RESUMO

BACKGROUND/AIMS: Currently, screening colonscopy is widely performed in the medical field. The initial time of screening is recommended at an age of 50 years, but the age to cease screening is unknown. Accordingly, we have investigated the diagnostic yield of colonoscopy according to indications in the elderly, and we evaluated if screening colonoscopy is useful in the elderly. METHODS: We recruited asymptomatic individuals undergoing screening colonoscopy according to age (2830 subjects aged 50-74 years and 111 subjects > or =75 years-old), The colonoscopy findings of the study subjects were compared. In addition, colonoscopy findings of asymptomatic subjects more than 75 years-old were compared with the findings of symptomatic subjects with the same age. RESULTS: The yield for overall neoplasia and advanced adenoma was higher in the group of subjects > or =75 years-old than in the 50-74 years age group (overall adenoma: 49.54% versus 24.98%, p or =75 years-old than in symptomatic subjects > or =75 years-old (49.54% versus 28.19%, p or =75 years-old. The lack of a decline in the frequency of adenoma, including advanced adenoma, justifies continuing screening colonoscopy in the elderly.

RESUMO

Primary liposarcomas of the pleura are extremely rare soft tissue sarcomas, for which radiographic findings have been reported in only a few cases. Although liposarcomas occur most commonly in the lower extremities and retroperitoneum, we encountered a case of pleural liposarcoma arising in the right hemithorax, invading the medial basal segment of the right lower lobe. Thus, herein we report a case of primary pleural liposarcoma, along with a review of the literature.

RESUMO

This study was carried out to evaluate outcome of treatment of acquired nasolacrimal duct obstruction complicated after acute conjunctivitis in children.The study included 16 eyes in 16 patients from March 1997 to June 1999.Probing was only successful in 2 cases.Silicone tube intubation was done in 14 cases at 16.4 months after acute conjunctivitis.The results were compared with 10 eyes of 12 patients who had undergone silicone intubation for congenital nasolacrimal duct obstruction.At two weeks after operation, congenital group showed better outcome compared to acquired group (Mann-Whitney test, p<0.05).But, at 8, 16 weeks and final follow-up, there was no statistically significant difference.In our case of acquired nasolacrimal duct obstruction complicated after acute conjunctivitis in children, silicone intubation was more needed than probing and it was worth trying because final outcomes of both groups were not significantly different in spite of slow improvement.

RESUMO

The recent hypothesis about the pathophysiology of schizophrenia has been centered mainly on two theories, i.e. dopamine hypothesis and serotonin hypothesis. We investigate the correlations between plasma metabolite concentrations and clinical symptoms in schizophrenic patients. The purpose of our study was to examine whether the plasma levels of HVA(homovanillic acid) and 5-HIAA(hydroxyindoleacetic acid) are significantly different in schizophrenics, compared to normal controls. And, with the intention of clarifying the interaction between dopaminergic system and serotoninergic system, the ratio of HVA/5-HIAA also was measured. The second purpose was whether the basal(pre-treatment) levels of these metabolites show the correlation with clinical symptoms. Finally, third purpose was whether basal HVA and 5-HIAA levels can be held as a predictor of treatment response. We used scale for the Assessment of Positive Symptoms(SAPS) and Scale for the Assessment of Negative Symptoms(SANS) as the clinical symptom rating scales. Our results were as followed, 1) only the level of basal plasma HVA was significantly differ in schizophrenics. 5-HIAA and HVA/5-HIAA were not. 2) basal HVA showed significant correlation with SAPS score, especially delusion subscale. 3) the higher was the basal HVA level, the more improvement in clinical symptoms was observed. The basal 5-HIAA level and the HVA/5-HIAA ratio did not show any significant findings. These results support the dopamine hypothesis of schizophrenia, but fail to examine on the possible involvement of serotonin in schizophrenia.
