An analysis of the data available on the reproductive status and social adjustment of 443 children who received three modalities of complex treatment for Hodgkin's disease in 1976-1994 established no impairment of reproductive function or its formation, although full therapeutic effect had been achieved. Nor were their intelligence or professional and social function affected in any way.

The high efficiency of treatment of lymphogranulomatosis in children by the currently available protocols determines the necessity of assessing them in terms of the quality of further life. The opportunity of creating a life and having the offspring, that of getting education and a profession are one of the main characteristics of the quality of life. The data on the reproductive function and social adaptation of 443 children receiving multimodality treatment for lymphogranulomatosis by 3 protocols (from 1976, 1978, and 1987). Two hundred and fifty eight individuals were 18 years old or more. Sixty four former patients have 88 children who are all healthy. The treatment performed in childhood intensively, which yielded a complete effect, exerts no substantial impact on the formation and performance of their reproductive function. The former patients have a high intellect, without limitations in getting professions, and they are easily adaptable to the changing social conditions.

[State of the reproductive function in women who had lymphogranulomatosis in childhood and adolescence]. / Sostoianie reproduktivnoi funktsii u zhenshchin, perenesshikh limfogranulematoz v detskom i molodnom vozraste.

As many as 33 women who received combined treatment (fully successful) at an age of 5-26 years for lymphogranulomatosis were examined. In 30 patients, the structure and function of the reproductive organs was within age norm. In 20 women, there were 20 deliveries at an age of 19-33 years, with the mean age amounting to 23 years; 10 boys and 10 girls were born. The children are healthy. The selective toxicity of the treatment as a whole and of each of its components with regard to the reproductive organs was not proved. In the authors' opinion, two cases of disease relapses in pregnant women do not form basis for recommending the woman do refrain from pregnancy in the long-term periods after the treatment of lymphogranulomatosis.

[Clinico-morphological characteristics of lymphogranulomatosis in young children]. / Kliniko-morfologicheskie osobennosti limfogranulematoza u detei rannego vozrasta.

Cases of lymphogranulomatosis underwent a clinico-morphological analysis in 67 children aged 1 year to 3 years 11 months. The morphological features of the process and distinctive traits of the clinical picture of the disease in infants were defined. The 5-year survival in patients undergoing the current treatment programs was estimated.

[Treatment of lymphogranulomatosis in children]. / Lechenie limfogranulematoza u detei.

The authors analyze the results of the treatment of 366 children afflicted with lymphogranulomatosis. All the patients received treatment from 1976 to 1990 according to three combined programs. The necessity is demonstrated of the individual treatment with regard to the predictors. Immunocorrection by means of taktivin was employed in patients with immunity disorders. The perfection of a complex of treatment measures favoured an increase of the survival to over 5 years in 93.8% of cases.

[Pathogenesis of immunologic deficiency in the initial stage of lymphogranulomatosis in children]. / Patogenez immunodefitsita v nachal'noi stadii limfogranulematoza u detei.

Antilymphocytic monoclonal antibodies of HKO and OKT series were used to study surface antigens of peripheral blood and lymph node lymphocytes in 13 untreated patients with Hodgkin's disease. The purpose of the study was to identify the pathogenetic mechanism underlying the development of immunodeficiency in the early disease stages. The data obtained have demonstrated that the deficiency of T lymphocytes in the peripheral blood was connected with that of mature T cells in the lymph nodes revealed with the use of differential antigens complex. Deficiency of immunologically differentiated T lymphocytes in the lymph nodes combined with high counts of the cells whose membranes carry Ia-like antigen point to the immunologically unfavourable course of the disease.

[Evaluation of 5-year results of the scheduled treatment of lymphogranulomatosis in children]. / Otsenka 5-letnikh rezul'tatov programmnogo lecheniia limfogranulematoza u detei.

A total of 186 children and adolescents aged 2.5 to 16 years with Hodgkin's disease were placed under observation. According to the accepted classification (Ann Harbour, 1971) they were divided into two groups. Exploration laparatomy was applied in the first group of the patients (n-103) as the final diagnostic stage in order to verify the pathomorphological stage of the process. In the second group (n-83) the clinical stage of the disease was only appraised. The pathological foci beneath the diaphragm were discovered, however, with the aid of ultrasonic tomography of the abdominal cavity, scanning with gallium-67 citrate, and computer-aided tomography. The latter technique was employed in case of controversies in the interpretation of the foci of lesion with the aid of the first two diagnostic methods. It is concluded that in view of the high tempo of the so-called silent generalization and dissemination of Hodgkin's disease in children, this condition should be regarded in childhood as a potentially systemic process. With such an attitude towards Hodgkin's disease in children it is advisable to abandon exploration laparotomy with splenectomy depriving the patient of the massive depot of humoral (B cell) immunity and creating the danger of an aggressive course of intercurrent infections in splenectomized children affected with Hodgkin's disease. In the choice of the initial treatment which provides the basis for obtaining the highest effect, preference should be given to polychemotherapy. It is indicated in the presence of "biological activity" of the process, general symptoms, generalization and dissemination of the process (stages III and IV), and in the presence of multiple foci in stage II.(ABSTRACT TRUNCATED AT 250 WORDS)

[Immunocytological characteristics of primary lesions of the lymph nodes in children with lymphogranulomatosis]. / Immunotsitologicheskie osobennosti pervichno porazhennykh limfaticheskikh uzlov u detei s limfogranulematozom.

Antilymphocytic monoclonal antibodies of UKO and OKT series were used to study surface lymphocytic antigens in 40 primarily affected lymphatic nodes in 19 children with lymphogranulomatosis. In 17 lymphatic nodes immunological reaction was determined as the T-type (OKT3+--42.3 +/- 2.73%); and in 23--the T-cell population depletion was revealed (OKT3+--15.1 +/- 1.37%, P less than 0.001). It was shown that in the course of the disease the depletion of T-cell lymphocytic population comes more rapidly than that of B-cell. It is accompanied by an increase in the number of lymphocytes carring Ia-like antigens on the membrane (UKO-1+).

[Study of the antitumor activity of carminomycin in malignant tumors in children (a preliminary report)]. / Izuchenie protivoopukholevoi aktivnosti karminomitsina pri zlokachestvennykh opukholiakh u detei (predvaritel'noe soobshchenie)

Side reactions and therapeutic effect of carminomycin were studied in 21 children with various malignant tumors. Carminomycin was administered intravenously in doses of 0.15-0.2 mg/kg twice a week for 2-3 weeks. The treatment courses were repeated in a month. The drug was better tolerated by children than by adults. After the treatment course leucopenia may develop. When used alone or in combination with vincristin or cyclophosphan carminomycin proved to be effective in treatment of sarcoma of the soft tissues (liposarcoma, synovial sarcoma), chondrosarcoma and reticulosarcoma.
