[Peri-infarct zone and the effect of mannitol on it]. / Periinfarktnaia zona i vozdeistvie na nee mannitola.

Apart from clinical examination, blood enzyme activity was measured and ST intervals recorded on electrocardiotopograms (ECTG) from 35 leads in 56 patients (40 males and 16 females) with acute transmural anterior myocardial infarction. The osmotic diuretic mannitol was administered to 21 patients. The untreated patients were subdivided into 2 groups with respect to biochemical patterns they exhibited: enzyme activity peaking on Day 2 and coming back to normal by Day 7-10 (Group 1), and enzyme activity peaking on Day 3-4 and coming back to normal by Day 15-20 (Group 2). Similar patterns could be observed in ECTG findings. The course of myocardial infarction was shown to be either recurrent, or nonrecurrent. The recurrent course, indicative of a spreading primary necrotic focus, was noted in 86% of the patients belonging to Group 2. The clinical, enzymatic and ECTG patterns of patients treated with mannitol were similar to those of the patients with nonrecurrent course of myocardial infarction, therefore mannitol treatment can be recommended for the control of the acute stage of the disease.

The concentrations of cyclic adenosine monophosphate (cAMP) in the blood plasma collected on an empty stomach were determined by the radiochemical method in 40 males from 37 to 72 years of age suffering from ischemic heart disease. The cAMP blood plasma content was significantly higher in patients with coronary atherosclerosis than in 20 healthy young persons of the control group. In patients with a diabetic character of the glycemic curve under conditions of the oral standard test for glucose tolerance, the concentration of cAMP in blood plasma was significantly higher than that in patients with normal or doubtful glycemic curves.

[Diagnosis of cardiomyopathy]. / K diagnostike kardiomiopatii

The problems of diagnosis of cardiomyopathies are discussed. Among 13 patients who died in the clinic, a correct diagnosis was made in 10. The observations permit to believe that cardiomyopathy is seen more often in males aged 41 to 55 years. The course of the disease is chronic recurrent, its onset being different in individual cases: rhythm disorders, cardiac pains, recurrent pulmonary infarctions, cerebral circulation disorders. The outcome of the disease consisted, as a rule, in progressive total cardiac insufficiency refractory to therapy. In all the cases cardiomegaly was observed, not so much at the expense of hypertrophy, but rather of dilatation of the cardiac chambers. The anatomic signs included, apart from the cardiac dilatation, moderate hypertrophy with a relative bi- and tricuspid valve insufficiency, also mural thrombi in the heart chambers.
