[A model of inflammatory process in the abdominal cavity in experiment].

The model of chronic inflammatory process in abdominal cavity was elaborated in experiment on big laboratory animals. Intraoperatively in 12 laboratory pigs a chronic infiltrate was formatted between adducting and abducting intestinal loops with its mesenterium; in a region of gastroduodenal ligament and liver; between spleen and great omentum. Dynamics of the infiltrate formation was studied on the 60th, 90th, 120th and 180th day in accordance to data of bacteriological and histological investigations. Application of the method proposed permits to create chronic infiltrate with prognosticated course, to reduce the experiment duration and quantity of experimental animals used.

[Transperitoneal and transvaginal hysterectomy using devices of the dosed high-frequency electrical current].

The experience of application of apparatuses of the dosed high-frequency electric influence EK300M-1 "Patonmed" and energetic platform "Forcetriad", manufactured by Valleylab firm, during hysterectomy performance, using various surgical accesses, was presented. The variants of the applied regimens, depending on the vessels diameter and coexistant diseases, were proposed.

[Practical application of a high-frequency coagulators with different electrophysical characteristics].

The results of experimental-clinical investigation on studying of technical characteristics and practical application of various types of electrocoagulators were adduced. Comparative analysis of possibility to perform the main stages of surgical interventions, using well-known apparatuses of various manufacturers, was conducted.

[Effect of using dosated ligating apparatus LigaSure during operative interventions on the abdominal organs].

There was summarized the experience of application of energetic platform, named "Force triad", manufactured by Valleylab firm, which secures electric vascular ligating regime "LigaSure" of second generation, while performing operative interventions on abdominal organs. Variants of the application regimes, depending on the vessels diameter and concomitant diseases presence, were summarized.

[Clinical introduction of the method of the abdominal cavity infiltrate dissection with saving of anatomic integrity and functional capacity of organs, included in the inflammatory process].

The results of application of the tissue dissection method and hemostasis performance while operation for the abdominal organs acute diseases were summarized. The proposed method introduction have permitted to reduce the intraoperative complications rate from 66.6%--in a control group to 12.2%--in the main group; postoperative complications--from 21.2 to 1.7%, postoperative lethality--from 7.4 to 1.7%.

[Investigation on the impact of hot temperature methods of the tissue dissection and coagulation on parenchymatous organs in experiment].

There are adduced the results of experimental operations on mongrel rabbits with dissection and coagulation of the liver and the spleen, using highly temperature coagulation apparatuses of various kinds. There was established, that while application of various highly temperature technologies a typical process occurs, consisting of the heat spreading inside the organ. The temperature raising grade depends on the method and duration of the impact.

[Classification of a high temperature methods for tissue dissection and achievement of hemostasis].

The results of experimental-clinical investigation, using apparatuses of a highly-temperature dissection and coagulation of a various type of action, were adduced. Classification of a highly-temperature dissection and coagulation in accordance to type of action and kind of the energy used was elaborated, basing on analysis of results of experimental operations and clinical application of apparatuses of various manufactures.

[Mobilization of the stomach and colon using high-frequency electric welding of tissues apparatus].

The results of experimental investigations were adduced, concerning studying of possibilities and the outcomes analysis of the ultrasound and a liquid-stream dissector application while operating on parenchymatous organs. There was established, that while operating on liver, there was not any essential difference between application of the ultrasound and stream of a liquid concerning the achievement of a quality of the stroma elements isolation; while performing operations on the spleen a liquid-stream dissector have had evolved as a more effective one, permitting to isolate a significantly more quantity of unaffected elements of stroma. The hemostasis methods are the main, which influence the organs stump formation and the typical cellular structure restoration, and they must be applied, using the both methods of dissection.

[Creation of models of incomplete external lip-like large bowel fistula in experiment].

The method of experimental simulation of the incomplete external lip-like large bowel fistula creation was elaborated on experimental animals for studying of possibilities forapplication of hyperthermic methods of the fistula channel obliteration and disinfectioning of the neighboring anterior abdominal wall tissues. There were studied up microflora and bacterial seeding of the large bowel contents in the zone of a fistula exit as well as dynamics of changes of inflammatory processes, occurring in tissues of anterior abdominal wall, neighboring the external orifice of a fistula channel.

[Experimental investigations of coagulation possibilities of noncoherent optico-electronic systems in operations on parenchymatous organs].

Results of experimental investigation on elaboration and trial of surgical hemostasis method, using the quartz heating optic noncoherent irradiators of light, are adduced. Basing on analysis of the results obtained, the apparatus for photooptic coagulation was elaborated and trialed on experimental animals.

[Clinical application of harmonic scalpels of various type in laparoscopic surgery].

The experience of clinical application of harmonic scalpels of various type while performing laparoscopic interventions was summarized. The advantages and disadvantages of instruments of various manufactures were estimated. The procedure of ultrasonic scissors application, depending on clinical situation present, was adduced.

[Consequences of influence of a high-pressure liquid stream on a hollow organ wall].

There are adduced the results of histological investigation of preparations, obtained in experiment intraoperatively on a mongrel rabbits, with the objective to study up the consequences of influence of a high-pressure liquid stream towards a wall of a hollow organ. There was established, that in application of a stream, measured 0.1 mm in diameter under pressure of 30 atmospheres a pathological changes in the organ do not occur and the reparative processes course is typical.

[A model of intestinal infiltrate in conditions of chronic experiment].

A chronic intestinal infiltrate model, aiming at a typical inflammatory process simulation, was elaborated in experiment. As a result of experiment on 30 rabbits the quantity of colonies of various causative agents was established, which did not cause the abscess and peritonitis formation. While doing the operation in 60 rabbits chronic infiltrate was formed between abducting and adducting small intestine loops, its mesentery and big omentum; between abducting and adducting large intestine loops, its mesentery and big omentum. The infiltrate formation process was studied up on the 3, 7, 14th and 28th day. Dynamics of changes in microorganisms species and quantity were studied, using microbiological investigations data. The stages of chronic intestinal infiltrate formation were followed up, using histological investigations. Application of the method proposed have permitted to form a chronic infiltrate owing predicted course to reduce the experiment duration and the investigated animals quantity.

[Surgical treatment of complete unformed high complicated fistula of the small intestine].

Experience of treatment of 9 patients, suffering complete high not formed complicated small bowel fistula, is adduced. In all the patients the fistula have formed on eventrated jejunal loops. Results of conservative therapy of high small bowel fistula were analyzed. A new approach to conduction of surgical interventions for external intestinal fistula, using liquor-jet dissector was proposed. The results of treatment witness perspectives of the method proposed.

[Possibilities of rational choice and application of autotissues for hermetization of the suture connections in biliary surgery].

The results of experimental investigation of peculiarities of morphological transformation course in autotissues under the influence of bile were adduced. There was proposed a choice of peritoneum as an optimal plastic material in conditions of possible deficiency of local tissues during biliary surgery performance.
