RESUMO

The article presents the results of low-intensity laser application in complex treatment of 137 children with acute purulent destructive pneumonia complicated by pneumothorax with bronchial fistulas. A method of intracavitary laser therapy, developed in the clinic, allowed obliteration of bronchopleural fistulas without application of bronchial occlusion and other invasive techniques. Evaluation of the kallikrein-kinin system of blood revealed prominent reduction of kininogenesis in most (87%) patients upon admission (3 weeks after the onset of the disease), which is an important link of the pathogenesis of late stages of complicated acute purulent lung destruction in children. The study also demonstrated that low-intensity laser emission modulates pyoinflammatory process due to its effect on cell-mediated immunity, neutrophilic phagocytosis and the kallikrein-kinin system of blood. Intracavitary laser therapy is the treatment of choice in children with acute purulent destructive pneumonia complicated by pneumothorax with bronchial fistulas. Application of intracavitary laser therapy in complex therapy of complicated acute purulent lung destruction in children allowed discharge from the hospital 5 to 7 day earlier, and prevented lung inflammatory process chronization. None of the patients have died within last 10 years.

Based on a study of the kininogenase activity of the total plasma kallikrein in the presence of 3 concentrations of the soybean inhibitor trypsin (0.5, 1.0, 10.0 micrograms/ml) one can measure at a time the activity of tissue kallikrein (without specifying the source) and the activity of 3 forms of plasma kallikrein, including its adsorption on kaolin that characterizes the conformational structure of the enzyme. Examination of 10 healthy subjects and 136 patients revealed a 10 to 20-fold increase in the content of tissue kallikrein in plasma of 70% of diabetes mellitus patients and a 2.5 to 3-fold elevation in 50% of patients with chronic occupational bronchitis, and in 30% of patients suffering from chronic hepatitis. The method suggested makes it possible to have a better insight into the physiological and pathogenetic role of the kinin system and may be used for laboratory control over the treatment efficacy.

The method is infusing an isolated jejunal strip with heparinized blood (3-5 units/ml) was used to determine total free kinins in peripheral blood of man and animals to a high precision (+/- 6.9%). Kinin formation was inhibited by trypsin inhibitor obtained from soybean (100-200 microgram/ml). Destruction of kinin was blocked by 8-hydroxyquinoline (0.03-0.05 ml of saturated solution per 1 ml of blood) or by unithiol (2,3-dimercaptopropanol-2 mg/ml). Since these inhibitors can induce hypesthesia of the jejunum to bradykinin (8-hydroxyquinoline in 47% of cats) or to potentiate contractility of the jejunal strip (unithiol in 50% of cats) it is advisable that appropriate corrections is made in order to restrict the number of tests, to raise the interval between the tests and to carry out additional analysis of jejunal sensitivity to bradykinin in the presence of 8-hydroxyquinoline or unithiol.
