RESUMO
A simple and highly accurate method for estimating the retraction of a blood clot (RBC) and spontaneous fibrinolysis is proposed for assessing the final stage of blood clotting. The method helps evaluate the RBC over time. It was used to study the effects of different infusion media on the final stage of blood clotting in more than 300 laboratory experiments and 50 clinical studies. Different effects of infusion media on the formation of a blood clot were revealed, which should be not disregarded in clinical practice.

Under study was the therapeutic effect of the infusion-transfusion maintenance of great and utmost great resections of the liver (154 operations) based on hemodilution and hydration with infusion-forced diuresis. The use of an original infusion-transfusion therapy proved to be the most important factor for safety of liver resection, for less amount of postoperative complications and lethality (up to 7.4%), gave a stable course of the postoperative period.

The authors have proved the possibility of using the hemiglobincyanide method for measurements of blood plasma free hemoglobin: free hemoglobin was measured in autoblood reinfused from the operation field by various methods (45-benzidine method, 3 parallel analyses in every study, and 45-hemiglobincyanide method, modified by increasing the amount of the tested material from 0.02 to 2.0 ml, also 3 parallel analyses per sample); a correction coefficient equal to 71.7 is suggested, and correction of the data according to a formula.

A method of evaluation of the operation-anesthesia risk in great and extremely great resections of the liver with special reference to specific features of the concrete operation is described. The method makes it possible to objectively compare the effectiveness of the therapy and work of anesthesiological, resuscitation and surgical departments.

The authors have developed a method of autohemotransfusion under conditions of hemodilution with stabilized reserved blood by the sorbent method which allows preparation of great (up to 50% of VCB) volumes of autoblood with preserving reserves of circulating erythrocytes at the expense of transfusion of suspension of cryopreserved erythrocytes during exfusion of blood. The use of special nomograms for prognosis of hematocrit changes in exfusion of blood and transfusion of the erythrosuspension is described and for quick orientation in intensity of diuresis. The method of autohemotransfusion was used in 11 surgical interventions on the liver.

Effects of complex therapy were examined in 76 patients with anastomositis following operations on the stomach. The intragastric administration of long chain alcohols was shown to improve the evacuation of food masses from the stomach. The volume of the congested gastric content was reduced from 1500 to 300 ml 6-7 days after the beginning of the treatment.

The levels of T- and B-lymphocytes were assayed and their functional activity studied in 23 patients with primary cancer and 21 cases of alveococcosis of liver. The lowered levels of T-lymphocytes in both groups and the decreased C3-fraction of B-lymphocytes in primary cancer of the liver were observed. Considerable changes in serum immunoglobulin system, i. e. an increased level of all immunoglobulins (G, A and M), are reported.
