-time curve from time 0 to infinity was marginally lower (∼22%) on day 1 compared to day 8. Dialysis clearance (0.415 L/h) was insignificant relative to dialyzer plasma flow (∼20 L/h), and the fraction of administered dose recovered in dialysate was small (6.74% of dose) with low intersubject variability (coefficient of variation, 14.7%). Thus, enarodustat can be administered regardless of dialysis schedule, and dose supplementation is not required in patients with end-stage renal disease on hemodialysis.

The paper describes the hydrography and vertical current structure along the shelf edge of South East Arabian Sea (SEAS) during summer and winter monsoons based on current profiles from moving Acoustic Doppler Current Profiler (ADCP). During summer monsoon, SEAS was characterized by upwelling with low saline water at the surface along the southern sector (8° N to 11° N). During winter, thermal structure was vertically homogeneous in the upper 80 m, and intrusion of low saline Bay of Bengal waters were found up to 14° N. In the southern sector, turbidity was more than the northern sector during winter and summer seasons. ADCP-derived current profiles during summer along 200-m isobath show dominant northward flow in the south, and southeasterly in the north as part of the West India Coastal Current (WICC). A comparison between ADCP current profiles and Ekman currents during summer indicates dominance of remote forcing (coastal Kelvin waves) over the local wind forcing in the 8-9° N sector whereas a combined influence of both remote forcing and wind in the 9-15° N sector. During winter, the direction of surface current reversed and was poleward generally except at the southern sector (7-8° N) where the flow was southwestward. Sector-wise comparison of ADCP and Ekman current showed less influence of wind on current fields throughout the sector except at south; wind has a major role in the current generation, whereas along the 8-15° N sector, the remote forcing dominates over the wind.

A new species of the genus Urocaridella Borradaile, 1915, Urocaridella arabianensis n. sp. is described and illustrated based on fifty-five specimens collected from the reef regions of Agatti Island, Lakshadweep, Arabian Sea. Urocaridella arabianensis n. sp. is morphologically close to U. pulchella Yokes Galil, 2006, reported from the Mediterranean Sea. However, the present specimen is clearly distinguished with its rostrum length, which is 1.7 to 2.1 times as long as carapace, 10 ventral rostral teeth, carpus of 1st pereopod is 2.5 times long as palm, carpus of 2nd pereopod is 1.8 times as long as palm, presence of a spine in latero-middle of the fourth and fifth abdomens and the sixth abdominal segment is 2.3 times long as the fifth segment. Partial segment of the mitochondrial COI, 16S rRNA and nuclear gene 18S rRNA and H3 indicates that the new species is sister to a clade comprising of U. pulchella, U. antonbruunii and U. degravei. The pair wise genetic distance of COI showed great distance with the closest congeneric species.
