RESUMO

Our study is aimed to confirm the advantage of simultaneous operations during combined gynaecological and surgical pathologies. During the 3 years in medical practice of of Otar Gudushauri National Medical Center 71 operations were conducted on patients genitalia and their other organs simultaneously at its Perinatal Department. The age of patients ranged from 19 to 57. All the necessary clinical-laboratory examinations were carried out within the pre-surgical preparation period. The preference was given to the routine planned operations; however urgent surgical interventions were performed in cases of emergency. None of the operations were followed by complications due to the increased scope of procedures. High efficacy of these operations, prevention of possible complications in the post--operational period, due to combined pathologies, high economical effect, reduce of stress situations, provide us with the grounds to conduct operations of this nature in a multiprofile clinic by highly qualified personnel.

RESUMO

Nine births affected by Down's disease in young families are reported. The mean age of fathers was 28.5 years and that of mothers 25.2 years. History revealed no parental exposures to occupational or household adverse factors or nutritional contaminants. The parents had normal karyotypes. However, a history of urogenital disease 1-3 years before the affected births was elucidated in all 9 families: three cases of gonorrhea, one case of trichomoniasis and nonspecific inflammation in the rest. After visits to genetic counseling clinics torpid urogenital infections were identified in all 9 couples: nonspecific prostatitis and urethroprostatitis in fathers, hysterosalpingoophoritis and vaginal dysbacteriosis in mothers. Teratozoospermia and oligozoospermia with abnormal sperm cell proportions of 56 to 92% occurred in the fathers. It is suggested that infectious-toxic disorders of chromosomal segregation during the meiosis may be a paternal cause of trisomy 21. The same mechanism is feasible in mothers. After reversal of urogenital infections and normalization of spermograms normal infants were born in 7 of 9 families, the other 2 currently refrain from childbirth.

RESUMO

The results of experimental pharmacological studies may help obstetricians to make a choice of a schedule of using the uterotropic drugs and their doses. In vivo and in vitro experiments on pregnant rats showed that administration of the Soviet-made prostaglandin E2--prostenon--in combination with oxytocin and obsidan in half doses increased its stimulating effect on the myometrium. The use of prostenon and oxytocin against the background of partusisten treatment was not followed by a significant change of their stimulating uterotropic action.

Experiments on pregnant rats revealed that a combination of low doses of partusisten and clophelin (1.25 mg/kg and 0.15 ml of 0.001% solution, respectively) potentiated a tocolytic effect. A combination of prostenon and oxytocin given in half doses (0.15 mg/kg and 0.25 IU in 0.1 ml, respectively) summarized their stimulant effect on the myometrium. Premedication with partusisten (2.5 micrograms/kg) failed to change the stimulant effect produced by a combination of prostenon and oxytocin given in half doses.
